In an experimental clinical study, 25 implants of pure titanium were inserted into the proximal tibia of 11 volunteer patients, four with rheumatoid arthritis and seven with osteoarthritis. The implants were removed from five weeks to 24 months later and detailed histological analysis was performed. The implants generally healed with direct bone-metal contact, showing so-called osseointegration. Only one of the 21 implants which had been in place for over five months did not show osseointegration, probably because of inadequate primary contact with bone. The presence of rheumatoid disease did

T cells are lymphoid-derived cells that influence other immune cells in the inflammatory microenvironment by secreting cytokines and growth factors to facilitate inflammation resolution and new tissue formation. Our group has previously shown the importance of αβ T cells along with subsequent subsets of CD4+ and CD8+ T cells, both in vitro and in vivo, in reducing the polarization of pro-inflammatory macrophages, promoting the recruitment and proliferation of mesenchymal stem cells (MSCs), and enhancing new bone formation around modified titanium (Ti) implants. Regulatory T cells (Tregs) are a

The compromised regulation of osteoimmune homeostasis poses a critical challenge for titanium implant osseointegration as conventional surface strategies lack dynamic responsiveness to postoperative microenvironmental evolution. Here, we developed a near-infrared (NIR)-responsive biointerface with photoelectronic immunomodulation fabricated via ultrasonic spray. By controlling the deposition of NIR-active gold/rare earth nanocomposites (Au@RE) on sandblasted acid-etched titanium (annotated as Au@RE/SLA Ti), we achieved NIR-triggered photoelectron generation for noninvasive macrophage polarizat

BACKGROUND: Long-term implant success depends on preventing infection and achieving initial stability, yet many antimicrobial coatings impair osseointegration, and most animal models overlook the impact of mechanical loading. To overcome the limitations of static in vivo models that do not reflect the mechanical environment of dental implants, this study developed a loaded animal model that evaluates the osteogenic properties of an Ag-GL coating under clinically relevant biomechanical forces.
METHODS: Silver nanoparticles (AgNPs) were synthesized with a redox reaction, while the antimicrobial

This narrative review, prepared in accordance with the Scale for the Assessment of Narrative Review Articles, surveys next-generation dental implant surface modifications for osseointegration, from nanoengineering to clinical translation. Peri-implantitis affects approximately 19.53% of patients and 12.53% of implants. Because peri-implant tissue destruction reflects a dysregulated host inflammatory response to an unavoidable, lifelong microbial challenge rather than bacterial colonization alone, the design target is shifting from surfaces that are simply bactericidal to surfaces that regulate

Bone defects, prevalent in orthopaedic practice due to trauma or disease, present apparent clinical repair challenges. This study innovatively designed vortex-enhanced gyroid gradient scaffolds through finite element hydrodynamic simulation and selective laser melting (SLM) 3D printing, systematically evaluating their permeability and osteogenic efficacy. Homogeneous triply periodic minimal surface (TPMS) gyroid structures (50%, 60%, 70% porosity) and a gradient porous scaffold (average 60% porosity) were modeled. ANSYS-based hydrodynamic analysis revealed that the gradient scaffold maximized

This review article summarizes the current modification methods employed to enhance the osseointegration properties of polyetheretherketone (PEEK), a novel biomaterial. Our analysis highlights that strategies such as surface treatment, surface modification, and the incorporation of bioactive composites can markedly improve the bioactivity of PEEK surfaces, thus facilitating their effective integration with bone tissue. However, to ensure widespread application of PEEK in the medical field, particularly in oral implantology, additional experiments and long-term clinical evaluations are required

Effective and robust osseointegration represents the critical foundation for function and long-term stability of dental implants. Although the immune response has been recognized as a critical orchestrator in this process, the contribution of neuro-immune communication within the peri-implant microenvironment remains largely underexplored. This study aimed to investigate how transient receptor potential vanilloid 1 (TRPV1) in sensory neurons influenced macrophage polarization and immune response during implant osseointegration. In vivo, TRPV1 activation in trigeminal ganglion (TG) enhanced rel

RATIONALE: Imbalances in the osteogenic-osteoclastic microenvironment hinder effective osseointegration between the prosthesis and host bone, which is a key factor contributing to the high incidence of prosthesis loosening and periprosthetic fractures in osteoporosis patients. Developing an implant interface that can reverse the dysregulated osteogenic microenvironment is an effective strategy to address this challenge.
METHODS: A novel bioinspired interface with a spatial gradient structure was engineered by 3D-printed porous titanium alloy implants and hollow mesoporous silica nanoparticles

