Mandibular Access Osteotomy for Tumor Ablation: Could a More Tissue-Preserving Technique Affect Healing Outcome?

PURPOSE: Paramedial mandibulotomy facilitates access for the resection of tumors in the oral and oropharyngeal space; however, severe complications related to surgical techniques and radiotherapy have been reported for this procedure. This study evaluated whether preservation of the periosteum during a mandibulotomy would decrease postoperative complications owing to the increased healing capacity provided by preserving more tissue. MATERIALS AND METHODS: Patients who underwent mandibulotomy for surgical tumor ablation from 2007 through 2012 were included in a retrospective controlled cohort

Oral carcinomas detected after extraction of teeth: a clinical and radiographic analysis of 32 cases with special reference to metastasis and survival.

A study was performed of 32 oral carcinomas that were diagnosed after recent dental extraction with particular reference to metastasis and survival. Eighty-four per cent of the patients complained of local swelling and/or pain. Infection had been suspected in 74% of the patients by the referring doctors. The involved teeth had been extracted in all patients. Ulceration and granulomatous tissue suggestive of carcinoma were found in the extraction sites in 24 patients when they were referred to us. Radiographs at presentation, available in 26 patients, showed diffuse radiolucencies with irregula

Reliability of internal jugular vein in oral microvascular reconstruction.

OBJECTIVE: The purpose of this study was to review the reliability of the internal jugular vein when used as a recipient vein in oral microvascular reconstruction. STUDY DESIGN: A retrospective study was done of 36 consecutive cases in which microvascular reconstruction was accomplished with a free flap using the internal jugular vein in an end-to-side fashion. The postoperative patency of 25 internal jugular veins used as recipient veins was examined by postcontrast computed tomography scanning. RESULTS: The overall flap success rate was 97%. Venous thromboses occurred in 2 cases. A common

[Bisphosphonates and osteonecrosis of the jaw--an increasing challenge in palliative care].

Bone metastases are one of the most common complications of cancer. Up to 70% of patients with advanced breast cancer metastasize to the bone. The standard long-term treatment of bone metastases is bisphosphonates. This effective therapy, however, is able to cause serious side effects. Osteonecrosis of the jaw (ONJ) is a recently detected adverse outcome associated in rare cases with bisphosphonate therapy.

Bone-remodeling-related gene expression in oral squamous cell carcinoma: impact on mandibular invasion and overall survival.

OBJECTIVES: Bone invasion by oral squamous cell carcinoma (OSCC) activates the remodeling process with the participation of RANK/RANKL/OPG and Interleukin 6 (IL-6) genes. This study aimed to evaluate gene expressions of RANK, RANKL, OPG, and IL-6 in OSCC associated with mandibular invasion (MI) and to identify their influence on patients' overall survival (OS). MATERIALS AND METHODS: A cross-sectional study selected patients treated with mandibulectomy for OSCC in a reference center, resulting in two hundred and twenty individuals. Forty age-match patients with material for molecular analysis

Osteonecrosis of the jaw after a single bisphosphonate infusion in a patient with metastatic renal cancer treated with sunitinib.

BACKGROUND: Osteonecrosis of the jaw (ONJ) is recognised as an important adverse effect of intravenous bisphosphonates. Recent reports have suggested that antiangiogenic agents may promote the development of this condition. CASE REPORT: We report a case of ONJ occurring within days after the first infusion of zoledronic acid in a patient being treated with sunitinib for metastatic renal cancer. No dental procedure contributed to the occurrence of ONJ. The patient had previously experienced oral mucositis caused by sunitinib. ONJ improved with conventional oral hygiene measures, zoledronic aci

A retrospective study on craniofacial fibrous dysplasia: preoperative serum alkaline phosphatase as a prognostic marker?

BACKGROUND: Craniofacial fibrous dysplasia (CFD) often requires surgery to correct facial deformity and prevent functional impairment. However, recurrence is common, and there is no reliable prognostic biomarker. The aim of this paper is to evaluate the possibility of using preoperative alkaline phosphatase (ALP) as a prognostic marker for CFD. MATERIAL AND METHODS: Forty-nine patients with CFD who underwent surgery from 2000 to 2011 were selected. The relationship between preoperative ALP and age, gender, lesion type and prognosis was investigated. RESULTS: The recurrence rate was 31.8% in

Bisphosphonate-induced osteonecrosis of the jaws: a case report and literature review.

