Surgical compared with conservative treatment for acute nondisplaced or minimally displaced scaphoid fractures: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: There is a current trend in orthopaedic practice to treat nondisplaced or minimally displaced fractures with early open reduction and internal fixation instead of cast immobilization. This trend is not evidence-based. In this systematic review and meta-analysis, we pool data from trials comparing surgical and conservative treatment for acute nondisplaced and minimally displaced scaphoid fractures, thus aiming to summarize the best available evidence. METHODS: A systematic literature search of the medical literature from 1966 to 2009 was performed. We selected eight randomized cont

Comparison of a PACS workstation with laser hard copies for detecting scaphoid fractures in the emergency department.

Picture archiving and communication systems (PACS) for imaging studies is rapidly being adopted in hospitals throughout the UK. However, very little comparison has been made between PACS and laser hard copies for assessing the diagnostic accuracy of detecting fractures by emergency physicians. A prospective paired comparison study was undertaken looking at correct reporting of scaphoid X-rays on PACS and conventional film by emergency department medical staff. A total of 34 imaging studies were reported by 38 physicians using both PACS workstations and laser-printed films. The percentage of em

[Consequennce of nontreatment scapholunate instability of the wrist].

Authors evaluate the results of patients with acute rupture of the scapholunate (SL) ligament, demonstrated by arthroscopy, in this study. One group of patients was treated with reinsertion of SL ligament by the help of Mitek anchor. In the other group there were patients with diagnosed total rupture of SL ligament which rejected the reinsertion of the ligament. Authors evaluate results of both groups 24 months after the injury. On the basis of clinical and X-ray results, even after such short time, reasonably worse results for the group without performed reinsertion were determined. Even X-ra

MRI and plain radiography in the assessment of displaced fractures of the waist of the carpal scaphoid.

We treated 50 patients with fractures of the waist of the scaphoid in a below-elbow plaster cast for up to 13 weeks. Displacement of the fragments was assessed independently by two observers using MRI and radiographs performed within two weeks of injury. The MRI assessments showed that only the measurement of sagittal translation of the fragments and an overall assessment of displacement had satisfactory inter- and intra-observer reproducibility and revealed that nine of the 50 fractures were displaced. Only three of the 49 fractures with adequate follow-up failed to unite, and all were displa

Treatment of scaphoid nonunion with casting and pulsed electromagnetic fields: a study continuation.

This article presents a continuation of a study of the treatment of scaphoid nonunion with pulsed electromagnetic fields (PEMF) and cast immobilization. Fifty-four patients were reviewed. The overall success rate for healing has decreased since the previous review from 80% to 69%. Proximal pole fractures healed in 50%. Success in nonunions with associated radiographic evidence of avascular necrosis decreased from 89% to 73%. Although we believe that the indications for use of PEMF have not changed significantly, this study suggests that a successful outcome with PEMF and casting is less likely

Herbert Screw Fixation of Scaphoid Fractures.

Forty-four scaphoid fractures treated with open reduction and internal fixation using the Herbert screw were available for evaluation at an average follow-up period of 2.1 years and a range of 1 to 6 years. They were classified according to Herbert into 25 unstable (type B); 8 delayed unions (type C); and 11 nonunions (type D). Bone grafting was required in 22 cases. Sound radiological union was achieved in 42 cases at an average of 3.2 months. The two nonunions healed after revision grafting. Using the Mayo wrist score, excellent results were achieved in 14 cases; good in 22; fair in 4; and p

The surgical outcomes of trans-scaphoid perilunate fracture-dislocations.

UNLABELLED: 1. BACKGROUND/AIM: Trans-scaphoid perilunate fracture-dislocation (TSPFD) is a rare injury. TSPFD is a fracture-dislocation that severely disrupts the anatomical structure of the carpal bones and may occur as a result of a high energy trauma of the wrist or a fall on an open hand. In this study, the aim is to provide midterm clinical and radiological evaluations of cases diagnosed and treated as TSPFD. MATERIALS AND METHODS: Eleven patients diagnosed with TSPFD as a result of wrist trauma were treated surgically and were analysed retrospectively. Clinical and radiological follow-

Ideal Starting Point and Trajectory of a Screw for the Dorsal Approach to Scaphoid Fractures.

PURPOSE: To determine the screw starting point and trajectory for the dorsal approach to scaphoid fractures that provides a combination of length and compression at the fracture site. METHODS: Computed tomography scans were obtained of 10 scaphoid fractures for 3 common fracture types. A computerized model was generated for each scaphoid. Screw starting point, length, and angle to the fracture plane were analyzed for starting points and trajectories within a safe zone that protected against cortical penetration. A novel analysis was developed to assess a combination of screw length and angle

Palmar divergent dislocation of scaphoid and lunate.

A 28-year-old man presented with a palmar divergent dislocation of the scaphoid and lunate. He was treated with an open reduction and an internal fixation with two Kirschner's wires after the 25th day of trauma due to a neurological injury. The results were satisfactory after 18 months follow up without any evidence of avascular necrosis and traumatic arthritis of the scaphoid and lunate. The patient had no limitation in motion or intermittent wrist pain. We reported this case with a brief review of relevant literatures.

Capitate and Hamate Fracture. Case Study.

