Transhepatic Transcatheter Pulmonary Valve Replacement.

Transcatheter pulmonary valve replacement (TPVR) is complicated in patients without adequate femoral or internal jugular vascular access. Transhepatic vascular access has been shown to be safe and effective across a spectrum of diagnostic and interventional procedures. Closure of the hepatic venous tract can be accomplished with a multitude of readily available vascular occlusion devices. The rates of major adverse events are low: 5% to 8% with hemoperitoneum and complete heart block are most significant. To our knowledge, this is the first report of using transhepatic access for TPVR; closure

Propensity Matched Analysis of 14 French Suture- Versus Plug-Based Vascular Closure During Transfemoral Transcatheter Aortic Valve Replacement (TAVR).

BACKGROUND: Plug-based vascular closure devices (Pb-VCD) and suture-based vascular closure devices (Sb-VCD) are used for percutaneous vascular access site closure during transcatheter aortic valve replacement (TAVR). Until now, no clear superiority of either device was shown in studies comparing 18 F VCDs solely. However, there is no data exclusively comparing the 14 F Pb-VCDs against Sb-VCDs after novel 14 F low-profile third-generation heart valve delivery sheath use with focus on vascular complications. AIMS: This study aimed to compare the safety and efficacy of 14 F Pb-VCD to Sb-VCD foll…

Factors in ProGlide® Vascular Closure Failure in Sheath Arteriotomies Greater than 16 French.

OBJECTIVES: The ProGlide® vascular closure device (Abbott Vascular, Redwood City, CA, USA) is approved for the closure of arterial punctures (typically 5-21 Fr sheath; maximum outer diameter, 26 Fr). However, a failure rate of about 2-8% is reported. This study was conducted to analyse factors predisposing to failure when the devices were used for the closure of large hole (16-26 Fr) arteriotomies, and to determine the predictive cut off values of predisposing factors. METHODS: In this retrospective study, the ProGlide® device was used to achieve vascular access site closure in 458 patients u

Transcatheter closure of post-operative type 1 Gerbode defect by Amplatzer Duct Occluder 2.

Acquired and congenital left ventricular to right atrial communication is rare, but nowadays, the frequency of the iatrogenic subgroup is increasing. Successful transcatheter closure of these defects with different devices has been reported. Herein, we presented successful closure of left ventricular to right atrial communication with Amplatzer Duct Occluder 2 after attempting to close with a failed Amplatzer Vascular Plug II device in a 7-year-old girl. This report supports that transcatheter closure of iatrogenic Gerbode defect with Amplatzer Duct Occluder 2 device is safe and effective.

Risk factors associated with device embolisation or malposition during transcatheter closure of patent ductus arteriosus.

BACKGROUND: Device embolisation is a serious adverse event during transcatheter duct closure. This study analyses risk factors for embolisation. METHODS: Demographic parameters, echocardiographic anatomy, haemodynamics, and procedural characteristics of consecutive duct closures in a tertiary centre over 8 years were analysed. Procedures complicated by embolisation were compared to uncomplicated procedures. RESULTS: Fifteen embolisations occurred during 376 procedures. All except one embolisation were in infants. The pulmonary artery: aortic pressure ratio was 0.78 ± 0.22. Embolisation was s

Real-World Experience of the MANTA Closure Device: Insights From the FDA Manufacturer and User Facility Device Experience (MAUDE) Database.

BACKGROUND/PURPOSE: The MANTA vascular closure device (VCD) is the first commercially available dedicated closure device for large-bore femoral arterial access-site closure and was approved by the United States Food and Drug Administration (FDA) in February 2019. Real-world data on the most commonly reported complications and modes of failure associated with the MANTA closure device are limited. We analyzed post-marketing surveillance data from FDA's Manufacturer and User Facility Device Experience (MAUDE) database for the MANTA VCD (Teleflex, Wayne, Pennsylvania). METHODS/MATERIALS: Post-mar

Stent exteriorization facilitates surgical repair for large-bore sheath complications.

Endovascular therapy for peripheral vascular and valvular disease has rapidly expanded and yet also produced new challenges for vascular access and closure. Current generation thoracic endograft (TEVAR) outer diameter sheath sizes range from 18 to 25 French, and the options for vascular access closure include off-label use of a closure device versus surgical cutdown. We describe a patient with a complex, post-dissection, descending aortic aneurysm who required TEVAR and had a vascular access complication repaired with stent exteriorization with open repair.

Comparison of Two Percutaneous Femoral Closure Strategies During Transcatheter Aortic Valve Implantation.

BACKGROUND: Percutaneous closure of femoral arterial access is a critical step in transcatheter aortic valve implantation (TAVI). A hybrid strategy combining a suture-based device (Perclose ProStyle) and a collagen-based device (Angio-Seal) has emerged as an alternative to the conventional double Perclose ProStyle technique. AIMS: The objective of the study is to compare femoral vascular complications between a hybrid strategy (1 Perclose ProStyle+ 1 Angio-Seal) and a non-hybrid strategy (2 Perclose ProStyle) in a large single-center experience. METHODS: This prospective, single-center, non-

Ascending aortic constriction in rats for creation of pressure overload cardiac hypertrophy model.

Ascending aortic constriction is the most common and successful surgical model for creating pressure overload induced cardiac hypertrophy and heart failure. Here, we describe a detailed surgical procedure for creating pressure overload and cardiac hypertrophy in rats by constriction of the ascending aorta using a small metallic clip. After anesthesia, the trachea is intubated by inserting a cannula through a half way incision made between two cartilage rings of trachea. Then a skin incision is made at the level of the second intercostal space on the left chest wall and muscle layers are cleare

Successful management and long term outcome of an accidental subclavian artery injury with a 9 french dilator during pacemaker implantation with collagen-based closure device.

