Surgical Strategies for Cervical Deformities Associated With Neuromuscular Disorders.

Neuromuscular disorders (NMDs) are diseases involving the upper and lower motor neurons and muscles. In patients with NMDs, cervical spinal deformities are a very common issue; however, unlike thoracolumbar spinal deformities, few studies have investigated these disorders. The patients with NMDs have irregular spinal curvature caused by poor balance and poor coordination of their head, neck, and trunk. Particularly, cervical deformity occurs at younger age, and is known to show more rigid and severe curvature at high cervical levels. Muscular physiologic dynamic characteristics such as spastic

A narrative review on spinal deformities in people with cerebral palsy: Measurement, norm values, incidence, risk factors and treatment.

Spinal deformities are common in people with cerebral palsy (CP), and there is a concern of an increase during the adult ageing period. There is especially a worry about the increase of scoliosis, thoracic hyperkyphosis, lumbar hyperlordosis, spondylolysis and spondylolisthesis incidence, though supporting literature is lacking. Therefore, the aim of this narrative review is to provide a scientific overview of how spinal curvatures should be measured, what the norm values are and the incidence in people with CP, as well as a description of the risk factors and the treatment regimens for these …

Unilateral lumbar paraspinal muscle degeneration is not sufficient to drive spinal deformity in mice.

PURPOSE: Paraspinal muscle degeneration and dysfunction are commonly observed in patients with adult spinal deformity, though their causative role remains unclear. This study aimed to evaluate the effect of unilateral glycerol-induced muscle degeneration on the development of spinal deformity. It was hypothesized that glycerol injections would increase fibrosis (collagen content) and lead to spinal curvature towards the left side. METHODS: Male and female mice were assigned to control (no injection), saline (sham), or glycerol-injection groups. Injections were administered to the right-side l

Relationship between altered axial curvature and neural tube closure in normal and mutant (curly tail) mouse embryos.

Neural tube defects, including spina bifida, develop in the curly tail mutant mouse as a result of delayed closure of the posterior neuropore at 10.5 days of gestation. Affected embryos are characterized by increased ventral curvature of the caudal region. To determine whether closure of the neuropore could be affected by this angle of curvature, we experimentally enhanced the curvature of non-mutant embryos. The amnion was opened in 9.5 day embryos; after 20 h of culture, a proportion of the embryos exhibited a tightly wrapped amnion with enhanced curvature of the caudal region compared with

Lumbatex: A Wearable Monitoring System Based on Inertial Sensors to Measure and Control the Lumbar Spine Motion.

Measuring the curvature of the lumbar spine is an important challenge in disciplines related to physical therapy, rehabilitation, and sports medicine seeking to solve the incidence of the low back pain and other spinal disorders in the population. In clinical practice, most of the methods used are manual or depend on the trained eye of the specialist who is measuring. We have developed Lumbatex: an integrated system based on inertial sensors integrated into a wearable textile device. This device is connected via Bluetooth to software, which interprets data from the sensors and provides real-ti

The Screw-Torque Pelvic Mechanism as a Primary Biomechanical Trigger for So-Called "Idiopathic" Scoliosis: A Synthesis of 30 Years of Research, FEM Analysis, and Clinical Prevention.

Despite decades of research, the primary initiation of "idiopathic" scoliosis remains debated. This study presents a perspective based on "asymmetrical pelvic correlation" as the fundamental mechanical trigger [1]. By shifting the diagnostic focus to the pelvic foundation, we propose a new paradigm in understanding the aetiopathogenesis of spinal curvatures. Based on 30 years of clinical observation (18,071 cases) and Finite Element Method (FEM) simulations, we demonstrate that early-stage scoliosis is a reversible biomechanical response to pelvic torsion.

Improvements in spinal alignment after high tibial osteotomy in patients with medial compartment knee osteoarthritis.

Since the correlation between spinal and lower extremity alignments is high, high tibial osteotomy (HTO) surgery may also affect spinal alignment, where the spinal alignment parameters are the most important parameters for the evaluation of spinal disorders. In this study, the effect of HTO surgery on spinal alignment during gait was investigated by comparing spinal alignment parameters between patients with knee osteoarthritis (OA) and healthy young controls. Eight patients (age, 55.0±5.1years; height, 160.3±7.0cm; weight, 71.3±14.1kg) with a medial compartment knee OA participated in the gai

Design Method to Structure Orthosis Design: Camptocormia Postural Brace Case Study.

The orthosis is considered a class 1 medical device which often originates from a nonstructured development process. As these devices are mainly developed by small- and medium-sized enterprises, with no standard research method, the result can be an unadapted device which may not respond to the user's needs and which in the short term may be abandoned. One way to solve this problem is to define and apply standard rules and procedures throughout the development/design process. Although methodologies may solve the "empiricism" in orthosis design problems, these design strategies are not applied

Staged bilateral pallidotomy for dystonic camptocormia: case report.

Camptocormia is a rare, involuntary movement disorder, presenting as truncal flexion while standing or walking, and is mainly observed as a feature of Parkinson's disease (PD) and primary dystonia. Deep brain stimulation (DBS) of the globus pallidus internus is effective for refractory camptocormia observed with PD or dystonia. However, the effectiveness of pallidotomy for camptocormia has not been investigated. The authors report the case of a 38-year-old man with anterior truncal bending that developed when he was 36 years old. Prior to the onset of the symptom, he had been taking antipsycho
