PURPOSE: Goniotomy is a surgical treatment of choice to treat primary infantile glaucoma. Goniotomy has been studied in vitro in animal and human cadaver eyes with several lasers. The objective of this study was to investigate the functional and morphologic effectiveness of goniotomy with the free electron laser in comparison with conventional needle goniotomy in early congenital glaucoma rabbits.

Primary open angle glaucoma may be a severe ocular disease and may lead to blindness if not diagnosed and/or treated in time. The pathogenic mechanism is a progressive loss of ganglion cells leading to cupping of the optic nerve head. The main risk factor is an elevated intraocular pressure, but optic nerve vascular deficiency, neuronal degeneration or genetic factors have to be considered as other potential risk factors. The actual diagnostic tools and the latest technologies for the medical, laser and surgical treatment are described in this paper.

OBJECTIVE: To investigate the method of localization of Schlemm's canal during trabeculotomy to evaluate the successful rate of trabeculotomy.
METHOD: The author treated 38 cases (46 eyes) of congenital glaucoma with trabeculotomy. Harms trabeculotomy knife was introduced into the Schlemm's canal through the cutting end of an external collecting channel. The follow-up period was over 6 months in 37 eyes.

The long-term results of 180 degrees argon laser trabeculoplasty (ALT) were studied in 61 eyes of 43 patients with uncontrolled primary open … urgery. A comparable pressure control was also attained with those eyes that had undergone previous trabeculectomy. Failures (26.3%) all occurred in the 1st year after treatment. The results and conclusions from our study are compared with those reported in the literature. The different theories regarding the mechanisms of action of ALT are discussed.
