Survey of UK speech and language therapists' assessment and treatment practices for people with progressive dysarthria.

BACKGROUND: Dysarthria knowledge is predominantly impairment-based. As a result, speech and language therapists (SLTs) have traditionally adopted impairment-focused management practices. However, guidance for best practice suggests that SLTs should consider the client holistically, including the impact of dysarthria beyond the impairment. AIMS: To investigate the current assessment and treatment practices used by UK SLTs with clients with progressive dysarthria and to identify whether these satisfy the needs of SLTs in their everyday practice. To investigate the extent to which they consider

A speech-to-video synthesis approach using spatio-temporal diffusion for vocal tract MRI.

Understanding the relationship between vocal tract motion during speech and the resulting acoustic signal is crucial for aided clinical assessment and developing personalized treatment and rehabilitation strategies. Toward this goal, we introduce an audio-to-video generation framework for creating Real Time/cine-Magnetic Resonance Imaging (RT-/cine-MRI) visuals of the vocal tract from speech signals. Our framework first preprocesses RT-/cine-MRI sequences and speech samples to achieve temporal alignment, ensuring synchronization between visual and audio data. We then employ a modified stable d

Movement cognition and narration of the emotions treatment versus standard speech therapy in the treatment of children with borderline intellectual functioning: a randomized controlled trial.

BACKGROUND: Borderline intellectual functioning (BIF) is defined as a "health meta-condition… characterized by various cognitive dysfunctions associated with an intellectual quotient (IQ) between 71 and 85 which determines a deficit in the individual's functioning both in the restriction of activities and in the limitation of social participation". It can be caused by many factors, including a disadvantaged background and prematurity. BIF affects 7-12% of primary school children that show academic difficulties due to poor executive functioning. In many children with BIF, language, movement and

Cognitive level and adaptive behaviour in the Klippel-Trenaunay-Weber Syndrome. An example of the potentials of an early intervention model applied to a complex pathology.

PURPOSE: The Klippel-Trenaunay-Weber syndrome is a rare, congenital disorder characterized by benign excessive growth of blood vessels on the skin. Little is still known about its cognitive development and adaptive behaviour functioning. CASE STUDY: This case study describes the clinical history of a female child suffering from this rare syndrome and admitted to our Pediatric Unit in order to provide motor rehabilitation, speech therapy and psychoanalytic psychotherapy following an early intervention model program. After five years of treatment her clinical picture witnessed a considerable im

Perception of speech and nonspeech stimuli by children with and without reading disability and attention deficit hyperactivity disorder.

The auditory temporal deficit hypothesis predicts that children with reading disability (RD) will exhibit deficits in the perception of speech and nonspeech acoustic stimuli in discrimination and temporal ordering tasks when the interstimulus interval (ISI) is short. Initial studies testing this hypothesis did not account for the potential presence of attention deficit hyperactivity disorder (ADHD). Temporal order judgment and discrimination tasks were administered to children with (1) RD/no-ADHD (n=38), (2) ADHD (n=29), (3) RD and ADHD (RD/ADHD; n=32), and (4) no impairment (NI; n=43). Contra

A two-year-old boy with language regression and unusual social interactions.

Jimmy, a 2 1/2-year-old boy, was seen for the first time by a new pediatrician after a recent family move. His mother made the appointment for a health supervision visit although she had concerns about his language and social skills. She stated that he spoke primarily with unintelligible sounds and often communicated by pointing with his finger. He spoke only 10 words that were clear enough to be understood. Jimmy's mother said that he could hear, but she was not sure whether he understood everything she said. Although he played at home with his 4-year-old brother, he typically played by himse

The contribution of central auditory factors to auditory disability.

Auditory disability increases with both hearing threshold level and age. It is often suggested that some or most of the age effect in auditory disability is underpinned by deficits in central auditory function. A sample of 240 individuals aged between 50 and 75 years was examined to provide a balance across the major variables of hearing level and age. The central auditory indices investigated were: the binaural masking difference, the effects of dichotic competition on the staggered spondaic word tests, the binaural advantage for dichotically presented words, the binaural advantage for dichot

Dermal sling as an alternative to palmaris longus tendon suspension in radial forearm flap lower lip reconstruction: A case report.

BACKGROUND: Lower lip reconstruction with the radial forearm free flap (RFFF) requires reliable static suspension to maintain oral competence. The palmaris longus tendon (PLT) is commonly used for this purpose, but its absence limits applicability. We describe a dermal sling harvested from the RFFF as an alternative suspension method. Case presentation After total lower lip resection for squamous cell carcinoma, reconstruction was performed using an RFFF. A full-thickness dermal strip was harvested from the proximal flap, deepithelialized, and used as a non-tunneled, onlay suspension sling anc

Motor symptoms of Parkinson's disease: critical markers for early AI-assisted diagnosis.

Parkinson's disease is a prevalent neurodegenerative disorder, where early diagnosis is essential for slowing disease progression and optimizing treatment strategies. The latest developments in artificial intelligence (AI) have introduced new opportunities for early detection. Studies have demonstrated that before obvious motor symptoms appear, PD patients exhibit a range of subtle but quantifiable motor abnormalities. This article provides an overview of AI-driven early detection approaches based on various motor symptoms of PD, including eye movement, facial expression, speech, handwriting,

Intra-arterial contrast administration: a confounding appearance on stroke CT imaging.

A female patient in her 70s presented with mild slurred speech. Clinical assessment revealed only minor dysarthria. A stroke protocol CT study was performed comprising a non-contrast CT head, CT angiogram (CTA) and CT perfusion (CTP) imaging. Despite a normal non-contrast CT head, CTP imaging demonstrated a large left anterior circulation territory abnormality compatible with core infarct. The CTA displayed absent left middle cerebral artery opacification and an abnormal pattern of thalamic and posterior fossa structure enhancement. Following a neuroradiology review, it was identified that the

Using deep learning to identify the recurrent laryngeal nerve during thyroidectomy.

Surgeons must visually distinguish soft-tissues, such as nerves, from surrounding anatomy to prevent complications and optimize patient outcomes. An accurate nerve segmentation and analysis tool could provide useful insight for surgical decision-making. Here, we present an end-to-end, automatic deep learning computer vision algorithm to segment and measure nerves. Unlike traditional medical imaging, our unconstrained setup with accessible handheld digital cameras, along with the unstructured open surgery scene, makes this task uniquely challenging. We investigate one common procedure, thyroide
