Editorial Commentary: Superior Capsular Reconstructions in Cases of Irreparable Rotator Cuff Tendons Only Partially Restore Anatomy, Yet Significantly Normalize Biomechanics-Without Resorting to Reverse Anatomy.

The goal of shoulder superior capsular reconstruction and/or anterior cable reconstructions, at least in terms of biomechanics, is to primar … ard normalization when tested with standard biomechanical methods. Glenohumeral abduction, superior humeral head migration, deltoid forces, and glenohumeral contact pressure and area, can be optimized toward the normal intact condition, as measured by motion tracking and pressure mapping in real time, using dynamic actuators. Insofar as restoring normal native …

Superior Capsular Reconstruction: Proposed Biomechanical Advantages.

The intact rotator cuff provides dynamic stability for the glenohumeral joint through range of motion by compressing the humeral head to the center of the glenoid. Maintenance of articular congruity provides a stable fulcrum for the more forceful muscles of the shoulder girdle. Massive rotator cuff tears disrupt the concavity-compression mechanism and lead to unopposed superior pull of the deltoid. As a result, superior migration of the humerus, abutment of the acromion, and, in some cases, progression to cuff tear arthropathy occur. Arthroscopic superior capsular reconstruction has emerged as…

Association of damage to the coracohumeral ligament with anterosuperior rotator cuff degeneration revealed by anatomical dissection.

The coracohumeral ligament (CHL) is an important structure of the biceps pulley which also merges with the rotator cuff. Which role it actua … tears and ruptures of the CHL was found. Cartilage damage within the anterosuperior section of the humeral head was observed in 20% cases. In rotator cuff degeneration and atraumatic rotator cuff tears of the elderly population, the pathomechanism of full-thickness RCT is based on repetitive anterosuperior glenoid impingement. This is especially supported by …

Biodegradable implantable balloons: Mechanical stability under physiological conditions.

Rotator cuff tendons injuries occurs as a result of trauma, e.g. due to falling, mechanical injuries and frequent overhead activity and as natural degenerative tears in elderly people. Biodegradable balloon shaped spacer of Poly-(L-lactide-co-ε-caprolactone) (PLCL) are applied in the treatment of these injuries. This type of treatment involves insertion of inflated biodegradable implant into the tissues of the damaged region in the shoulder to avoid shoulder impingement and reduce friction between the acromion and the humeral head and propagation of inflammation. The implant must maintain inte…

Does Postoperative Glenoid Retroversion Affect the 2-Year Clinical and Radiographic Outcomes for Total Shoulder Arthroplasty?

BACKGROUND: While glenoid retroversion and posterior humeral head decentering are common preoperative features of severely arthritic glenohumeral joints, the relationship of postoperative glenoid component retroversion to the clinical results of total shoulder arthroplasty (TSA) is unclear. Studies have indicated concern for inferior outcomes when glenoid components are inserted in 15° or more retroversion. QUESTIONS/PURPOSES: In a population of patients undergoing TSA in whom no specific efforts were made to change the version of the glenoid, we asked whether at 2 years after surgery patient…

Influence of preoperative simulation on the reduction quality and clinical outcomes of open reduction and internal fixation for complex proximal humerus fractures.

PURPOSE: Proximal humerus fractures (PHFs) are common. With the development of locking plates, open reduction and internal fixation (ORIF) of the proximal humerus can provide excellent clinical outcomes. The quality of fracture reduction is crucial in the locking plate fixation of proximal humeral fractures. The purpose of this study was to determine the impact of 3-dimensional (3D) printing technology and computer virtual technology assisted preoperative simulation on the reduction quality and clinical outcomes of 3-part and 4-part proximal humeral fractures. METHOD: A retrospective comparat

Arthroscopic Articular Reconstruction of Capitellar Osteochondral Defects.

BACKGROUND: Articular reconstruction is recommended for some unstable capitellar osteochondritis dissecans (OCD) lesions. Capitellar osteochondral autograft transplantation (OAT) through an open approach for optimal visualization and perpendicular graft implantation has been advocated. However, arthroscopic capitellar OAT may achieve the same treatment goals with decreased morbidity and ability to treat the entire joint for pathologic changes. PURPOSE/HYPOTHESIS: The purpose of this study was to assess the ability to resurface the capitellum arthroscopically. The hypothesis was that arthrosco

Anterosuperior impingement of the shoulder as a result of pulley lesions: a prospective arthroscopic study.

Lesions of the biceps pulley and the rotator cuff have been reported to be associated with an internal anterosuperior impingement (ASI) of t … ad of the biceps tendon, causing increased passive anterior translation and upward migration of the humeral head, resulting in an ASI. In addition, a partial articular-side subscapularis and supraspinatus tendon tear reinforces the ASI.

Distal humerus fractures treated with the anconeus-triceps lateral flap approach: a case series of 10 consecutive patients.

BACKGROUND: The surgical management of distal humeral fractures remains challenging. Posterior approaches involving the triceps mechanism are crucial for achieving stable fixation and functional recovery but are associated with potential approach-related complications. Traditional approaches often involve olecranon osteotomy to achieve adequate exposure, but this technique is associated with specific complications. The purpose of the present study is to describe the clinical outcomes and surgical rationale of the anconeus-triceps lateral flap (ATLF) approach applied to open reduction and inter

Biomechanical effects of teres minor trophicity on external rotation following lower trapezius transfer for posterosuperior massive rotator cuff tears.

BACKGROUND: Lower trapezius tendon (LT) transfer has shown promise in managing irreparable posterosuperior massive rotator cuff tears (PSMRCTs) with weakness of active external rotation (ER). However, the influence of teres minor (TM) trophicity on restoring ER strength has not been thoroughly investigated. This cadaveric biomechanical study evaluated the effect of atrophic, normotrophic, and hypertrophic TM on humeral head stability and ER recovery following LT transfer in PSMRCTs. METHODS: Eight fresh-frozen cadaveric shoulders were tested under 5 conditions: (1) Intact rotator cuff, (2) PS…

Predicting functional outcome after nonoperative treatment of proximal humeral fractures involving the surgical neck.

BACKGROUND: In nonoperative treated proximal humeral fractures (PHFs), uncertainty remains regarding functional outcome. Therefore, the aim was to identify predictors of functional outcome following nonoperative treatment of PHF and to develop predictive models. METHODS: Adults with a nonoperatively treated PHF involving the surgical neck were followed for 1 year. Radiographic parameters included fracture configuration, displacement, bone quality, and the critical shoulder angle. The neck-shaft angle and humeral head offset assessed displacement. The greater tuberosity index (GTI) addressed g…