The different macro and micro geometries of dental implants are parameters that directly affect osseointegration, making them an important area for research. The objective of this preclinical study was to compare, through histological and histomorphometric analyses, the biological response of two different dental implant surfaces in osseointegration. Surface morphology and chemistry were characterized by SEM/EDX, optical-emission spectroscopy, protein adsorption (BSA), and adipose-derived stem-cell morphology. For the in vivo arm, ten commercially pure titanium implants (n = 5 LS160 + 5 SBAE)

The repair of bone defects faces the dual challenges of limited autograft availability and the single functionality of artificial materials. Consequently, the development of new biomaterials with mechanical adaptability, controlled drug release, and osteoinductive properties has become a major research focus in bone tissue engineering. In this study, we designed an injectable, photocrosslinkable hydrogel by incorporating icariin (ICA)-loaded mesoporous silica nanoparticles (MSNs) into gelatin methacryloyl (GelMA). The MSNs increased the compressive modulus by 1.5-fold and enabled pH-responsive

Osseointegration between biomaterial and bone is critical for the clinical success of many orthopaedic and dental implants. However, the mechanisms of in vivo interfacial bonding formation and the role of immune cells in this process remain unclear. In this study, we investigated the bone-scaffold material interfaces in two different 3D printed porous scaffolds (polymer/hydroxyapatite and sintered hydroxyapatite) that elicited different levels of foreign body response (FBR). The polymer/hydroxyapatite composite scaffolds elicited more intensive FBR, which was evidenced by more FBR components,

Recently, a large number of studies have reported that lithium (Li) displayed a positive effect on osteogenesis. However, only a few studies have investigated the Li-incorporated surfaces through electrochemical deposition. In this study, electrochemical deposition was conducted on a CHI600E electrochemical workstation. The characterization of electrochemical deposition (ECD) and ECD-Li surfaces were detected by field-emission scanning electron microscopy with energy-dispersive spectrometer. rBMSCs were cultured on two surfaces for subsequent adhesion, proliferation and live/dead assay. BACKGROUND: Type 2 diabetes mellitus (T2DM) has a harmful effect on the stability and osseointegration of dental implants. T2DM induces mitochondrial damage by inhibiting AMPK signaling, resulting in oxidative stress and poor osteogenesis in the peri-implant bone area. Genipin is a major component of gardenia fruits with strong antioxidant, anti-inflammation, and antidiabetic actions, and it also can activate mitochondrial quality control via the AMPK pathway. The purpose of this study was to investigate the effects of genipin and insulin treatment on implant osseointegration in T2DM rats and

Poly(methyl methacrylate) (PMMA) bone cement has been widely used in orthopedic surgeries including total hip/knee replacement, vertebral compression fracture treatment, and bone defect filling. However, aseptic loosening of the interface between PMMA bone cement and bone often leads to failure. Hence, the development of modified PMMA that facilitates the growth of bone into the modified PMMA bone cement is key to reducing the incidence of aseptic loosening. In this study, MgAl-layered double hydroxide (LDH) microsheets modified PMMA (PMMA&LDH) bone cement with superior osseointegration perfor

INTRODUCTION: Osseointegrated (OI) prostheses have a unique benefit-risk profile among prosthetic alternatives and have been marketed in the United States under a Humanitarian Device Exemption since 2015. Information about upper limb prosthesis user perspectives on benefits and risks, prosthesis-user subpopulations for whom OI is most acceptable, and outcomes that matter most to patients could help inform clinical and regulatory decision-making. Recent 21st Century Cures legislation expanded the role of patient experience data in the decision-making process of the U.S. Food and Drug Administra

Over the last few decades, a wide variety of dental implants have been successfully placed in jaw bones to restore tooth function. But major challenges still remain in patients with osteoporosis involving compromised osseointegration, and the therapeutic methods is far from optimism. Gallium can directly inhibit bone osteolysis, prevent bone calcium release and augment bone mass, which makes Ga unique among the potential antiresorptive drugs. Silicon, as an indispensable modulator in bone formation, presents its bone anabolic effects, while reduces, at least doesn't increase, bone resorption.

BACKGROUND: Compressive osseointegration is a promising modality for limb salvage in distal femoral oncologic tumors. However, few studies have explored short-term survival rates in a large patient cohort of distal femur compressive endoprostheses or highlighted the risk factors for spindle failures.
QUESTIONS/PURPOSES: We asked: (1) What is the frequency of compressive osseointegration spindle failure in distal femoral reconstructions? (2) What are the characteristics of rotational failure cases with distal femur compressive osseointegration endoprostheses? (3) What are the risk factors for
