The Central Role of HLA Class II-Restricted Helper Neoantigen Vaccines in Cancer Immunotherapy.

The discovery of neoantigens and their application in cancer vaccines have brought about a paradigm shift in cancer immunotherapy, similar to the way in which immune checkpoint inhibitors redefined therapeutic strategies for cancer treatment. Early neoantigen vaccine approaches primarily focused on eliciting HLA class I-restricted CD8+ cytotoxic T lymphocyte (CTL) responses, whereas it has become clear that such strategies alone may be insufficient to establish durable and effective antitumor immunity. Increasing attention has therefore shifted toward HLA class II-restricted neoantigens ("help…

Neoantigen-driven personalized vaccines for breast cancer: Platforms, nano-delivery systems, and combination strategies.

Breast cancer (BC) is the most common malignancy among women globally. Despite significant advances, therapeutic resistance and tumor recurrence remain major clinical challenges. Personalized cancer vaccines have recently emerged as a promising strategy to induce durable, highly specific anti-tumor immune responses and improve long-term survival. However, challenges persist in neoantigen prediction, vaccine formulation, and practical implementation, which can hinder the overall effectiveness and widespread adoption of these personalized cancer vaccines. Ongoing technological advancements are e…

Unusually long remission in two patients with follicular lymphoma receiving immunomodulating therapeutic cancer vaccines.

The immune checkpoints programmed death-ligand (PD-L)1 and PD-L2 are expressed in follicular lymphoma (FL), yet immune checkpoint blocking antibodies have failed in FL. We previously initiated a phase I immune modulating vaccination (IMV) trial targeting PD-L1 and PD-L2 in patients with advanced follicular lymphoma. The study suggested a clinical effect of the vaccine, however due to relatively short follow-up time and a heterogeneous patient population no clear conclusions could be made after the trial. In this study we present follow-up data on the eight patients included in the trial. Notab

Therapeutic vaccines targeting HPV epitopes in human papillomavirus - positive oropharyngeal cancer: a critical review.

Human papillomavirus-positive (HPV+) oropharyngeal squamous cell carcinoma (OPSCC) is a peculiar entity, with distinct patient, tumor, and biological characteristics, and a different prognosis compared to HPV-negative (HPV-) OPSCC. Due to the rising incidence, there is a need to develop novel therapeutic approaches, especially considering the long-term morbidities of traditional treatments such as surgery and chemoradiotherapy. In this regard, therapeutic vaccines targeting HPV epitopes have been put at the forefront of the immunotherapeutic strategies for HPV+ OPSCC. Multiple clinical trials

A Rationally Engineered Spleen-Tropic One-Component Lipid-mRNA Complex (OncoLRC) for Cancer Vaccines.

mRNA vaccines offer great potential for cancer immunotherapy, yet efficient delivery of antigen-encoding mRNA to antigen-presenting cells (APCs) in lymphoid organs remains a significant challenge. Here, OncoLRC is introduced, a rationally engineered, spleen-tropic, one-component lipid-mRNA complex that selectively delivers mRNA to splenic APCs following systemic administration. Through a systemic screening and optimization process, a dimethylamino (DMA)-lipidoid-based OncoLRC formulation that achieves nearly exclusive spleen-targeting mRNA delivery, outperforming its conventional four-componen

GRP-based vaccines as a novel approach in cancer immunotherapy: mechanisms, challenges, and prospects.

Glucose-regulated proteins (GRPs), key members of the heat shock protein (HSP) family, function as molecular chaperones that are upregulated … the cell surface or secreted. As a result, these findings have led to the development of GRP-based cancer vaccines that can elicit robust adaptive immune responses by chaperoning tumor antigens to antigen-presenting cells. Current evidence suggests GRP75 (HSPA9/mortalin) is less immediately tractable than ER-resident GRPs (78/94/170) for vaccine design, primaril …

Alpha-galactosylceramide improves the potency of mRNA LNP vaccines against cancer and intracellular bacteria.

Although various types of mRNA-based vaccines have been explored, the optimal conditions for induction of both humoral and cellular immunity remain rather unknown. In this study, mRNA vaccines of nucleoside-modified mRNA in lipoplexes (LPXs) or lipid nanoparticles (LNPs) were evaluated after administration in mice through different routes, assessing mRNA delivery, tolerability and immunogenicity. In addition, we investigated whether mRNA vaccines could benefit from the inclusion of the adjuvant alpha-galactosylceramide (αGC), an invariant Natural Killer T (iNKT) cell ligand. Intramuscular (IM)

Live-attenuated lymphocytic choriomeningitis virus-based vaccines for active immunotherapy of HPV16-positive cancer.

Infection with human papillomavirus (HPV) is associated with a variety of cancer types and limited therapy options. Therapeutic cancer vaccines targeting the HPV16 oncoproteins E6 and E7 have recently been extensively explored as a promising immunotherapy approach to drive durable antitumor T cell immunity and induce effective tumor control. With the goal to achieve potent and lasting antitumor T cell responses, we generated a novel lymphocytic choriomeningitis virus (LCMV)-based vaccine, TT1-E7E6, targeting HPV16 E6 and E7. This replication-competent vector was stably attenuated using a three…

STING agonist formulated cancer vaccines can cure established tumors resistant to PD-1 blockade.

