Flexible fibroscopy is the method of choice in the diagnosis of upper airway (UA) pathologies. Some Authors believe that it should be carried out under local anaesthesia even in children less than one year of age. Equipping ureteral instruments with the guide-eye feature has expanded the instrumentation options available to urologists. The guide-eye facilitates repetitive instrument access, allows coordinated instrument combinations, and enhances sequential placement of a series of instruments. We believe this concept increases the safety and efficacy of our present instruments and clearly encourages innovative designs for the future.

Flexible carbon dioxide cystoscopy has been carried out in 65 patients using an Olympus BF type 1 TR endoscope (usually used for bronchoscopy). Good views were obtained and comparison with standard cystoscopy confirmed that no lesions were missed. The procedure was well tolerated under local anaesthesia. The flexible cystoscope was particularly useful in examining patients with diverticula, anterior bladder neck lesions or the post-radiation fibrotic pelvis.

In order to determine whether larger biopsy forceps improve the diagnostic yield over smaller standard forceps, we performed 30 transbronchial biopsy procedures in 28 patients with lung infiltrates using three types of instruments. We found that compared to the standard forceps specimens the large forceps acquired larger specimens in 22 of 29 procedures and that the alligator forceps acquired larger specimens in 7 of 10 procedures. Endoscopic duodenal biopsies were taken from 27 patients with suspected coeliac disease and compared with intubation capsule jejunal biopsies. The specimens were reported without knowledge of the patients' names or symptoms. In 24 patients (89%), coeliac disease could either be diagnosed or excluded with 100% accuracy, despite the inability to orientate the biopsies correctly. Primary pulmonary fibrosarcoma is a rare malignant tumor in childhood. In the absence of metastases, complete resection is curative. An 8-year-old boy suffered from unresolving pneumonia due to an obstructing lesion in the left main bronchus. Cytology of the bronchoalveolar lavage fluid and histology of bronchial biopsy revealed the diagnosis of pulmonary fibrosarcoma. The tumor did not respond to chemotherapy, and a total lobectomy with sleeve resection was performed with complete removal of the neoplasm. Two years after the operation the child has no evidence of disease.

We investigated the effects of semiconductor diode laser contact transscleral cyclophotocoagulation in human cadaver eyes by gross examination, light microscopy, and scanning electron microscopy to determine optimum clinical treatment parameters. Irradiation was performed using a continuous-wave semiconductor diode laser with a fiber-optic probe.
