Damage Assessment of Retrieved Birmingham Monoblock Cups: Is Conversion to Dual-Mobility Head a Viable Revision Option?

BACKGROUND: A novel revision technique for failed hip resurfacings involves retention of the acetabular cup, if well-fixed, which is mated to dual-mobility (DM) prosthesis in a traditional hip replacement configuration. It is unknown whether existing damage on the retained cup will result in unacceptable wear of the DM prosthesis. METHODS: Thirty retrieved Birmingham (Smith & Nephew) monoblock cups were visually scored for damage features and area of coverage. Surface roughness measurements were obtained within each damage feature as well as reference points on each cup. Analysis of prior met

Systemic review of published literature on Candida infection following total hip arthroplasty.

Candida species (Candida spp.) infection following total hip arthroplasty (THA) is a rare but potentially devastating complication. The purpose of this study was to provide an evidence-based summarization of the treatment and outcome of Candida spp. infection after THA. We conducted a systemic review of the literature and pooled analysis of Candida spp. infection after THA published between 1950 and 2014. A total of 20 articles with 37 patients were identified. Median time between THA and readmission for Candida spp. infection was 25 months (range 0.5-184 months). Thirty-two patients (87%) und

Flabby ridge correction using an auxiliary technique with connective tissue graft.

A common finding in patients with edentulous maxilla and partially dentate mandible is mainly the presence of flabbiness in the anterior edentulous alveolar ridge that can compromise the retention and stability of a denture. Thus, this case report presents the correction of a flabby ridge, using an auxiliary technique combining surgical excision and autogenous connective tissue grafting. The technique improved the quality of the osteomucosal support of the alveolar ridge and increased the vestibule deepness, whose result increased the success rate of the new conventional total prosthesis.

[Case of the maxillofacial defect patient. Prosthetic treatment after reconstruction of the palate and face using deltopectral flap].

The patient, a 72-year-old edentulous man, had an extensive maxillofacial defect caused by treatment of the maxillary cartinoma. As his left maxillary and zygomatic bone, a portion of the soft palate, orbital cavity contents and soft tissues of the cheek had been excised, his face was ugly, and his speech and eating were extremely poor. For this patient, first of all, plastic surgeons reconstructed the cheek and palate using deltopectral flap. Secondly, dentists prepared a facial prosthesis and a set of complete dentures. Four months after prosthetic treatment, plastic surgeons corrected the l

Research progress on debridement, antibiotics, and implant retention (DAIR) for the treatment of periprosthetic joint infection after artificial joint replacement.

Periprosthetic joint infection (PJI) is one of the most serious complications following artificial joint replacement, posing significant treatment challenges and a heavy clinical burden. Debridement, antibiotics, and implant retention (DAIR) has become the preferred treatment strategy for acute PJI due to its advantages of minimal invasiveness, functional preservation, and cost-effectiveness. This article reviews the latest advances in optimizing the indications, timing of intervention, key technical details, and multimodal anti-infection strategies for DAIR. It focuses on discussing patient s

Influence of Skeletal and Local Bone Density on Dental Implant Stability in Patients with Osteoporosis.

BACKGROUND AND PURPOSE: Osteoporosis is a major skeletal disease affecting millions of people worldwide. Recent studies claim that patients with osteoporosis do not have a higher risk of early implant failure compared to non-osteoporotic patients. The aim of this study was to assess the effect of skeletal osteoporosis and local bone density on initial dental implant stability. MATERIALS AND METHODS: Seventy-three patients were recruited and were assigned (based on a Dual-energy X-ray Absorptiometry scan) to either the osteoporosis (Opr), osteopenia (Opn), or control (C) group. Forty nine of t

Fully digital workflow for reinforced mandibular implant overdenture - A novel method.

