Addressing Psychosocial Adversity Within the Patient-Centered Medical Home: Expert-Created Measurable Standards.

The Patient-Centered Medical Home (PCMH) may be improved by embedding identification and response for patients' experiences with psychosocial adversity, but how this might optimally occur in practice has not been well-specified. We sought input from an expert panel to define feasible elements that could adapt the PCMH to adequately respond to patients' experiences with psychosocial adversity. From December 2012 through September 2013, we used a Delphi process to systematically obtain expert opinions and reach consensus. We invited 37 experts to participate in three successive and iterative rou

Facing the challenges of chronic pruritus: a report from a multi-disciplinary medical itch centre in Germany.

The complex nature and difficult-to-establish aetiology of chronic pruritus (CP) makes it challenging to provide medical care for patients with CP. This challenge can only be met with a multidisciplinary approach. The first multidisciplinary Itch Centre in Germany was established at the University of Münster in 2002 to meet the needs of this patient population. More than 2,500 outpatients and 400 inpatients are diagnosed and receive treatment each year. To ensure evidence-based medical care, an electronic system for medical documentation and patient-reported outcomes was established. Automated

A unique model for treating chronic hepatitis C in patients with psychiatric disorders, substance abuse, and/or housing instability.

BACKGROUND: Individuals with psychiatric disease, substance abuse, and/or housing instability have a high prevalence of chronic hepatitis C virus (HCV) infection. However, such individuals are often excluded from treatment for HCV infection because of a perceived inability to adhere to the rigorous medication regimen required. METHODS: A pilot program using a multidisciplinary group medical visit model to treat HCV infection in the aforementioned population was created. Medication adherence and virologic response rates were prospectively followed. RESULTS: Approximately 80% of patients were

The relation between social problems and substance use in adolescent boys: an investigation of potential moderators.

This study investigated the relation between social problems and adolescent male substance use in the context of other potential moderating variables. Two hundred eighty adolescent boys completed measures of social problems, cognitive distortions, constructive thinking, affiliation with delinquent peers, and multiple aspects of substance use. Results revealed that social problems were not directly related to most substance use variables. Cognitive distortions moderated the relation between social problems and substance use in social situations, such that the relation was significant only at hi

The older adult: a comparison of long-term care in Glasgow and San Francisco.

Population trends show an increasing proportion of people older than 65 years of age. This report aims to describe the facilities/services accessible to the older adult living in Glasgow, Scotland and in San Francisco, United States of America. This comparative study serves to develop a broader understanding of what is involved in the process of ageing, and describes how these two cities are responding to the needs of older people. Long-term caring provision is complex and involves multiple agencies, often leaving both lay and professional individuals ill-informed as to which services exist an

Improving the current system for supplying organs for transplantation.

The United States currently relies on a voluntary, altruistic system for supplying organs for transplantation. It is now generally recognized that this system, as currently operated, produces a seriously inadequate supply of organs. A number of scholars have argued that some type of (generally unspecified) market system is necessary. Two articles appearing in the Journal of Health Politics, Policy and Law have proposed relatively specific market systems for increasing the supply of organs. In this paper we argue that market systems are at best premature. In particular, there is little to sugge

Sustainable improvement of interprofessional care for better resident outcomes: protocol for the INTERSCALE hybrid type III effectiveness cluster-randomized trial comparing individualized and collaborative delivery of an evidence-based care model for long-term care.

BACKGROUND: Over recent decades, multifaceted nurse-led care models have been developed to reduce unplanned hospital transfers from long-term care facilities (LTCFs). In Switzerland, the INTERCARE model has demonstrated effectiveness, with core components including deployment of nurses in expanded roles (INTERCARE nurses), evidence-based communication tools, and advance care planning. However, resource-intensive implementation strategies such as 1:1 support meetings for model implementers pose challenges for scale-up, underscoring the need for more scalable implementation support. The INTERSCA

Comparative evaluation of nurse-led versus traditional trauma services in plastic surgery: a retrospective cohort study.

BACKGROUND: Traditional trauma care often experiences significant delays in surgical readiness due to fragmented coordination between surgical and nursing teams. Nurse-Led Trauma Services (NLTS) have emerged as a collaborative alternative to optimize clinical efficiency. This study evaluates the efficacy and safety of an NLTS model compared to Traditional Trauma Services (TTS) in an emergency plastic surgery department. METHODS: This retrospective cohort study analyzed 248 patients (NLTS: n = 107; TTS: n = 141) admitted to a tertiary emergency center in China for plastic surgical intervention

Optimizing pediatric bronchiolitis management through an integrated hub-and-spoke network: evidence from a regional Italian experience.