Capitate and hamate fractures are infrequent injuries and are uncommon in isolation. A capitate fracture is usually associated with a scaphoid fracture. The primary mechanism of injury is a fall with the wrist in hyperextension. Other possible ways for capitate fractures are axial down trauma of the third metacarpal and direct trauma. Hamate fractures have a 2% incidence among carpal bone fractures, probably due to underreporting. They can occur on the hamate body or the hamulus or hook. Combined capitate and hamate fractures are uncommon and relevant studies, especially case reports, are scar

Talo-patello-scaphoid osteolysis, synovitis, and short fourth metacarpals in sisters: a new syndrome?

Osteolysis syndromes are characterized by resorption of affected bones with associated swelling and pain. Various forms of multicentric osteolysis syndromes including autosomal dominant and recessive carpal-tarsal osteolysis, Torg, François, Whyte-Hemingway, Hajdu-Cheney, Winchester, and other forms have been described. Most present in pre-school years with extensive involvement and destruction of multiple bones. We present a sister-pair, both of whom presented in early teenage, i.e., 13 and 15.5 years, respectively, with bilateral ankle, knee, and later, wrist pain. Radiological examination r

Long-term results of bone-retinaculum-bone autograft for scapholunate instability.

PURPOSE: To report long-term follow-up of scapholunate interosseous ligament reconstruction with bone-retinaculum-bone autograft in patients with dynamic scapholunate instability. METHODS: Of the 14 patients from the previously reported cohort who had bone-retinaculum-bone autograft for dynamic instability, 6 returned for clinical examination and radiographs, 3 were reached by telephone, and 2 were lost to follow-up. The remaining 3 had salvage procedures (2 total wrist arthrodeses and 1 proximal row carpectomy) between the prior report and the current study and thus reached an endpoint, at 2

[Scapho-capitate fracture syndrome of both hands--first description of a bilateral occurrence of a rare carpal injury].

The scapho-capitate fracture syndrome is a rare but severe injury of the carpus. It is characterized by a rotation of the proximal fragment of the capitate bone of 90 or 180 degrees and a fracture of the navicular bone in the intermediate third. This injury is caused by a fall on the outstretched and dorsiflected hand. The scapho-capitate fracture syndrome was firstly described in 1956 by Fenton. There are 31 cases described in literature but none of these involves both hands. Here we describe the first case of scapho-capitate fracture syndrome in both hands, in a 21 year old woman who jumped

Locally Invasive Pigmented Villonodular Synovitis of the Wrist Treated by Arthroscopic Resection and Bone Grafting.

Pigmented villonodular synovitis (PVNS) is a benign but locally aggressive neoplasm that can affect tendon sheath, bursae, or joint. The wrist joint however is uncommonly involved and here we present a case of chronic monoarticular joint pain and swelling in a healthcare professional that was later histologically verified to be PVNS of the radiocarpal joint. The patient had a magnetic resonance imaging (MRI) performed prior to surgery which showed a locally invasive bony tumor of the scaphoid. He subsequently underwent a wrist arthroscopic evaluation and resection with bone grafting as the ind

Non-union of the scaphoid treated with styloidectomy and compression screw fixation.

17 patients were re-examined 13 to 19 years (mean 16 years) after operation for non-union of the scaphoid. A standardized method including styloidectomy and compression screw fixation according to McLaughlin was used. The mean interval between trauma and operation was 34 months. All patients were male and the mean age at surgery was 31 years. Bone union was obtained in 65% (11/17) and radio-carpal arthrosis was found in 83% (14/17) compared with 41% (7/17) at the time of operation. Impaired motion as well as marked decrease in strength was noted in comparison with the healthy side. 88% of the

Imaging of Wrist Injuries: A Standardized US Examination in Daily Practice.

The keys to successful ultrasonography (US) of the wrist include knowledge of the relevant anatomy and understanding the biomechanical aspects. A wide spectrum of pathological findings including bone fractures (scaphoid, triquetrum) and ligament lesions (dorsal intercarpal and radiocarpal ligaments, scapholunate ligament) can be caused by an identical traumatic mechanism determined on the basis of the position of wrist at the time of injury. In the setting of wrist trauma, an early diagnosis can minimize the potential for inappropriate or delayed treatment. We describe a practical radiological

Treatment of scapho-lunate instability.

Scapho-lunate dissociation is one of the most frequent reasons of carpal instability. Diagnosis is mostly based on a careful physical examination where stability of the SL joint is specifically checked through different clinical maneuvers, followed by static and dynamic radiographic examination of the wrist in a variety of positions and loading conditions aiming at demonstrating the presence of a gap between the two dissociated bones. When in doubt, arthroscopy is used. In the treatment five factors are to be considered: integrity of the dorsal SL ligament, its healing potential, status of oth

Electrical stimulation applied to bone and nerve injuries in the upper extremity.

In conclusion, electrical stimulation of bone has advanced from the laboratory to clinical reality. Despite the lack of good double-blind clinical studies, it is impossible to ignore the excellent results reported from numerous multicenter trials. Doubts and controversies will and should continue. Electrical stimulation has a definite place in the treatment of scaphoid nonunion as well as other failures of osteogenic biology in the upper extremity. The future may realize the enormous potential of electrical stimulation in areas of nerve repair, wound healings, or osteoporosis. The hand surgeon
