Death during transforaminal epidural steroid nerve root block (C7) due to perforation of the left vertebral artery.

Treatment for individuals suffering from migraines and pain due to an inflammation or impingement of a nerve range from noninvasive methods such as massage, physical therapy, and medications to invasive methods such as epidural steroid injections and surgery. Each method of treatment has an associated level of risk. While minor to moderate complications from such procedures do occur, deaths are very rare. We report the first cited case of a death associated with the pain management procedure called nerve root block, also referred to as a transforaminal epidural steroid injection. We present th

The use of hydrocoil for parent artery occlusion.

The HydroCoil Embolic System (HES) comprises a platinum microcoil coated with an expansile polymer and is approved by the United States Food and Drug Administration for cerebral aneurysm occlusion. We report a series of five cases, two in the internal carotid artery and three in the vertebral artery, in which the HES was used for parent artery occlusion procedures. In each of these cases, the HES allowed rapid, short-segment occlusion.

Benign arterial dissections of the posterior circulation.

Four young adults with spontaneous dissection of the vertebrobasilar system are reported. Clinically, two patients presented with subarachnoid hemorrhage and two with brain-stem ischemia. In two cases of ruptured arterial dissection of the posterior cerebral artery, angiography demonstrated fusiform and "sausage-like" dilatation of the involved vessel. In two cases of occlusive dissection of the basilar artery, angiography revealed the typical "string sign." All four patients were treated conservatively: three survive in good clinical condition and one remains disabled. Follow-up angiograms sh

Far Lateral Approach (Transcondylar, Transtubercular) for Bypass and Trapping of a Ruptured, Dissecting PICA Aneurysm.

Aneurysms of the posterior circulation pose a unique challenge due to higher rupture rates, higher recurrence rates following endovascular treatment, and extended open cranial base approaches required to reach the ventrally located brainstem circulation. While endovascular therapy has made tremendous strides in successful treatment for most posterior circulation aneurysms, open microscopic approaches remain essential in specific circumstances. Here, we present a case of a patient who presented with acute, severe headache, and sixth nerve palsies, and who was found to have hydrocephalus and a d

Imaging features of vertebral artery fenestration.

PURPOSE: Vertebral artery fenestration (VAF) is a rare congenital vascular anomaly which has been associated with intracranial aneurysm. VAF can share some similar imaging features with vertebral artery dissection (VAD), which may confound diagnosis of the latter on CT and MR angiography. The purpose of this investigation is to assess the prevalence of VAF, evaluate its association with other vascular anomalies, and identify imaging features to help in distinguishing VAF and VAD. METHODS: Using keyword search on CTA and MRA head and neck imaging reports from 2010 to 2017, cases of VAF and VAD…

Stent retriever angioplasty for acute atherosclerotic occlusion of vertebral artery.

The established effectiveness of mechanical thrombectomy using a stent retriever or aspiration catheter for emergent large-vessel occlusion caused by cardiogenic embolic stroke is widely recognized. However, in cases of acute artery occlusion resulting from atherosclerotic disease, mechanical thrombectomy often encounters challenges in achieving consistent recanalization, and aggressive percutaneous transluminal angioplasty (PTA) with a balloon and/or stenting can potentially lead to arterial dissection or additional perforator infarction. We present the case of an 88-year-old man who experien

Hybrid Approach to a Right Aortic Arch With a Large Kommerell's Diverticulum.

A 60-year-old woman with a family history of aortic dissection in her mother was investigated for dysphagia. Her scan showed a right aortic arch with a large Kommerell's diverticulum causing esophageal compression. Her left vertebral artery, which originated abnormally low in the thorax, was embolized preoperatively to limit operative difficulties. Two weeks later, a right thoracotomy allowed the resection of the aneurysm with an associated left carotid-axillary bypass. Partial cardiopulmonary bypass in moderate hypothermia with beating-heart perfusion of the brain was conducted. Five-year fol

Cerebellar border zone infarcts are often associated with presumed cardiac sources of ischaemic stroke.

