Concomitant Sternal Fractures: Harbinger of Worse Pulmonary Complications and Mortality in Patients With Rib Fractures.

BACKGROUND: Sternal and rib fractures are common concomitant injuries. However, the impact of concurrent sternal fractures on clinical outcomes of patients with rib fractures is unclear. We aimed to unveil the pulmonary morbidity and mortality impact of concomitant sternal fractures among patients with rib fractures. METHODS: We identified adult patients admitted with traumatic rib fractures with vs. without concomitant sternal fractures using the 2012-2014 National Inpatient Sample (NIS). After 2:1 propensity score matching and adjustment for residual imbalances, we compared risk of pulmonar

Using electromagnetic navigation for intraoperative rib fracture localization during rib plating: A case report.

INTRODUCTION: The precise localization of fractured ribs represents one of the primary challenges of surgical rib fixation. Computed tomography (CT) provides the facture's general location, but it is difficult to use the imaging alone to properly place a surgical incision. We used electromagnetic navigation to identify the exact location of the fracture on the patient's skin. PRESENTATION OF CASE: A 64-year-old man fell and suffered multiple left-sided nonunion rib fractures (4th to 9th). He was initially treated with a chest tube and analgesia, but he developed chronic pain from the injury.

Radiation-induced skin ulcer and rib fractures following percutaneous coronary intervention (PCI): A case of right back skin ulcer and adjacent rib fractures after single PCI.

We experienced a 75-year-old male patient with a refractory and severely painful skin ulcer on the right back. He had suffered from ischemic heart disease and undergone percutaneous coronary intervention 5 months prior to the consultation with us. The characteristic clinical appearance, location of the lesion and his past medical history led us to the diagnosis of radiation-induced skin ulcer. Magnetic resonance imaging, computed tomography as well as bone scintigraphy showed fractures of the right back rib adjacent to the ulcer, which was thought to be attributable to bone damage due to X-ray

Comparison of flail and multiple simple rib fracture patterns among patients included in rib fracture studies; A systematic review and meta-analysis.

PURPOSE: Comparing baseline characteristics of patients with flail versus multiple simple rib fracture patterns, hypothesizing greater injury severity in the flail group. METHODS: A systematic review of Cochrane Central, Embase, Medline Ovid, and Web of Science identified 66 cohorts from 1,799 records. Random effects models were used, with publication bias assessed via funnel plots. Primary outcome was injury severity score (ISS); secondary outcomes included number and level of fractured ribs and associated injuries. RESULTS: Patients with flail fractures had significantly higher ISS (mean 27

Traumatic arterial thoracic outlet syndrome after multiple rib fractures not including the first rib in Korea: a case report.

Arterial thoracic outlet syndrome (TOS) resulting from thoracic trauma is an exceedingly rare condition, typically caused by a fracture of the first rib or clavicle. In this report, the author presents a case of traumatic arterial TOS precipitated by multiple left rib fractures, notably excluding the first rib, following a fall from a 2-m high stepladder. The patient was treated successfully with first rib resection via a transaxillary approach, and the postoperative course was uneventful. The literature includes no known reports of traumatic arterial TOS in patients with multiple fractures th

Delayed life-threatening aortic rupture after rib fractures: a case report.

BACKGROUND: Aortic injury secondary to rib fractures is an extremely rare but life-threatening complication of blunt thoracic trauma. Most of the documented cases result in prehospital mortality or death shortly after arrival because of massive hemothorax. Although the surgical stabilization of rib fractures (SSRF) has become increasingly integrated into thoracic trauma care, there remains a lack of awareness regarding the potential for posteriorly displaced rib fractures to cause delayed aortic injury. CASE PRESENTATION: A 71-year-old female patient sustained multiple left posterior rib frac

Rib fractures in frail geriatric patients: Does surgical stabilization improve outcomes?

BACKGROUND: Frail geriatric patients with multiple rib fractures face significant morbidity. While surgical stabilization of rib fractures (SSRF) has demonstrated improved outcomes in adults, its efficacy in frail patients remains unclear. We aimed to compare in-hospital outcomes between SSRF and nonoperative management in frail geriatric patients, hypothesizing that SSRF would be associated with lower complications and mortality. METHODS: A 6-year (2017-2022) retrospective analysis of Trauma Quality Improvement Program database was conducted. Frailty was defined using an 11-factor modified f

Surgical stabilization of rib fractures in anticoagulated patients: Proceed with caution?

BACKGROUND: Surgical stabilization of rib fractures (SSRF) is increasingly performed, however the outcome of patients undergoing SSRF while on pre-injury antithrombotic therapy remains unknown. We compared surgical variables and outcomes of patients who were and were not on antithrombotic therapy. We hypothesize pre-injury anticoagulation is associated with delay in SSRF and worse outcomes. METHODS: For this retrospective cohort study, we queried the Chest Injury International Database, for patients undergoing SSRF between 08/2018 and 03/2022. Antithrombotic therapy was categorized into antip

Multimodal analgesia reduces opioid requirements in trauma patients with rib fractures.

BACKGROUND: Rib fractures are common in trauma patients and are associated with significant morbidity and mortality. Adequate analgesia is essential to avoid the complications associated with rib fractures. Opioids are frequently used for analgesia in these patients. This study compared the effect of a multimodal pain regimen (MMPR) on inpatient opioid use and outpatient opioid prescribing practices in adult trauma patients with rib fractures. STUDY DESIGN: A pre-post cohort study of adult trauma patients with rib fractures was conducted at a Level I trauma center before (PRE) and after (POST

Intravenous Lidocaine for Rib Fractures: Effect on Pain Control and Outcome.

BACKGROUND: Multimodal analgesia in rib fractures (RFs) is designed to maximize pain control while minimizing narcotics. Prior research with intravenous lidocaine (IVL) efficacy produced conflicting results. We hypothesized IVL infusion reduces opioid utilization and pain scores. METHODS: A retrospective review of RF patients at an ACS-verified Level I trauma center from April 2018 to 2/2020 was conducted. Patients (pts) stratified as receiving IVL vs no IVL. Initial lidocaine dose: 1 mg/kg/hr with a maximum of 3 mg/kg/hr. Duration of infusion: 48 h. Pain quantified by the Stanford Pain Score

[Symmetrical rib fractures associated with chronic cough. Report of one case].

Cough may be associated with complications such as syncope, urinary incontinence, pneumothorax, and less frequently, pulmonary hernia and costal fractures. Chronic cough is a cause of rib fractures and when they occur it is likely to affect more than one rib. We report a 53 year-old obese male in treatment with enalapril 10 mg for hypertension with a dry cough lasting five months. He consulted for bilateral chest pain and a Chest X ray examination showed symmetrical fractures in the seventh left and right ribs. Enalapril was discontinued, cough and pain subsided in two weeks.

Cough-induced nonunion rib fractures and herniation: surgical repair and review.

We present the case of a 57-year-old man who had suffered pain in the left hemithorax for a year, which started after a period of severe coughing during pneumonia. The pain was triggered by lying down. A computed tomography scan revealed two nonunion costal fractures. In the operating room, intercostal diastasis with pulmonary herniation was encountered in addition to the costal fractures. This report describes the technique used to reconstruct the thoracic wall with mesh and plate-osteosynthesis.
