Hyperkinetic movement disorders misdiagnosed as tics in Gilles de la Tourette syndrome.

OBJECTIVE: To describe the gamut of movements misdiagnosed as tic exacerbations in Gilles de la Tourette syndrome (GTS) in a referral tertiary-care center. BACKGROUND: Movements seen in GTS can be classified as: (a) tics; (b) movements related to conditions associated with GTS, specifically obsessive-compulsive disorder (OCD), attention deficit hyperactivity disorder (ADHD), and antisocial behaviors; and (c) movements secondary to treatment. METHODS: We reviewed a videotape database and patient records from a tertiary treatment center for GTS and collected GTS cases referred for disease exac

Effects of acamprosate on sleep during alcohol withdrawal: A double-blind placebo-controlled polysomnographic study in alcohol-dependent subjects.

BACKGROUND: Sleep disturbances are frequently encountered in alcohol-dependent patients. Drugs improving sleep during abstinence from alcohol may play an important role in the recovery process. METHODS: In the present study, the effects of acamprosate, a drug successfully used in maintaining abstinence following alcohol withdrawal, were assessed by polysomnographic recordings. A parallel double-blind placebo-controlled study was conducted in 24 male DSM-IV alcohol-dependent subjects aged 35.9+/-1.2 years. Treatments (2 tablets of 333 mg acamprosate vs placebo t.i.d.) were initiated 8 days bef

[Early detection of psychotic disorders].

UNLABELLED: Over the last decade there has been considerable interest in early intervention in schizophrenia and other psychotic disorders, driven by observations that early intervention might favorably alter the course of illness. New clinical and research programs have been established around the globe aiming to reduce treatment delays in psychosis and, more recently, to identify and possibly treat individuals in the pre-psychotic phase who are at imminent risk of developing psychosis. Since May 2004, a service for individuals at high risk (HR) for psychosis has been established at Vienna Ge

Economic Analyses of Obsessive-Compulsive Disorder Interventions: A Systematic Review.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a neuropsychiatric condition featuring patterns of obsessions, compulsions, and avoidant behaviors that are often time consuming and distressing to affected individuals. Cognitive-behavioral therapy (CBT) with exposure and response prevention and/or serotonin reuptake inhibitors are first-line treatments for OCD, though other therapeutic interventions may serve as economically practical modalities under various circumstances. Exploring and understanding the cost effectiveness of all indicated OCD interventions are important to inform therapeut

Subgrouping outpatients of an environmental medicine unit using SCL-90-R and cluster analysis.

Between 1999 and 2003 all consecutive outpatients of the environmental medicine unit of the Charité hospital in Berlin were invited to participate in a study on environmentally related disorders. One hundred and sixty-nine of the patients completed a psychosocial questionnaire which comprised SCL-90-R and 14 other tests. When compared with clinical controls, SCL-90-R mean scores of the environmental patients (EP) were found to lie in between those of inpatients of a psychosomatic clinic and melanoma aftercare patients; but they were, with exception of the somatisation dimension, much closer to

Assessment of Public Stigma, Mental Health Literacy, and Help-Seeking Intentions Based on Different Dimensions of Obsessive-Compulsive Content: A Study of the Spanish Mental Health-Naïve Population.

BACKGROUND: Obsessive-compulsive (OC) disorder is a debilitating disorder with a high delay in help-seeking that could be associated with two barriers that may differ between OC content dimensions: public stigma and mental health literacy. OBJECTIVES: We aim to describe and analyze the differences among OC content dimensions in public stigma, social distance desire, mental health literacy, and help-seeking intention in a sample of the mental health-naïve population. METHODS: A total of 487 participants from the Spanish community with no previous knowledge of or experience with OCD were rando

Early onset of obsessive-compulsive disorder and associated comorbidity.

BACKGROUND: Previous studies have aimed to identify subtypes of obsessive-compulsive disorder (OCD) based on their age of onset (AOO). Obsessive-compulsive spectrum disorders (OCS disorders) such as tic disorders have been particularly associated with an early onset in some studies. However, subtypes of early- and late-onset OCD are unevenly determined, and the biological and the clinical validity of these subtypes are unknown. This study was undertaken to discriminate the subtypes of OCD in different AOO levels and to test the hypothesis that different AOO bands are associated with a differen

Deontological Guilt Differentially Affects Moral Behaviour in Participants With and Without Obsessive-Compulsive Disorder (OCD).

Obsessive-compulsive disorder (OCD) is characterised by a dysfunctional sensitivity to a sense of guilt that significantly interferes with everyday functioning and is believed to be a key mechanism in symptom maintenance. Mounting evidence indicates that individuals with OCD are particularly sensitive to deontological guilt, which stems from the perception of violating an internalised rule, as opposed to altruistic guilt, which arises from the feeling of having harmed others. Here, we assess the impact of deontological vs. altruistic guilt on moral behaviour in participants with OCD. Twenty pa

Investigating the events that predispose vulnerable and feared self-concepts in obsessive-compulsive disorder: A qualitative analysis.

OBJECTIVES: The feared self-concept, the self that a person fears they may be or could become, has been increasingly shown to drive the development and maintenance of obsessive-compulsive disorder (OCD). It is proposed that the feared self develops from a vulnerable self-concept, which itself is a result of normative self-concept development being disrupted by negative early life experiences. However, it is currently unclear which experiences lead to the development of vulnerable self-concept in people with OCD. This study aimed to describe the events and life experiences that people with OCD

Functional neuroimaging subtypes of obsessive-compulsive disorder: A systematic review and meta-analysis.

