The effects of changing position and angle of the proximal segment after intraoral vertical ramus osteotomy.

The authors evaluated changes in position and angle of the proximal segment, including the condyle, after intraoral vertical ramus osteotomy (IVRO) with and without a Le Fort I osteotomy to verify whether displacement of the proximal segment could induce postoperative complications. Changes in condylar angle, ramus angle, and displacement of proximal segment were measured pre- and postoperatively. The position of the temporomandibular joint (TMJ) disc was examined pre- and postoperatively. Trigeminal nerve hypoesthesia in the lower lip was assessed bilaterally. The postoperative horizontal con

Changes in overnight arterial oxygen saturation after mandibular setback.

Mandibular setback reduces space in the pharyngeal airway, and it has been suggested that it might induce sleep-disordered breathing. We report on its effects on space in the pharyngeal airway and respiratory function during sleep. We studied 78 patients (29 men and 49 women) in whom skeletal class III malocclusions had been corrected. The mean (range) age at operation was 24 (16-38) years and body mass index (BMI) 21.4 (16.1-30 .9)kg/m(2). Morphological changes were evaluated on lateral cephalograms taken three times: preoperatively, a few days postoperatively, and more than 6 months postoper

The Effect of Mandibulectomy and Mandibulotomy on Cochlear Function.

Mandibulectomy and mandibulotomy procedures are performed with the oscillating saw and the acoustic energy generated during the osteotomies is transferred to the cochlea via temporomandibular joint and air conduction. The aim of this study was to assess the effect of mandibulectomy and mandibulotomy on cochlear function. This study was carried out on 15 patients who underwent at an otolaryngology department of a tertiary medical center between January 2013 and August 2015. The study was designed as a prospective study. All of the data were prospectively collected. These included demographic da

An excessive coronoid hyperplasia with suspected traumatic etiology resulting in mandibular hypomobility.

There are multiple theories as to the causes of coronoid process hyperplasia of the mandible, including trauma, temporalis muscle hyperactivity, hormonal stimulus, and genetic inheritance. The excess growth of the coronoid process can cause impingement on the zygomatic processes and may result in mandibular hypomobility. A case of an excessive unilateral coronoid hyperplasia with suspected traumatic etiology, which was successfully treated by coronoidectomy and postoperative physiotherapy, is presented. The patient was a 21-year-old man whose maximum mouth opening was 23 mm. The attachments of

Assessment, documentation, and treatment of a developing facial asymmetry following early childhood injury.

Prepubertal trauma is often implicated as the cause of asymmetric growth of the mandible. A series of photographs taken before and after early childhood injury to the orofacial complex illustrates the development of a three-dimensional dentofacial deformity in a patient. The diagnosis and combined surgical orthodontic treatment plan to correct the facial asymmetry and malocclusion are discussed.

Modified maxillomandibular advancement for obstructive sleep apnoea: towards a better outcome for Asians.

Excluding tracheostomy, maxillomandibular advancement (MMA) is the most effective surgical treatment for obstructive sleep apnoea (OSA). However, the anticipated facial changes may prevent acceptance of this procedure by patients with bimaxillary protrusion, a common feature of Asian faces. We therefore developed a modified MMA technique for such cases, consisting of anterior segmental osteotomies together with standard Le Fort I and bilateral sagittal split osteotomies. A prospective study of 20 consecutive Taiwanese adults with moderate-to-severe OSA who underwent modified MMA and postsurgic

Microsurgical reconstruction for radiation- and bisphosphonate-induced mandible osteonecrosis based on patient-specific physiopathologic mechanisms.

BACKGROUND: Radiation- and bisphosphonate-induced mandible osteonecrosis have distinct underlying physiopathologic mechanisms, but both can constitute a serious problem and lead to functional impairment and facial disfigurement. METHODS AND RESULTS: We describe different clinical situations related to several grades of osteonecrosis, where different options of free transfer should be considered based on case-specific physiopathologic mechanisms. We propose a simple clinical grading system to predict the best treatment option. CONCLUSIONS: For patients with advanced refractory disease, pathol

Temporomandibular joint bilateral post-traumatic ankylosis: a report of a case treated with interpositional arthroplasty.

Ankylosis of the temporomandibular joint (TMJ) is a severe disorder which leads to jaw function impairment and restricted mouth opening. The surgical approach to TMJ ankylosis can be performed according to different techniques. The present paper is a report of a bilateral post-traumatic case of TMJ ankylosis. The patient suffered bilateral condylar fractures as a consequence of a road accident and showed a limited mouth opening (22 mm) along with dental occlusion abnormalities. Conservative treatments were uneffective to improve jaw impairment, so a computerized tomography was requested for su

Effect of the rs7858836 Single-Nucleotide Polymorphisms of the ASTN2 Gene on Pain-Related Phenotypes in Japanese Women Who Underwent Laparoscopic Gynecologic Surgery.

BACKGROUND: In our previous study, the minor T allele of the rs7858836 C/T single-nucleotide polymorphism (SNP) in the ASTN2 gene, which encodes astrotactin 2, was associated with reduced fentanyl requirements after laparoscopic-assisted colectomy and after mandibular osteotomy. In this study, we investigated the effects of this SNP on pain-related phenotypes in patients who underwent laparoscopic gynecologic surgery (LGS). METHODS: We studied 333 Japanese women, 21-69 years, who underwent LGS at Juntendo University Hospital between 2017 and 2019. We evaluated associations between SNP genotyp…
