Screening colonoscopy for the detection of neoplastic lesions in asymptomatic HIV-infected subjects.

BACKGROUND: Although non-AIDS defining malignancies are rapidly increasing as HIV-infected subjects live longer, little is know about the results of screening for colonic neoplasms (adenomatous polyps and adenocarcinomas) in this population. METHODS: We conducted a screening colonoscopy study to determine the prevalence of colonic neoplasms in 136 asymptomatic HIV-infected subjects >or=50 years of age and 272 asymptomatic uninfected control subjects matched for age, sex, and family history of colorectal cancer. Advanced neoplasms were defined as adenomas >or=10 mm or any adenoma, regardless o

High constant incidence rates of second primary neoplasms.

The incidence of contralateral breast cancer is high and constant with age, around five per 1000 women who had a primary breast cancer. For other neoplasms, the pattern of incidence of second primary neoplasms with age is less known, particularly as for only a few neoplasms the site of origin is not totally removed, and hence remains at risk of a second primary. Using the dataset from the Cancer Registry of the Swiss Canton of Vaud, we show that the incidence of second neoplasms is constant with age also after oral and pharyngeal, colorectal cancers, cutaneous malignant melanoma (CMM) and basa

[Colorectal precancerous conditions from the morphologic viewpoint].

Until their manifestation colorectal cancers pass certain precancerous lesions. This knowledge is extraordinarily significant for prophylaxis of cancer. Among precancerous lesions the different adenomas have the greatest importance. They can occur as solitary or multiple adenomas or adenomatosis coli. Within the so-called cancer-families also the adenomas are considered to be precursors of colorectal cancer. The dignity of neoplasms is determinable by their macroscopic shape and size, by their histological type and the grade of atypia. An exact classification of colorectal adenomas with evalua

The Four Horsemen in Colon Cancer.

Worldwide, neoplasms of the gastrointestinal tract have a very high incidence and mortality. Among these, colorectal cancer, which includes colon and rectum malignancies, representing both highest incidence and mortality. While gallbladder cancer, another neoplasm associated to gastrointestinal tract occurs less frequently. Genetic factors, inflammation and nutrition are important risk factors associated with colorectal cancer development. Likewise, pathogenic microorganisms inducing intestinal dysbiosis have become an important scope to determine the role of bacterial infection on tumorigenes

Adenocarcinoma of colon differentiating as dome epithelium of gut-associated lymphoid tissue.

AIMS: An early adenocarcinoma of the ascending colon was confined to a mass of gut-associated lymphoid tissue (GALT). The first description of an adenocarcinoma of colon differentiating as dome epithelium is presented. METHODS AND RESULTS: A plaque-like carcinoma was identified opposite the ileocaecal valve in an asymptomatic 56-year-old man with a family history of colorectal cancer. Malignant epithelium was confined to a mass of GALT filling but limited to the submucosa. Characterization of the neoplasm was undertaken by means of mucin histochemistry, immunohistochemistry, electron microsco

Endoscopic hand suturing using a modified through-the-scope needle holder for mucosal closure after colorectal endoscopic submucosal dissection: Prospective multicenter study (with video).

OBJECTIVES: Endoscopic hand suturing (EHS) is a novel technique for closing a mucosal defect after endoscopic submucosal dissection (ESD). We investigated the technical feasibility of colorectal EHS using a modified flexible through-the-scope needle holder. METHODS: This was a prospective multicenter study conducted at two referral centers between June 2022 and April 2023. This study included colorectal neoplasms 20-50 mm in size located in the sigmoid colon or rectum. A modified flexible through-the-scope needle holder, with an increased jaw width to facilitate needle grasping, was used for …

C-reactive protein and risk of colorectal adenomas or serrated polyps: a prospective study.

Serum C-reactive protein (CRP) is a sensitive marker of systemic inflammation. Because there is a well-recognized relationship between local inflammation and colorectal cancer, we aimed to evaluate whether serum CRP levels were associated with the occurrence of colorectal adenomas and serrated polyps using data from a large adenoma prevention trial. A total of 930 participants with a history of colorectal adenomas were enrolled in a randomized trial of calcium supplementation (1,200 mg/day) for the prevention of colorectal adenomas. Outcomes in this analysis are metachronous adenomas (and adva

Occult appendix pathology in patients undergoing colorectal cancer resection and prophylactic appendectomy.

BACKGROUND: The risk-benefit balance of prophylactic appendectomy in patients undergoing left colorectal cancer resection is unclear. The aim of this report is to assess the proportion of histologically abnormal appendices in patients undergoing colorectal cancer resection in a unit where standard of care is appendectomy, with consent, when left-sided resection is performed. METHODS: A retrospective study on a prospectively collected database was conducted in a single tertiary-care center. Overall, 717 consecutive patients undergoing colorectal cancer resection between January 2015 and June 2

Neuroendocrine carcinomas arising in ulcerative colitis: coincidences or possible correlations?

