Anterior cruciate ligament injuries in patients with open physes.

From July 1988 to August 1989, six children with open physes and injuries to the anterior cruciate ligament were treated operatively. All injuries involved twisting episodes during sports activities. All six patients had meniscal abnormalities, and, additionally, one patient sustained a Grade III medical ligament tear and had a lateral patellar dislocation. Operative reconstruction used hamstring tendons and place a groove over the front of the tibia and a groove over the top of the femur without violation of the growth plates. The anterior cruciate ligament was primarily repaired (three patie

Long-term results of total arthroplasty in adolescents with debilitating polyarthropathy.

The authors retrospectively reviewed 29 hip and 13 knee arthroplasties performed in 17 adolescents and young adults from 1973 through 1979. Most patients had severe multiple joint involvement; 16 had juvenile rheumatoid arthritis. Clinical and roentgenographic evaluations were performed before operation and at routine intervals for up to 11 years after operation. The average final follow-up evaluation was at ten years seven months. The modified Harris rating improved from 17 before operation to 68 at final evaluation. A dramatic improvement was noted in the ambulatory ability of 13 patients in

Functional deficits and significant patellofemoral chondral deterioration two years after trochleoplasty and medial patellofemoral ligament reconstruction.

PURPOSE: Patellofemoral instability is a common knee condition and often associated with trochlear dysplasia. Sulcus-deepening trochleoplasty is a surgical option to reshape the trochlear groove and stabilise the patella. Although this procedure is considered safe and effective, the development of postoperative patellofemoral chondral lesions remains a topic of concern. The aim of this study was to evaluate the clinical and radiological outcomes following combined trochleoplasty and medial patellofemoral ligament reconstruction with a minimum follow-up of 2 years. METHODS: We assessed 23 pati

Survivorship, complications, and outcomes following distal femoral replacement for neoplastic indications.

BACKGROUND: Distal femoral replacements (DFRs) are excellent treatment options for limb salvage procedures in patients who have bone loss secondary to neoplasm. Multiple studies report adequate survivorship and complication rates following DFR implantation, primarily for non-neoplastic indications. However, current literature regarding neoplasm-specific reports is often limited by sample size, survivorship, and patient reported outcome measurements. Therefore, we sought to examine patients who received a DFR for a neoplastic indication at multiple tertiary academic centers. Specific outcomes a

Effects of knee flexion and extension on the tibial tuberosity-trochlear groove (TT-TG) distance in adolescents.

BACKGROUND: Measurement of the tibial tubercle-trochlear groove (TT-TG) distance is used to assess patellofemoral instability and rotation. Since patellofemoral instability and acute patellar dislocation are common among adolescents, it is important to clarify the relationship between TT-TG distance and various flexion and extension angles in asymptomatic children. The purpose of the present study was to determine how knee flexion and extension influence TT-TG-distance values measured using 3D imaging in an anatomic axial plane among asymptomatic adolescents. METHODS: We performed magnetic re

Intertunnel Relationships in the Tibia During Reconstruction of Multiple Knee Ligaments: How to Avoid Tunnel Convergence.

BACKGROUND: Knee dislocations are rare but potentially devastating injuries, often involving tears of multiple knee ligaments. Several studies have reported improved clinical and functional outcomes with surgical management of torn knee ligaments compared with nonsurgical management. Most multiple ligament reconstruction techniques involve creating several tunnels for various reconstruction grafts, posing a risk of tunnel convergence in the proximal tibia. PURPOSE: To assess the risk of tunnel convergence and determine the optimal tunnel placement for the reconstruction of multiple ligaments …

Outcomes After Isolated Medial Patellofemoral Ligament Reconstruction for the Treatment of Recurrent Lateral Patellar Dislocations: A Systematic Review and Meta-analysis.

BACKGROUND: A patellar dislocation is a common knee injury in the young, athletic patient population. Recent trends indicate that the use of long-term nonoperative treatment is decreasing, and surgical intervention is more commonly recommended for those patients who fail initial nonoperative management with recurrent patellar dislocations. Medial patellofemoral ligament (MPFL) reconstruction has become increasingly utilized in this regard. PURPOSE: To evaluate outcomes, particularly return to sports and its relationship to postoperative instability, of isolated MPFL reconstruction for the tre

[Activity of plasminogen activators and plasmin inhibitors in the joint capsule in various articular diseases (author's transl)].