Stimulator of interferon genes (STING) is a cytosolic receptor that senses both exogenous and endogenous cytosolic cyclic dinucleotides (CDNs), activating TBK1/IRF3 (interferon regulatory factor 3), NF-κB (nuclear factor κB), and STAT6 (signal transducer and activator of transcription 6) signaling pathways to induce robust type I interferon and proinflammatory cytokine responses. CDN ligands were formulated with granulocyte-macrophage colony-stimulating factor (GM-CSF)-producing cellular cancer vaccines--termed STINGVAX--that demonstrated potent in vivo antitumor efficacy in multiple therapeut…

WT1 peptide cancer vaccine for patients with hematopoietic malignancies and solid cancers.

Wild-type Wilms' tumor gene WT1 is expressed at a high level in hematopoietic malignancies including acute leukemia, chronic myelogenous leukemia, and myelodysplastic syndromes, as well as in various kinds of solid cancers. Human cytotoxic T lymphocytes (CTLs), which could specifically lyse WT1-expressing tumor cells with HLA class I restriction, were generated in vitro. It was also demonstrated that mice immunized with the WT1 peptide rejected challenges by WT1-expressing cancer cells and survived with no signs of autoaggression to normal organs that physiologically expressed WT1. Furthermore

Nanoparticles-Assisted Peptide Cancer Vaccine Delivery.

Efficient delivery of tumor antigen peptides and adjuvants to dendritic cells in lymphoid organs is critical for eliciting potent tumor anti … f antigen-specific T cell responses. Here we summarize the methods for preparing sHDL-based peptide cancer vaccines and protocols for analyzing the T cell responses against different tumor antigens. We envision that these methods will be helpful in developing and assessing other nanoparticle-based peptide cancer vaccines in the future.

Super-adjuvant nanoparticles for platform cancer vaccination.

We report on the utility of a "super-adjuvant" nanoparticle (NP) system as a modular, customizable platform for next-generation cancer vaccination. Using nanomaterials engineering technology, we aim to harness not only the effective adjuvanticity of whole-pathogen vaccines, but also the safety of subunit vaccines. Our lipid-based NP platform co-encapsulates agonists of the stimulator of interferon genes (STING) and Toll-like receptor 4 (TLR4) pathways to promote synergistic production of type I interferons and other proinflammatory cytokines in innate antigen-presenting dendritic cells and mac

Key considerations for a prostate cancer mRNA vaccine.

Prostate cancer has the second highest cancer mortality rate in the UK in males. Early prostate cancer is typically asymptomatic, with diagnosis at a locally advanced or metastatic stage. In addition, the inherent heterogeneity of prostate cancer tumours differs significantly in terms of genetic, molecular, and histological features. The successful treatment of prostate cancer is therefore exceedingly challenging. Immunotherapies, particularly therapeutic vaccines, have been widely used in preclinical and clinical studies to treat various cancers. Sipuleucel-T was the first cancer vaccine appr

Human Papillomavirus-Related Cancer Vaccine Strategies.

BACKGROUND: Human papillomavirus (HPV) persistent infection is a major pathogenic factor for HPV-related cancers, such as cervical cancer (CC), vaginal cancer, vulvar cancer, anal cancer, penile cancer, and head and neck cancer (HNC). Since the introduction of the world's first prophylactic HPV vaccine, there has been a decline in the incidence of HPV infections and associated cancers. This article reviews the latest literature on the research progress, efficacy, and safety of HPV vaccines for these cancers, providing a reference for HPV vaccination strategy. METHODS: By utilizing databases s

HPV Vaccination: An Underused Strategy for the Prevention of Cancer.

Human papillomavirus (HPV) vaccination prevents cervical, head and neck, and anogenital cancers. However, global HPV vaccine coverage falls short of global targets and has seen unexpected and dramatic declines in some countries. This paper synthesizes the impact of HPV on the global burden of cancer and the potential benefit of HPV vaccination. Approximately 5% of the world's cancers are specifically attributed to HPV. While the greatest global burden of HPV is cervical cancers in low- and middle-income countries, HPV-associated head and neck cancers are increasing in high-income countries and

A review on development of MUC1-based cancer vaccine.

Mucin 1 (MUC1) is a transmembrane mucin glycoprotein expressed on the surface of almost all epithelial cells. Aberrantly glycosylated MUC1 is associated with cellular transformation from a normal to malignant phenotype in human cancers. Therefore, MUC1 is the major target for the design and development of cancer vaccines. MUC1-based cancer vaccines are a promising strategy for preventing cancer progression and metastasis. This review summarizes the most significant milestones achieved to date in the development of different MUC-1-based vaccine approaches in clinical trials. Further, it provide…

[Challenges in the implementation of HPV vaccination for cancer prevention].

In France, every year, HPVs cause approximately 6,300 cancers -cervix, anus, oropharynx, penis, vulva, vagina, oral cavity, larynx- representing 2% of all incident cancers, 35,000 precancerous le-sions of the cervix, and at least 50,000 condylomas in the under- 30s. All 3,000 new cases of cervical cancers are attributable to HPV infections. Cervical cancer prevention is based on the combination of two complementary strategies: vaccination of 11 to 14-years-old girls against HPV and organized screening of cervical cancer for women between 25 and 65 years old. In France, the vaccination coverage