Bisphosphonates are nonmetabolized analogues of pyrophosphate that are used for treating skeletal complications secondary to bone metastases. These drugs are capable of localizing to bone and inhibiting osteoclastic activity; however, their exact mechanisms of action remain unclear. Recently, it has been documented that administration of bisphosphonates has been associated with osteonecrosis of the jaws. This article details the case of a woman who received intravenous administration of bisphosphonate for metastatic breast cancer and subsequently developed osteonecrosis in both the maxillary a

First presentation of a frameshift mutation in the SETD2 gene of a juvenile psammomatoid ossifying fibroma (JPOF) associated with an aneurysmal bone cyst.

BACKGROUND: The rarity of juvenile psammomatoid ossifying fibroma (JPOF) and lack of cytogenetic studies prompted us to report a novel SETD2 gene mutation in a benign odontogenic tumour. CASE PRESENTATION: A 21-year-old man presented with a hard, expanded mandibular cortex. Computed tomography revealed multilocular radiopacity in the mandible; this was reconstructed via segmental mandibulectomy using a vascularised iliac crest flap. Based on the clinical and histological findings, we diagnosed JPOF associated with an aneurysmal bone cyst. Microscopically, the solid area was characterised by m

Ameloblastic Fibro-Odontoma.

The ameloblastic fibro-odontoma (AFO) is a rare, mixed odontogenic tumor exhibiting the histological characters of the ameloblastic fibroma and complex odontoma. It is comprised of proliferating ectodermal and mesenchymal components of odontogenic tissue as well as enamel and dentin. AFO normally presents as an asymptomatic swelling of the posterior maxilla or mandible and is usually associated with developing teeth, occurring predominantly in children and adolescents. Such lesions are generally found upon radiographic examination of patients whose tooth eruption is delayed. This lesion often

Stage 0 osteonecrosis of the jaw in a patient on denosumab.

Osteonecrosis of the jaws (ONJ) is a complex disease involving multiple tissue and cell-type responses to wound healing or infection. AAOMS defines bisphosphonate related ONJ (BRONJ) as exposed, necrotic bone in the maxillofacial region that has persisted for more than 8 weeks in a patient with current or previous antiresorptive treatment, without a history of radiation therapy to the jaws. Since the first reported ONJ cases in 2003 and 2004, there has been little advancement in understanding the etiology and pathophysiology of ONJ. Many hypotheses have been proposed, including bisphosphonate

Unicystic ameloblastoma mimicking apical periodontitis: a case series.

INTRODUCTION: The purpose of this study was to describe a series of unicystic ameloblastomas (UAs) that presented clinical and radiographic similarities to apical periodontitis in order to call the attention of clinicians to the fact that several different diseases are able to mimic endodontic periapical lesions. METHODS: A retrospective clinicopathological analysis of 5 patients who had periapical lesions in the mandible initially diagnosed as apical periodontitis but with a final histologic diagnosis of UAs is presented. RESULTS: Three men and 2 women with a mean age of 26.4 years presente

Metastatic carcinoma presenting as neuralgia involving the trigeminal nerve.

Breast carcinoma is the leading cause of cancer death in women. Metastatic tumours of the oral cavity arising from breast cancer (and other forms of cancer) may be similar to hyperplastic or reactive lesions. They are uncommon and typically appear when various organs have already been affected by the neoplasia. A 53-Year-old woman presented with "discomfort" in her left mandible, specifically, swelling of recent onset accompanied by progressive loss of sensation. The mandibular swelling had been preceded by neuralgia involving the trigeminal nerve. Biopsy and histologic examination of the intr

Osteochondroma of the mandibular condyle: surgical excision followed by occlusal stabilization.

Osteochondroma of the mandibular condyle in adults can be treated by surgical excision, condylectomy followed by costochondral graft or orthognathic surgery. Such complex treatment plan may not be appropriate for patients with old age, affected with chronic osteochondroma of the condyle. In this clinical report, we present a patient with osteochondroma of the condyle treated by surgical excision. The patient's postoperative occlusion was a contraindication for orthognathic surgery because of the severe abrasion of the teeth and the chronic compensation of the dentition to the deviated mandible

Not all radiolucencies of the jaw require enucleation: a case of brown tumour.

We describe a case of brown tumour from primary hyperparathyroidism, which presented with radiolucency in the jaw. It was treated by parathyroidectomy, which resulted in complete resolution of the lytic lesion without any surgery to the jaw. It is important to be aware of endocrine causes of a common radiological sign to avoid unnecessary local surgical intervention.
