Tuberculosis tenosynovitis of the flexor tendons in the wrist: a case report.

Extra-pulmonarytuberculosis is a rare form of tuberculosis that can pose diagnostic and therapeutic challenges. We report a case of extra-articular tuberculosis tenosynovitis in a 45-year immunocompetent patient. The symptoms of the patient mimicked De Quervain's disease and carpal tunnel syndrome. Following pathological confirmation of the diagnosis, the patient recovered following a six-month antitubercular chemotherapy.

Tenosynovitis with rice body formation presenting as a cutaneous abscess.

A 62-year-old woman with a past medical history of rheumatoid arthritis was referred to the Department of Dermatology because of an enlargin … as were filled by fluid and multiple small nodular lesions. A diagnosis of non-infectious rice body tenosynovitis was rendered and surgical removal was performed. Total recovery was observed with no evidence of recurrence after 6 months of follow-up. To our knowledge, this is the first report of rice body tenosynovitis presenting as a pseudoinflammatory cutaneo …

Arthroscopic treatment of tenosynovitis of the flexor hallucis longus tendon.

BACKGROUND: Tenosynovitis of the flexor hallucis longus (FHL) tendon is a condition typically found in ballet dancers and sometimes in soccer players and is related to chronic overuse. A traumatic cause for this situation, such as an ankle sprain, is considered rare. In case of failure of conservative treatment, the tendon can be surgically released, which is usually done through an open procedure. This article presents the results of an arthroscopic release of the FHL. METHODS: Twenty-seven patients underwent surgery for FHL tenosynovitis over a period of 18 months. The mean age of the patie…

Rice-body formation and tenosynovitis of the wrist: a case report.

A 61-year-old woman presented with mild pain and swelling on the volar aspect of the wrist, hand, and little finger. Radiography showed a soft-tissue mass shadow, and magnetic resonance imaging showed acute tenosynovitis of the flexor tendons and an inflammatory mass inside the carpal tunnel. Laboratory test results were normal, except for an elevated erythrocyte sedimentation rate (40 mm/h). The patient had an ambiguous Mantoux test result but no history of mycobacterial exposure. Exhaustive investigations for rheumatic disease were negative. Surgical exploration of the lesion revealed rice b…

Gouty flexor tenosynovitis of the digits: report of three cases.

Gouty tenosynovitis in the hand is a rare entity. To date the limited cases described have involved predominantly extensor tendons in the fingers and flexor tendons in the palm or wrist. Flexor tendon involvement is less common and has been reported only twice to our knowledge distal to the palm. We report 3 cases of extra-articular gout presenting as tophaceous involvement of flexor tendons in the fingers. All cases were treated surgically. These cases are presented to heighten awareness of this rare entity and to provide a setting for a discussion of management.

[Tuberculous tenosynovitis of the wrist: ultrasonographic diagnosis and contribution of MRI].

Chronic tenosynovitis of the hand due to Mycobacterium tuberculosis has become rare. In the two cases presented here, the final diagnosis was obtained by isolation of the organism in the intercarpal fluid (1 case), or by a synovial biopsy (1 case). Ultrasonography showed a hypoechoic thickening of one or several tendon sheaths. MR enabled better differentiation between the thickening of the synovium itself and a fluid effusion. The criteria permitting to differentiate tuberculous from non-infectious tenosynovitis are: young age, male sex, specific epidemiological factors, elevated erythrocyte …

Use of contrast radiography in the investigation of tenosynovitis and bursitis in horses.

Contrast radiography, using a 25 per cent solution of sodium diatrizoate, has been used for the investigation of tenosynovitis and bursitis in horses. The procedure was undertaken on a series of 32 clinical cases and on specimens obtained at autopsy. Lesions affecting the extensor tendon sheaths at the carpus, the digital sheath, the tarsal sheath and the bursae at the elbow, hock and fetlock were examined by this means. The results showed the procedure offered the clinician useful information about the nature of the interior of these structures, particularly as regards the presence of adhesio…

Extensor tenosynovitis and subcutaneous abscess caused by hypervirulent Klebsiella pneumoniae.

We report a rare case of invasive Klebsiella pneumoniae infection in a previously healthy 52-year-old Japanese man who presented with extensor tenosynovitis associated with subcutaneous abscess of the right hand, in the setting of concomitant liver and prostatic abscesses. Cultures from blood, liver abscess aspirate, synovial fluid and aspirate from the subcutaneous tissue of the hand all grew K. pneumoniae. Although genomic biomarker testing was not available, a positive string test along with the characteristic clinical presentation suggested a hypervirulent phenotype. Surgical debridement o…

Skin Depigmentation After Particulate Steroid Injection for de Quervain's Tenosynovitis.

BACKGROUND: Corticosteroid injections are an effective treatment for several medical conditions, including de Quervain's tenosynovitis. Depigmentation of the skin after steroid injection is rarely reported. CASE REPORT: We present a case of a 43-year-old man who has no significant past medical history and came to us with an 8-week history of left wrist pain consistent with de Quervain's tenosynovitis. He underwent injections of triamcinolone and bupivacaine into the tendon sheath of the extensor pollicis brevis and abductor pollicis longus. Shortly after 3-5 days posttreatment, he had 100% pa…

A new high-frequency ultrasound classification of De Quervain tenosynovitis.

PURPOSE: This study aimed to introduce a new high-frequency ultrasound classification of De Quervain tenosynovitis based on a large group of patients. Detailed characteristics of classification are also reported. METHODS: From January 2014 to February 2024, patients diagnosed with De Quervain tenosynovitis were retrospectively reviewed. High-frequency ultrasound (7-14 MHz) scanning was performed to identify the anatomy of the extensor pollicis brevis and abductor pollicis longus tendons, presence of intertendinous septa, and pathologic changes. The affected wrist was compared to the contralat…

Tuberculous Flexor Tenosynovitis with Median Nerve Compression: A Case Report.

CASE: A 65-year-old female patient presented with complaints of diffuse pain and swelling in her right wrist with paresthesia in her right hand with thenar wasting. Her magnetic resonance imaging scan was suggestive of flexor tenosynovitis of the wrist with compression of the median nerve with multiple rice bodies. She underwent excisional biopsy along with median nerve decompression. Mycobacterium tuberculosis was detected by polymerase chain reaction (GeneXpert), and histopathology identified caseous granulomas. The patient was started on antitubercular chemotherapy postoperatively. CONCLUS…

Nonsurgical Treatment of De Quervain Tenosynovitis: A Prospective Randomized Trial.

Background: De Quervain tenosynovitis is commonly seen in patients who perform repetitive wrist ulnar deviation with thumb abduction and extension. Previous studies comparing nonsurgical options have contributed to a lack of consensus about ideal management. This study's purpose was to analyze results in prospectively randomized patients treated with either corticosteroid injection (CSI) alone versus CSI with immobilization. Methods: Radial sided wrist pain, first dorsal compartment tenderness, and positive Finkelstein test were used to define De Quervain. Pain score of 4 or higher on a visual…