The activity of plasminogen activators and plasmin inhibitors in articular capsules of the hip and knee joints in rheumatoid arthritis, chronic traumatic inflammation, arthrosis and aseptic loosening of prosthesis was determined histochemically and correlated with the degree of mobility in osteoarthrosis of the hip. Microscopically unchanged articular capsules were employed as reference sites. In inflammatory articular diseases the plasminogen activator activity was significantly reduced, whereas it remained unchanged in arthrosis and necrosis of the heas of the femur, and significantly enhanc

Predicted effects of hip distraction on knee ligament mechanics following total knee arthroplasty: A finite element analysis.

BACKGROUND: Hip distraction is essential for hip arthroscopy and other orthopaedic surgeries, but high traction forces often lead to complications like nerve injury and soft tissue impairment. Patients with total knee arthroplasties (TKAs) may face increased risk due to altered joint mechanics; however, the impact of specific TKA configurations on knee behavior during distraction is poorly understood. METHODS: A validated finite element model of the pelvis and lower extremity was developed from patient CT and MRI data. Three configurations were modeled: bi-cruciate retaining (BCR), posterior-

[Graft remodeling following Hertel ACL reconstruction in MR imaging].

MRI and clinical assessment of 21 patients operated on with Hertel method due to knee instability has been presented. Position of the graft in reference to the Blumensaat line has been evaluated in MRI with the use of 3 point scale after mean follow-up of 9-months. Four points scale has been used to grade signal intensity as indicator of graft remodeling. Lysholm scale served for clinical assessment. Mean grade for graft position was 2.3 and for signal intensity 2.7. Better remodeling of the graft correlated positively with its better positioning, but clinical assessment correlated poorly with

Combined Soft Tissue Reconstruction of the Medial Patellofemoral Ligament and Medial Quadriceps Tendon-Femoral Ligament.

Patellar dislocation is a major orthopaedic concern in pediatric and adult populations, with both conservative and surgical options available. Several surgical techniques of the combined or isolated reconstruction of the medial patellofemoral ligament are described in the literature using different grafts, locations, and types of fixations, providing different advantages and disadvantages. New recent developments in cadaveric dissection studies have unveiled the importance of structures that were neglected until recently, such as the medial quadriceps tendon-femoral ligament, which provides a

Modified technique for tibial tubercle elevation with realignment for patellofemoral pain. A preliminary report.

Malalignment of the quadriceps mechanism was corrected and the tibial tuberosity elevated in 17 knees in 16 patients without the use of a free bone graft. The indications for operation were patellofemoral pain unresponsive to conservative treatment, recurrent subluxation or dislocation of the patella, and patients who had had a previous patellectomy with subsequent lateral subluxation of the patella tendon associated with pain. A bone block including the attachment of the patellar tendon is transposed medially to correct the quadriceps angle (Q-angle), elevate the tibial tuberosity, and thereb

Osteochondrosis of the temporomandibular joint presenting as an apparent parotid mass.

A mass in the preauricular area usually indicates the presence of a neoplastic or inflammatory process within the parotid gland. Osteochondrosis is an unusual disease process affecting large joints, particularly the knee. Rarely, the temporomandibular joint (TMJ) may be affected. The disease process involves the synovial lining of the TMJ and is commonly ascribed to a benign neoplastic process or metaplasia, although trauma and inflammation have also been implicated. This process can result in single or multiple loose cartilage bodies (joint mice) within the joint cavity. Symptoms are usually

Functional outcomes after Medial Patellofemoral Ligament Reconstruction show inverted J-shaped relation with Body Mass Index.

PURPOSE: Medial patellofemoral ligament reconstruction (MPFL-R) is a recognised surgical procedure for the treatment of recurrent patellofemoral instability with excellent outcomes reported. The aim of this study is to determine if body mass index can influence these outcomes. METHODS: This is a single-centre retrospective analysis of a longitudinal patellofemoral database. Patients with recurrent patellar instability, without patellar alta or severe trochlear dysplasia were treated using a MPFL-R between 2015 and 2019 at The Robert Jones & Agnes Hunt Orthopaedic Hospital. The clinical effica

Clinical outcomes and predictive factors for failure with MPFL reconstruction combined with tibial tubercle osteotomy and lateral retinacular release for recurrent patellar instability.

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction combined with tibial tubercle osteotomy (TTO) and lateral retinacular release (LRR) is one of the main treatment methods for patellar instability. So far, few studies have evaluated the clinical effectiveness and assessed potential risk factors for recurrent patellar instability. PURPOSE: To report the clinical outcomes of MPFL reconstruction combined with TTO and LRR at least three years after operation and to identify potential risk factors for recurrent patellar instability. METHODS: A retrospective analysis of medical recor