Patients with inflammatory bowel disease (IBD) are at increased risk of colorectal malignancies. Adenocarcinoma is the commonest type of colorectal neoplasm associated with ulcerative colitis (UC) and Crohn's disease, but other types of epithelial and non-epithelial tumors have also been described in inflamed bowel. With regards to non-epithelial malignancies, lymphomas and sarcomas represent the largest group of tumors reported in association with IBD, especially in immunosuppressed patients. Carcinoids and in particular neuroendocrine neoplasms other than carcinoids (NENs) are rare tumors an

[Early colorectal cancer].

Early colorectal cancer is situated at the field with a rich stock of topics in Japan. For this reason, there are increasing number of patients with colorectal cancer, many reports of depressed early cancer and rapid advances of molecular biology about colorectal neoplasm. Most of early colorectal cancers are discovered to be the protruded type by all accounts in spite that many cases of depressed type early cancer have been reported in many Japanese institutes. The final diagnosis of early colorectal cancer is usually made by histological examination after endoscopic resection, however invasi

Colorectal malignancies in HIV-positive patients.

Due to the development of more effective medications, those infected with HIV are living longer. Consequently, more tumors and infections have been added to the AIDS-defining criteria in the last decade. Our aim was to review the occurrence and clinical course of colorectal (CR) malignancies in HIV infected/AIDS patients from a single institution. A retrospective review of HIV/AIDS patients with colorectal malignant tumors was undertaken. We included adult patients, with ELISA and Western blot test positive for HIV, and primary malignant tumors located in the colon or rectum. Malignant neoplas

From Detection to Delay: Real-World Gaps in Post-Cologuard® Colonoscopy Adherence.

PURPOSE: Colorectal cancer remains a leading cause of cancer deaths, highlighting the need for early detection. Cologuard®, a non-invasive stool DNA test, detects biomarkers for CRC and precancerous lesions but requires follow-up colonoscopy and has a high false-positive rate. This study evaluates colonoscopy follow-up rates and diagnostic outcomes after positive Cologuard® results. METHODS: We conducted a retrospective cohort study using the TriNetX database, a global federated real-world data platform, to analyze patients aged ≥ 18 years who tested positive on Cologuard. The primary outcome

Colorectal cancer screening in asymptomaic adults: comparison of colonoscopy, sigmoidoscopy and fecal occult blood tests.

BACKGROUND AND PURPOSE: Fecal occult blood tests (FOBT) and flexible sigmoidoscopy have previously been recommended for colon cancer screening. More recently, studies have recommended colonoscopy due to the high rates of advanced neoplasm not detected by FOBT and sigmoidoscopy. Previous studies of the effectiveness of colonoscopic screening in Taiwan were limited to families of patients with colorectal cancer. This study compared colonoscopy, sigmoidoscopy and FOBT for colorectal cancer screening in asymptomatic adults. METHODS: Screening colonoscopies and FOBT were performed in asymptomatic

Laparoscopic colorectal surgery and cancer.

Review article of the actual level of knowledge about laparoscopic colorectal surgery and laparoscopic-assisted procedures. At present time laparoscopy for the cure of colorectal neoplasia should only be performed within prospective randomized trials. In the meantime it is perfectly appropriate to perform laparoscopy for palliation of metastatic disease. Patients with benign colorectal neoplasms can achieve better outcome if treated by laparoscopy rather than laparotomy. Other areas in which laparoscopy may be useful include enterolysis and possibly, treatment of rectal prolapse. Laparoscopy c…

Perception errors with double-contrast enema after a positive guaiac test.

Between 3 and 6 of 12 colorectal neoplasms larger than 1 cm in diameter were overlooked by each of 4 radiologists in 117 individuals investigated with double-contrast enema (DCE) after a positive guaiac test. Even when the individual results of 2, 3, or 4 experienced radiologists were combined, 2 small carcinomas were overlooked due to perception errors. One of these carcinomas was diagnosed by rectosigmoidoscopy, which is an important and necessary complement to DCE. Although small carcinomas may be missed even with this combination, meticulous exploitation of the 2 methods is absolutely nece…

Differences in risk factor-colorectal adenoma associations according to non-steroidal anti-inflammatory drug use.

OBJECTIVE: Because multiple observational studies and large, randomized controlled trials indicate that NSAIDs strongly reduce the risk of colorectal neoplasms, we investigated whether NSAID use masks associations of various other risk factors with colorectal neoplasms. MATERIALS AND METHODS: Using pooled data from three case-control studies of incident, sporadic colorectal adenoma (pooled n=789 cases, 2035 polyp-free controls), using multivariable logistic regression, we investigated various risk factor-colorectal adenoma associations stratified by NSAID use. RESULTS: Example multivariable-…

Unexpected Metastasis of Primary Colonic Adenocarcinoma: A Case Report and Literature Review.

Colorectal cancer is a common cancer worldwide. The major sites of colorectal cancer metastasis are the liver, lungs, peritoneum, lymph nodes, and bones. However, secondary localization in the bladder is extremely rare. Herein, we present the case of a 36-year-old patient who underwent surgery for colonic adenocarcinoma. Subsequently, the patient presented total hematuria during adjuvant chemotherapy. Cystoscopy and biopsy identified a bladder metastasis. In our discussion, we aim to delve into the distinct characteristics of bladder metastases originating from digestive neoplasms.