Mandibular overdenture is suspected to fracture, especially in the midline and anterior region due to low surface area of coverage and minimum acrylic thickness in addition to attachments pickup holes that weakens the denture. Dentures used to be conventionally reinforced with metal meshwork which cannot be done in a digital workflow. This in vitro report introduces a novel approach of digital overdenture reinforcement using computer-aided design, computer-aided manufacturing, and rapid prototyping technologies. This novel approach provided digital reinforced, stable, and well-adapted overdent

Self-aligning prosthetic device for older patients with vascular-related amputations: protocol for a randomised feasibility study (the STEPFORWARD study).

INTRODUCTION: The majority of older patients with a transtibial amputation are prescribed a standard (more rigid, not self-aligning) prosthesis. These are mostly suitable for level walking, and cannot adjust to different sloped surfaces. This makes walking more difficult and less energy efficient, possibly leading to longer term disuse. A Cochrane Review concluded that there was insufficient evidence to recommend any individual type of prosthetic ankle-foot mechanism. This trial will establish the feasibility of conducting a large-scale trial to assess the effectiveness and cost-effectiveness

Full mouth rehabilitation of a patient with mandibular implant screw retained Fp-3 prosthesis opposing maxillary acrylic removable over-denture.

A hybrid denture is one that is fabricated over a metal framework and retained by screws threaded into the implant abutments. The anterior part of a mandibular hybrid denture is fixed on implants while the posterior part of the denture is extended and cantilevered from implants. This article presents the fabrication of a maxillary over-denture opposing mandibular implant retained hybrid prosthesis. A total of four implants were placed in the mandibular arch. Castable abutments were used to produce the optimal angulations. Framework was waxed, cast recovered, and the fit was refined until the f

Prosthodontic Rehabilitation of an Ectodermal Dysplasia Patient with Implant Telescopic Crown Attachments.

Telescopic crown attachments have been successfully used in completely and partially edentulous patients. This type of attachment provides retention, support, and stability with optimal hygiene for the removable dental prosthesis (RDP). With the emergence of CAD/CAM technology, telescopic crown attachments can be virtually designed and milled precisely to ensure a passive fit of the attachment parts and maximal functionality of the RDP. This article reviews both the clinical outcome and prosthodontic rehabilitation of telescopic crown attachments of an edentulous ectodermal dysplasia patient w

Fibre-endoscopic insertion of oesophageal tube for palliation in malignant oesophagogastric obstruction.

In 28 patients with malignant obstruction of the oesophagus or cardia, an intraluminal prosthesis was endoscopically inserted as palliation, to permit food intake. Of the 25 who survived the procedure, 23 were able to swallow satisfactorily and required no supporting parenteral nutrition. Complications arose in 13 patients (46%), however. Four of the tubes became obstructed and two were displaced during the follow-up period, and there were two cases of late massive bleeding from mucosal erosions. Perintubation perforation of the oesophagus occurred in two patients. One patient had immediate po

Pterygoid implants for maxillofacial rehabilitation of a patient with a bilateral maxillectomy defect.

Bilateral maxillectomy is known to have serious esthetic and functional consequences. The retention and support of a maxillary obturator prosthesis in these patients is particularly challenging. Surgical placement of implants is also challenging because of the lack of available bone. Therefore, implant placement into remote sites such as zygoma has been advocated. Very few articles in the literature have discussed the use of pterygoid/pterygomaxillary implants in patients undergoing maxillectomy. This case report describes the maxillofacial rehabilitation of an elderly man who underwent a bila

Prosthetic rehabilitation of an extensive midfacial and palatal postsurgical defect with an implant-supported cross arch framework: a clinical report.

When aggressive cancer is surgically treated, the extent of the lesion dictates the amount of tissue excised. The removal of extensive amounts of tissue, including crucial landmarks, complicates prosthesis design. This clinical report details the rehabilitation of a patient who had a total maxillectomy coupled with extensive surgical removal of the midface. With the aid of computer-generated implant placement, a large computer-aided design and computer-aided manufacturing milled titanium framework, a Hader bar, and magnetic keepers, a successful implant retained definitive obturator and facial