It has been suggested that most border zone cerebellar infarcts are embolic infarcts or infarcts due to hypercoagulatble states. The aim of this study was to test this hypothesis. Risk factors for the presumed mechanism of stroke (TOAST criteria) were studied in 14 consecutive patients (nine men, five women; age range 29-84 years) with a total of 17 border zone cerebellar infarcts. The presumed cause of stroke was "cardioembolism" in nine patients. Three patients had a dissection of the vertebral artery. Two patients had a negative diagnostic investigation, and one had a cardiac arrest. These

Cervicocranial fibromuscular dysplasia in Taiwanese ischemic stroke patients.

INTRODUCTION: Clinical research of cervicocranial fibromuscular dysplasia (FMD) is rare in Asian populations. Our study reviewed Taiwanese ischemic stroke patients with cervicocranial FMD and compared them with previous reports. METHODS: Between 2000 and 2011, we collected 19 consecutive cervicocranial FMD patients who received demographic registration, a blood test for excluding vasculitis, and comprehensive angiography. Cerebral ultrasound, vascular images and clinical outcomes (Barthel index, modified Rankin scale, recurrent stroke, or death) were monitored during follow-up. RESULTS: Of t

The critical role of non-contrast chest CT in avoiding thrombolysis catastrophe: A case of acute aortic dissection masquerading as ischemic stroke.

INTRODUCTION AND IMPORTANCE: Aortic dissection (AD) complicating acute ischemic stroke poses significant therapeutic challenges, particularly regarding thrombolytic contraindications that may substantially increase mortality risk. This underscores the critical need for expedited differential diagnosis in such clinical scenarios. CASE PRESENTATION: An 80-year-old male presented with acute-onset left-sided hemiplegia and unresponsiveness. CLINICAL DISCUSSION: Initial cerebral neuroimaging demonstrated a substantial ischemic penumbra (136.2 ml Tmax >6 s) in the right middle cerebral artery (MCA

Mild hyperhomocyst(e)inemia: a possible risk factor for cervical artery dissection.

BACKGROUND AND PURPOSE: The pathogenesis of cervical artery dissection (CAD) remains unknown in most cases. Hyperhomocyst(e)inemia [hyperH(e)], an independent risk factor for cerebrovascular disease, induces damage in endothelial cells in animal cell culture. Consecutive patients with CAD and age-matched control subjects have been studied by serum levels of homocyst(e)ine and the genotype of 5,10-methylenetetrahydrofolate reductase (MTHFR). METHODS: Twenty-six patients with CAD, admitted to our Stroke Unit (15 men and 11 women; 16 vertebral arteries, 10 internal carotid arteries), were compar

Typology and formation of the arterial circle at the base of the brain in neonatal Saanen goats.

Diseases affecting the central nervous system of domestic animals account for a substantial proportion of the conditions encountered in clinical and surgical practice involving production animals. While the cerebral vascular anatomy is well characterized in humans; however, detailed information remains limited for many mammalian species, particularly regarding the origin of the basilar artery and the contribution of the vertebral artery. The study of the arteria distribution responsible for cerebral irrigation in domestic animals and species used as experimental models, from a phylogenetic per

CT versus MR Techniques in the Detection of Cervical Artery Dissection.

BACKGROUND: Spontaneous cervical artery dissection (sCAD) is an important etiology of juvenile stroke. The gold standard for the diagnosis of sCAD is convential angiography. However, magnetic resonance imaging (MRI)/MR angiography (MRA) and computed tomography (CT)/CT angiography (CTA) are frequently used alternatives. New developments such as multislice CT/CTA have enabled routine acquisition of thinner sections with rapid imaging times. The goal of this study was to compare the capability of recent developed 128-slice CT/CTA to MRI/MRA to detect radiologic features of sCAD. METHODS: Retrosp
