Dermoid tumours in children usually occur in two locations: at the anterior fontanelle and on the occipital squama. An exceptional site of origin for a posterior fossa dermoid cyst is the extradural space. There are only six previous cases of this situation reported in the literature. A series of 103 subscalp and calvarial masses in children were reviewed and three children are reported with extradural dermoids of the posterior fossa, which communicated with the skin through midline occipital dermal sinuses. All three children were seen after the rapid growth or the formation of an abscess in

PURPOSE: The blood-supplying state of skull base chordomas has been scarcely investigated. We aimed to elucidate the role and superiority of CT perfusion (CTP) in predicting the tumor blood-supplying state to guide surgical strategy. METHODS: Between 2023 and 2024, 79 consecutive patients with skull base chordoma treated at Beijing Tiantan Hospital were retrospectively identified, and their clinical, imaging, and pathological data were analyzed. We further attempted to establish a grading system for the tumor blood-supplying state. Differences among blood-supplying state groups were assessed,

BACKGROUND: Both endonasal and transoral approaches can provide minimally invasive access to the infratemporal fossa (ITF); however, the appropriate indications for each corridor have not been sufficiently defined. This study aimed to explore the indication for selection of an endonasal or a transoral approach to the ITF.
METHODS: Cadaveric dissection was performed on 6 specimens, one side for an endonasal approach and the contralateral side for a transoral approach. A complementary subdivision of the ITF into four quadrants (A-D) based on the crossing line at the lower border of the lateral

BACKGROUND: Based on our experience with the endoscopic endonasal approach (EEA) and transorbital approach (TOA) for middle cranial fossa tumors, we evaluated the efficacy and limitations of visualization and the clinical outcomes associated with the approaches depending on the surgical corridors. In addition, we determined the optimal strategy for each approach.
METHODS: Between September 2015 and May 2018, we retrospectively reviewed clinical outcomes in 21 patients who underwent minimally invasive endoscopic surgery for middle cranial fossa tumors involving the cavernous sinus. We classifi

BACKGROUND: Protecting the venous drainage route during surgery in cases of petroclival meningioma (PCM) is important. Identifying venous congestion preoperatively can be valuable in reducing the risks associated with venous congestion during surgery. In this study, we examined the utility of susceptibility-weighted imaging (SWI) in identifying the presence of venous congestion in PCM cases preoperatively and identified the factors associated with it.
METHODS: We retrospectively examined 24 patients who had undergone surgery for primary PCM. The areas of the basal and internal cerebral veins

The authors presented our institutional experience with skull base reconstruction techniques and developed a stratified algorithm for different causes of cerebrospinal fluid (CSF) leak. This is a retrospective review of patients who were diagnosed as CSF leak treated with skull base reconstruction or who underwent endoscopic transsphenoidal surgery for sellar and parasellar lesions at our department from August 2012 to April 2017. The authors totally identified 57 (59 operations in total) patients who were divided into 3 groups according to the causes of CSF leak and different reconstruction t

BACKGROUND: Intraoperative neurophysiologic monitoring of the extraocular cranial nerve (EOCN) is not commonly performed because of technical difficulty and risk, reliability of the result and predictability of the postoperative function of the EOCN.
METHODS: We performed oculomotor nerve (CN III) and abducens nerve (CN VI) intraoperative monitoring in patients with skull base surgery by recording the spontaneous muscle activity (SMA) and compound muscle action potential (CMAP). Two types of needle electrodes of different length were percutaneously inserted into the extraocular muscles with t

OBJECTIVE: The surgical approach to lesions involving the inferior petrous apex (IPA) is still challenging. The purpose of this study is to demonstrate the anatomical features of the IPA and to assess the applicability of an endoscopic endonasal approach through the foramen lacerum (translacerum approach) to the IPA.
METHODS: The surgical simulation of the endoscopic endonasal translacerum approach was conducted in 3 cadaver heads. The same technique was applied in 4 patients harboring tumors involving the IPA (3 chordomas and 1 chondro-sarcoma).
RESULTS: By removing the fibrocartilaginous c

