We report a case of an acute anaphylactic reaction to intravenous cyclophosphamide in a patient with systemic lupus erythematosus. Skin testing for reactivity to cyclophosphamide and one of its metabolites, 4-hydroperoxycyclophosphamide, was performed on the patient and on 6 controls. The patient exhibited positive skin test responses; all controls had negative responses. The results suggest an acute hypersensitivity reaction related to sensitivity to cyclophosphamide and to 4-hydroperoxycyclophosphamide, through a shared antigenic determinant or dual sensitization.

A 52-year-old male developed cutaneous necrotizing vasculitis, renal failure, massive proteinuria, and elevated serum IgE levels following oral naproxen therapy. Renal biopsy specimen revealed cellular crescents in more than 40% of the glomeruli and a rare arteriole displayed thickening and eosinophilia of intima. Electron microscopic examination showed fusion of epithelial foot processes. Discontinuation of naproxen therapy was associated with disappearance of cutaneous lesions and improvement of renal function.

Hypersensitivity vasculitis refers to a large and heterogeneous group of clinical syndromes that have in common the predominant involvement of small vessels, especially in the skin. But any organ can be involved with its associated specific clinical and pathologic manifestations. As involvement of the upper airways has not previously been reported, a case of vasculitis associated with a nasopharyngeal tumour is presented. The possible etiology and relation to other diseases in the upper respiratory tract, especially Wegener's granulomatosis, is discussed.

A 15-month-old girl initially suspected of having Kawasaki disease is presented. The diagnosis was based on the combination of prolonged fever, conjunctivitis, edema of hands and feet, exanthem and lymphadenopathy. A workup for infectious etiologies was negative. She was subsequently found to have acetaminophen hypersensitivity. To our knowledge this clinical presentation of acetaminophen hypersensitivity has not previously been described in medical literature in English.

Seven patients with hypersensitivity reactions to phenobarbital are described. Clinical manifestations consisted of fever and pruritic skin rash, often associated with adenopathy and conjunctivitis. Reexposure to phenobarbital or other anticonvulsants resulted in redevelopment of symptoms. In vitro lymphocyte stimulation studies with anticonvulsant drugs suggested a cell-mediated hypersensitivity reaction to phenobarbital.

Phytohemagglutinin (PHA) was capable of inducing non-cytotoxic histamine release from human leucocytes. In the presence of deuterium oxide (D2O), PHA caused significantly greater histamine release. Dibutyryl cyclic AMP (d-cAMP) could enhance the histamine release in the presence of D2O although it was an inhibitor of the release if used alone. However, a beta agonist, isoproterenol, which increases intracellular level of cAMP was inhibitory with or without D2O. These data ask the question about dual effect of cAMP and suggest the possibility of different polls of cAMP in the target cells.

The frequency of severe reactions to allopurinol has probably been underestimated. A retrospective study encompassing a five-year period has yielded 20 patients with severe hypersensitivity reactions to allopurinol. Patients with preexisting renal impairment or who were receiving concomitant thiazide diuretics appeared to be especially predisposed.
