The purpose of this article is to report an unusual bilateral inferior subluxation of the lens in a patient with Marfan's syndrome. A 14-year-old boy presented with gradual painless diminution of vision in both eyes. His family history showed that his maternal uncle also had similar complaints. A healthy man in his 40s presented 12 hours after blunt eye trauma with painful vision loss in the right eye. Examination showed an intact crystalline lens dislocated into the anterior chamber and occluding the pupil, with intraocular pressure (IOP) 44 mm Hg. Intracapsular clear crystalline lens extraction and intraocular lens (IOL) implantation were performed in 4 highly myopic eyes of 2 patients with Marfan's syndrome. One eye of each patient received an anterior chamber IOL and the other, a scleral-fixated posterior chamber IOL. The preoperative spherical equivalent ranged between -14.50 and -28.00 diopters (D) and axial length range, between 25.32 and 36.02 mm. The SRK II formula was used. Isolated sulphite oxidase deficiency is a rare metabolic disorder characterised by neurological abnormalities, lens subluxation and seizures. Inheritance is autosomal recessive. We report two siblings with onset of clinical symptoms at 6 months of age, progressing to severe mental retardation, spasticity and seizures which were difficult to control. In conclusion, the incidence of retained lens fragments appears to have increased in recent years. The timing of vitrectomy can be delayed to permit improvement of corneal edema, ocular congestion, and acute postoperative inflammation. Placement of a posterior chamber intraocular lens at the time of the initial cataract surgery does not adversely affect final visual results. Retinal detachment is a frequent adverse event in these eyes. Vitrectomy and removal of retained lens fragments restores good visual acuity and reduces secondary glaucoma in the majority of patients.

A rare case of congenital bilateral ectopia lentis et pupillae (ELEP) is reported in a 32 year old female presenting with complaints of diminution of vision in both eyes. Associations of optic nerve hypoplasia and myelinated nerve fibers are being reported for the first time in such a case. The aetiopathogenesis of ELEP is controversial. Combined neuroectodermal and mesodermal origin is being postulated for this syndrome on the basis of aforesaid abnormalities and presence of pupillary dilator muscle hypoplasia, "annular endotheliopathy", and persistent pupillary membrane in both eyes.

Records of 48 dogs (57 eyes) that underwent intracapsular lens extraction (ICLE) of displaced lenses were reviewed. Preoperatively, 73% (19/26 eyes) of eyes with anterior luxations had secondary glaucoma compared to 43% (10/23 eyes) with subluxations and 38% (3/8 eyes) with posterior luxations. Forty-one of 57 eyes (72%) had vision and intraocular pressure (IOP) less than 30 mm Hg four to six weeks after surgery.
