UNLABELLED: We describe modifications to previously described techniques for evacuation of Soemmerring ring during secondary intraocular lens (IOL) implantation in aphakic eyes following previous pediatric cataract surgery. A new anterior capsulotomy is initiated using a cystotome to incise the anterior capsule close to its attachment to the posterior capsule. CASE REPORT: Shortly after birth, a massive enlargement of the right eye was observed in an otherwise healthy male child. The cornea of the affected eye was vascularized and completely cloudy without a sharp border between cornea and sclera. The diagnosis of a congenital glaucoma was made but an operation was not undertaken because of the difficult anatomical situation and the lost function. When the child was almost 3 years old enucleation was performed to prevent complications due to corneal exposure, and to improve the cosmetic aspect.

Cataracts in children present a greater management challenge, compared with cataracts in adults, because of the wider variety of causes and the vulnerability of the developing visual system. A completely new surgical technique to obtain pupillary dilatation is presented. It permits achievement of a mydriasis sufficient to observe the vitreous cavity in both phakic and aphakic eyes during vitreous surgery. Miotic immobile pupils not dilatable pharmacologically are enlarged by means of a silastic ring with a C-shaped groove on its outer part. This is introduced into the eye through a limbic opening. Aphakic eyes possess considerably less oblique astigmatism than phakic eyes. The possibility that posterior iris displacement after lens extraction accounts for the observed reduction in oblique astigmatism is tested in this study. Oblique astigmatism was calculated for a phakic and an aphakic schematic eye. With no iris displacement little reduction of oblique astigmatism occurred contrary to experimental findings. The follow up of the patients was short and there may be failures later because of frequent lens loss or deposits on the lenses. In this study the failures which occurred did so within the first few weeks. It is probable that we can advise our patients that Extended Wear Contact Lenses are worn successfully by 70% of the patients with gratifying results.

A technique is described for implantation of a posterior chamber intraocular lens during keratoplasty for bullous keratopathy when a large capsulotomy and vitreous prolapse are present. Seven cases of pseudophakic or aphakic bullous keratopathy with anterior chamber, iris-fixated, or no intraocular lens in place are reported.