Subclavian artery puncture and dilation with 9 Fr dilator during pacemaker implantation is a rare complication. We present a case, where this complication occurred and the injury was sealed with collagen-based closure device without any complications. Long term follow-up and the control angio-CT revealed no further late vascular complications. Therefore, use of a collagen based closure device for treating accidental iatrogenic subclavian artery injury can be a simple, successful alternative therapy when surgical direct vascular repair is not possible or not available.

Percutaneous closure of nonrestrictive aortopulmonary window in three infants.

Aortopulmonary (AP) window, a communication between ascending aorta and pulmonary artery, is usually nonrestrictive and causes severe pulmonary vascular obstructive disease early in life. Only in 10% of the cases it is restrictive. There are sporadic case reports [1-8] of device closure of AP window which are mostly confined to these restrictive AP windows, that too in adults or relatively older children. Till date there is a single case report of device closure of nonrestirctive AP window in an infant [8]. We report our single experience of device closure of large, nonrestrictive AP windows i

Fully Endoscopic Mitral Valve Repair with Percutaneous Cannulation of Groin Vessels.

Endoscopic mitral valve surgery (EMS) has become a standard of care at specialized heart centers, further reducing surgical trauma compared to a traditional minimally invasive, thoracotomy-based approach. Exposure of the groin vessels for the establishment of cardiopulmonary bypass (CPB) via surgical cutdown in minimally invasive surgery (MIS) may result in wound healing disorders or seroma formation. The avoidance of surgical exposure of the groin vessels by using fully percutaneous techniques for the insertion of a CPB cannula with the implementation of vascular pre-closure devices has the p

Femoral and Brachial Artery Cut-Down for Vascular Access in Interventional Procedures.

Femoral and brachial artery cut-downs are classic surgical methods for establishing vascular access in interventional procedures. Although largely replaced by less invasive percutaneous techniques and vascular closure devices, these open approaches remain essential when anatomy is challenging or large devices are needed. This paper outlines the indications, preoperative assessment, and step-by-step surgical techniques for safely exposing the femoral and brachial arteries. Direct visualization during cut-down facilitates reliable hemostasis and mitigates access-related complications, such as ps…

Percutaneous Closure of a Large-Bore Carotid Arteriotomy Using a Collagen-Based Vascular Plug.

Background: Inadvertent arterial cannulation during central venous catheter placement is a recognized complication with potentially serious consequences, particularly when involving large-caliber catheters. While management strategies have evolved from mandatory surgical repair to various percutaneous approaches, limited data exist regarding collagen-based vascular closure devices for large-bore carotid arteriotomies. Case Presentation: We report the case of a 59-year-old male patient with acute Stanford Type A aortic dissection who underwent emergency surgical repair of the ascending aorta. D…

Ultrasound-Guided vs Non-Ultrasound-Guided Angio-Seal Vascular Hemosasis After Endovascular Treatment for Peripheral Artery Disease: An Observational Study.

Background: Vascular access complications after endovascular treatment for peripheral artery disease (PAD) are relatively frequent, even after the use of vascular closure devices (VCDs). This study investigates the impact of a protocol change toward the use of ultrasound-assisted Angio-Seal closure on vascular complications, compared with non-ultrasound-guided vascular closure. Methods: All endovascular procedures for PAD from 2017 to 2018 (group 1: non-ultrasound-guided) and 2020 to 2022 (group 2: ultrasound-guided) were included in this retrospective study. A Challenging Interventional Procedure: Transcatheter Closure of Tubular Patent Ductus Arteriosus in Patients with Pulmonary Hypertension.

Transcatheter closure of the tubular ducts remains the most challenging procedure, with higher complication rates than other types. This study evaluates the characteristics of transcatheter closure of tubular ducts with pulmonary hypertension. 73 patients with tubular ducts who underwent cardiac catheterization for transcatheter PDA closure were analyzed. The mean age and weight were 1.93 ± 2.68 years and 8.83 ± 6.14 kg, respectively. Transcatheter closure was attempted in 72 patients. Four cases (5.5%) were referred to surgery, while the procedure was completed in the remaining (94.5%). Ampla

Vascular Complications Following Transcatheter Aortic Valve Implantation, Using MANTA (Collagen Plug-Based) versus PROSTAR (Suture-Based), from a French Single-Center Retrospective Registry.

TAVI requires a large-bore arteriotomy. Closure is usually performed by the suture system. Some studies report a vascular complication rate of up to 21%. MANTA is a recently developed percutaneous closure system dedicated to large caliber vessels based on an anchoring system. Early studies report a lower rate of vascular complications with MANTA devices. This single-center retrospective study included all patients who underwent femoral TAVI at the Brest University Hospital from 20 November 2019 to 31 March 2021. The primary endpoint is the rate of vascular complications (major and minor) pre a

Transcatheter closure of a huge complex pulmonary arteriovenous malformation with four devices: a case report.

Pulmonary arteriovenous malformations are abnormal vascular connections between pulmonary arteries and veins, often causing right-to-left shunting. In this report, a 4-year-old boy with low oxygen saturation was diagnosed with a large, complex pulmonary arteriovenous malformation involving four feeding arteries. Percutaneous transcatheter closure was performed using four devices to occlude the major feeding arteries, resulting in increased arterial oxygen saturation from 72 to 98%. This report depicts the successful use of multiple devices for percutaneous closure of a complex pulmonary arteri