UNLABELLED: This study aimed to describe the implementation and functioning of a regional hub-and-spoke model ("Gaslini Diffuso") for managing pediatric bronchiolitis in Liguria, Italy, during the 2023-2024 season, focusing on severity stratification, resource allocation, and outcomes. A retrospective observational study was conducted across one tertiary hub (IRCCS Istituto Giannina Gaslini, Genoa) and four affiliated spoke hospitals. Medical records of all patients aged 0-2 years hospitalized with bronchiolitis (ICD-9-CM 466.19) between October 2023 and March 2024 were reviewed. Demographic,

Weaponizing the Appraisal: Justice Violations, Psychological Safety, and Employee Voice in a Saudi Multinational Organization.

BACKGROUND: Conduct that degrades employees - belittling speech, the coercive use of rank, and treatment stratified by contractual or national category - ranks among the most consequential and least tractable problems confronting the human resource (HR) function, which authors the policies prohibiting such conduct, administers the channels through which it is reported, and answers for the outcome when a report yields nothing. This study asks how such conduct persists within an organization that formally forbids it, and where HR can intervene, by reconstructing the fairness appraisals and safet

Application of Behavioral Theories to Disaster and Emergency Health Preparedness: A Systematic Review.

BACKGROUND: Preparedness for disasters and emergencies at individual, community and organizational levels could be more effective tools in mitigating (the growing incidence) of disaster risk and ameliorating their impacts. That is, to play more significant roles in disaster risk reduction (DRR). Preparedness efforts focus on changing human behaviors in ways that reduce people's risk and increase their ability to cope with hazard consequences. While preparedness initiatives have used behavioral theories to facilitate DRR, many theories have been used and little is known about which behavioral t

Exploring menstrual health knowledge, attitude, and experiences among adolescent girls in public schools in Georgia: a qualitative study.

BACKGROUND: Despite being a physiological process for human development, menstruation can be burdened with a complex interplay of interpersonal, social, and structural factors. Evidence on menstrual health remains scarce across Eastern Europe and Central Asia, and published research specifically addressing adolescent girls' menstrual health experiences in Georgia is lacking, despite cultural and historical factors that suggest unique challenges in this setting. Public schools represent a critical but understudied environment where infrastructural, educational, and social barriers to menstrual

Mediating Effects of Professional Quality of Life in the Association of Team Job Crafting With Career Success Among Nurses: A Structural Equation Model.

BACKGROUND: Team job crafting has emerged as an important organizational resource that may enhance nurses' professional functioning. However, its potential mechanisms in shaping nurses' career success remain insufficiently understood. OBJECTIVE: This study examined the direct association between team job crafting and career success among nurses and explored the mediating roles of the three dimensions of Professional Quality of Life (ProQoL), including compassion satisfaction (CS), burnout (BO), and secondary traumatic stress (STS). METHODS: This secondary data analysis used cross-sectional s

The impact of high-performance work systems on employees' subjective well-being: the mediating effect of emotional commitment and the moderating effect of interpersonal fairness.

Employee well-being has become a strategic imperative as organizations worldwide confront rising burnout, disengagement, and the human costs of intensified competition. Yet organizations face a paradox: the high-performance work systems (HPWS) they adopt to remain competitive may either nourish or erode the well-being of the employees who enact them. Motivated by this tension, this study investigates not only whether HPWS influences employees' subjective well-being (SWB), but also how and when this influence occurs. Drawing on self-determination theory, it proposes that HPWS enhances SWB by sa

Health Care Analytics Challenges: A 3-Pillar Framework Connecting Analytics Maturity, Workforce Agility, and Technical Enablement.

Health care organizations face a "triple threat" of low analytics maturity, high workforce instability, and semantic technical barriers that together produce a crisis of "institutional amnesia." Leadership turnover, workforce shortages, and widespread intent to leave among informatics specialists systematically erase the tacit knowledge required to navigate complex clinical data schemas, trapping organizations in a cycle where knowledge loss outpaces knowledge capture. Viewed through the socialization, externalization, combination, and internalization model of knowledge creation by Nonaka, the
