Peutz-Jeghers syndrome (PJS) is an autosomal dominant disorder characterized by the association of mucocutaneous pigmentation and multiple gastrointestinal hamartomatous polyps and with an increased risk of developing gonadal sex tumors besides other malignancies. 78 patients with chronic anal fissures have been mainly operated on by lateral internal sphincterotomy (LATS). Continence have been evaluated by questionnaire at least 9 months postoperatively. Patient without any additional proctological operation had minor disturbances of continence in 17%. Patient with additional operations had disturbances of continence in 30%. Especially the subgroup of patients with LATS and haemorrhoidectomy had bad results. In this group only 45% were fully continent.

In order to determine the frequency of tumours in patients presenting with frank rectal bleeding, a prospective study was carried out in 337 patients over the age of 40 attending our Proctology Clinic. After making a clinical diagnosis, flexible sigmoidoscopy followed by barium enema was performed. Excluding 7 digitally palpable rectal cancers, there were 30 cancers (9.1%), 34 polyps (10.3%), 7 proctocolitis (2.1%) and 25 diverticula (7.6%) detected, giving a total diagnostic yield of 29.1%. A 75-year-old man developed neurilemmoma of the left eighth cranial nerve, adenomatous polyp of the sigmoid colon, adenocarcinoma of the prostate, mixed cell type lymphoma of the right neck, and infiltrating ductal carcinoma of the right breast metachronously during a period of 38 years. The cancer of the right breast occurred 7 years after the long-term stilbestrol treatment of prostatic carcinoma, which suggests a causal relationship between the hormonal therapy and development of breast carcinoma.

The UCLA Family Practice Residency continues to use 60-cm flexible sigmoidoscopy in the care of patients. Between July 1980 and June 1983, 450 patients received an examination with the 60-cm flexible sigmoidoscope. One or more adenomatous polyps were detected in 21 patients. The indications and limitations of colonoscopy in the diagnosis of inflammatory bowel disease are well defined. The endoscopic examination is usually easily performed and well tolerated by the patient, but, since endoscopic examination of the colon is an invasive procedure (and potentially dangerous), colonoscopy should only be performed in those patients in whom the indication is clear and the benefits identifiable. Colonoscopy may provide valuable information in the diagnosis and may help outline the course of therapy in patients with inflammatory bowel disease.