INTRODUCTION: Schwannoma of the vagal nerve are rare and usually accidentally found by exploring a slowly growing and painless neck tumor.
CASE: We report on a 49-year-old patient with an extensive schwannoma of the vagal nerve. The tumor grew from the brainstem to the lower neck vessel sheath and destroyed the skull base. The tumor had already involved all nerves of the foramen jugulare. The surgery was performed in cooperation with the neurosurgeons. Initially the cervical part of the tumor was removed by transcervical approach. Then the endocranial part of the tumor was removed by retrosig

The author reviewed 34 years of personal experience with inpatients in a large municipal hospital to analyze the seats and causes of involvement of single pairs of cranial nerves. Among 578 cases, the sixth (n = 234) and second (211) nerves predominated, followed by the fourth (48), seventh (30), third (27), and eighth (18) cranial nerves. Trauma (99), infection (94), tumor (92), increased intracranial pressure (85), vascular disease (74), and demyelination (66) were common causes.

OBJECTIVE: Tumours or vascular lesions of the clivus and juxtaclival region present a unique challenge to neurosurgeons and a variety of techniques, with a wide spectrum of complexity, have been advocated. This report presents the use of a conservative transpetrosal approach which combines partial removal of the postero-lateral petrous bone with preservation of the labyrinth, with particular focus on efficacy and the reduction of complications.
PATIENTS AND METHODS: Over 9 years, 52 patients underwent a conservative combined supra-infratentorial, labyrinth-preserving transpetrosal approach. T

The anesthetic management of complex skull base surgical procedures provides unique problems and concerns for the neuroanesthesiologist. Positioning to access the skull base could put the patient at risk for peripheral nerve injury and some of the positions may increase the risk for air emboli. In addition, tumor pathology and involvement with vital structures could increase the chances for substantive blood loss, destruction of associated nerves or vessels, and may require temporary occlusion of the carotid artery necessitating intraoperative neuroprotection. Neurophysiological monitors may a

The choice of the surgical approach and operative technique for the management of cerebrospinal fluid (CSF) fistulas of the anterior cranial fossa are still a controversially discussed topic. Although "extracranial" approaches through the paranasal sinuses are becoming increasingly more popular among otolaryngologists and maxillo-facial surgeons, most neurosurgeons traditionally prefer the "intracranial" repair of CSF fistulas by a craniotomy. We present an approach through the frontal sinus for the repair of dural defects behind the posterior wall of the frontal sinus and at the floor of the

Re-irradiation can offer a potentially curative solution in case of progression after initial therapy; however, a second course of radiotherapy can be associated with an increased risk of severe side-effects. Particle therapy with protons and especially carbon ions spares surrounding tissue better than most photon techniques, thus it is of high potential for re-irradiation. Irradiation of tumors of the brain, head and neck and skull base involves several delicate risk organs, e.g. optic system, brainstem, salivary gland or swallowing muscles. Adequate local control rates with tolerable side-ef

A case of metastasis to the sella turcica from a follicular adenocarcinoma of the thyroid gland is presented. Metastasis to this site is rare and review of the literature reveals only 12 cases of metastatic thyroid carcinoma involving the sella turcica and pituitary gland. The optimal treatment strategy is still to be determined. A 43-year-old woman presented with headache, nausea, visual impairment and galactorrhea. An MRI scan of the cranium revealed an enhancing destructive sellar lesion. The patient underwent transsphenoidal removal of the lesion to alleviate visual loss. The histological

BACKGROUND: Tentorial meningiomas are notorious for their critical location. Selection of a suitable approach that exposes the multicompartmental growth of tumor is important for a complete and safe resection. This paper discusses about various operative approaches and their overall surgical outcome.
METHODS: We retrospectively reviewed our 41 patients with tentorial meningiomas. They were classified according to the modified Yasargil classification. The symptomatic improvement and progression-free survival (PFS) were analyzed at follow-up.
RESULTS: Tumors were almost equally distributed in