Obsessive-compulsive disorder (OCD) exhibits substantial clinical heterogeneity that may reflect underlying neurobiological diversity. Neuroimaging-based subtyping may advance precision psychiatry by identifying biologically distinct subgroups with differential treatment responses. This study systematically synthesized evidence from functional neuroimaging subtyping studies in OCD to identify reproducible neurobiological subtypes, characterize their clinical profiles, and establish a consensus-based classification framework. We reviewed 40 original studies employing machine learning, clusterin

Distinguishing Suicidal Obsessions from Suicidal Ideation: A First-Person Suicide Image Pilot Study.

Individuals with obsessive-compulsive disorder (OCD) experience unwanted and intrusive thoughts that cause anxiety and/or distress. The content of these thoughts can vary widely. Suicidal obsessions (SOs) are unwanted, intrusive, and distressing thoughts about suicide, death, or self-harm. Their clinical presentation differs markedly from that of suicidal ideation (SI), yet most suicide risk assessments fail to distinguish between the two phenomena. The distinction is critical, as SOs and SI differ in their clinical risk and require different treatments. This pilot study compares the self-repo

Neuroimaging and Machine Learning in OCD: Advances in Diagnostic and Therapeutic Insights.

BACKGROUND/OBJECTIVES: Obsessive-Compulsive Disorder (OCD) is a chronic mental health condition characterized by intrusive thoughts and repetitive behaviors. Traditional diagnostic methods rely on subjective clinical assessments, delaying effective intervention. This review examines how advanced neuroimaging techniques, such as Magnetic Resonance Imaging (MRI) and Diffusion Tensor Imaging (DTI), integrated with machine learning (ML), can improve OCD diagnostics by identifying structural and functional brain abnormalities, particularly in the cortico-striato-thalamo-cortical (CSTC) circuit. 'Not my mess'?: How do supporters of individuals with hoarding difficulties rate the quality of the support they offer?

OBJECTIVES: Hoarding difficulties (HD) affect many people and cause upset and danger for the person, as well as friends and family. Previous research found that people with HD feel less adequately socially supported compared with individuals with obsessive compulsive disorder (OCD). This study used the perspective of those offering support to infer whether people with HD view their support differently, or if there is a gap in support quality compared with those with OCD. DESIGN: The design was cross-sectional, comparing those supporting OCD with those supporting HD. METHODS: An online questi

Heightened but Inefficient Thought-Action Fusion in Obsessive-Compulsive Disorder: New Insight From a Multiple Trial Version of the Classic Thought-Action Fusion Experiment.

OBJECTIVE: Thought-action fusion (TAF), which is a tendency to make the relationship between one's thoughts and external consequences, is a dysfunctional belief linked to obsessive-compulsive disorder (OCD). While the TAF is commonly evaluated using the Thought-Action Fusion Scale (TAFS), it cannot fully reflect the actual experience of experimentally evoked TAF. In the present study, we applied a multiple-trial version of the classic TAF experiment and evaluate two variables, reaction time (RT) and emotional intensity (EI). METHODS: Ninety-three patients with OCD and 45 healthy controls (HCs

Mental Health Problems and Associated Factors among High School Students in Shandong Province of China: A Cross-Sectional Study.

Background: Although many studies have analyzed the mental health problems (MHP) of Chinese adolescents, the associations of Chinese high school students’ MHP with personal and family circumstances, parents’ educational level, physical activity, and school adaptation are rarely discussed. Methods: The participants were 9398 students who were randomly recruited from 30 high schools in Shandong, China. Self-reported questionnaires were used to collect data. Multivariate logistic regression models were used to investigate associations between MHP and related factors. Results: The positive rate of

Increased Risk of Sjögren's Syndrome in Patients with Obsessive-Compulsive Disorder: A Nationwide Population-Based Cohort Study.

Obsessive-compulsive disorder (OCD) includes a wide range of symptoms and is often associated with comorbidities. Although psychiatric involvement may be an early manifestation of Sjögren's syndrome (SS), only a few studies have demonstrated the relationship between OCD and SS. This is a nationwide cohort study identifying the risk of SS in OCD patients. We studied a longitudinal health insurance database for the period from 1999 to 2013. The study group was OCD patients with at least three outpatient visits or one hospitalization. The comparison cohort was matched by age and sex, as well as c

Remote memory of drug experiences coexists with cognitive decline and abnormal adult neurogenesis in an animal model of cocaine-altered cognition.

Cocaine addiction is a chronic disorder in which the person loses control over drug use. The past memories of the stimuli associated with the drug are a relevant clinical problem, since they trigger compulsive drug-seeking and drug-taking habits. Furthermore, these persistent drug-related memories seemingly coexist with cognitive decline that predicts worse therapeutic output. Here, we use a new animal model of cocaine-altered cognition that allowed to observe these events in the same individual and study their relationship. Mice were chronically administered cocaine in a conditioned place pre

Distress, omnipotence, and responsibility beliefs in command hallucinations.

OBJECTIVES: Command hallucinations are considered to be one of the most distressing and disturbing symptoms of schizophrenia. Building on earlier studies, we compare key attributes in the symptomatic, affective, and cognitive profiles of people diagnosed with schizophrenia and hearing voices that do (n = 77) or do not (n = 74) give commands. METHODS: The study employed a cross-sectional design, in which we assessed voice severity, distress and control (PSYRATs), anxiety and depression (HADS), beliefs about voices (BAVQ-R), and responsibility beliefs (RIQ).
