Reliability of capillary refill time for evaluation of tissue perfusion in simulated vascular occluded limbs.

PURPOSE: No standardized execution or evidence demonstrates the area of the digit giving the most accurate capillary refill time (CRT). This study investigated the reliability and validity of CRT, and the relative merits of areas where the test could be performed. METHODS: In all, 127 healthy volunteers were assessed for normal CRT at the fingernail, lateral paronychia, and proximal and distal pulps of the index finger. The predictive validity of the CRT for the diagnosis of compromised vascular perfusion was also investigated on 24 subjects, using an inflated tourniquet. Three raters assesse

Coagulase-Negative Staphylococcus Skin and Soft Tissue Infections.

Coagulase-negative staphylococcus organisms may be normal flora of human skin, however these bacteria can also be pathogens in skin and soft tissue infections. A summary of skin and soft tissue infections caused by coagulase-negative staphylococcus species is provided in this review. We conducted a search of the PubMed database using the following terms: abscess, auricularis, biofilm, capitis, cellulitis, coagulase, contaminant, cyst, draining, epidermidis, felon, folliculitis, furuncle, haemolyticus, hominis, indolent, infection, lugdunensis, mecA, microbiome, negative, osteomyelitis, paronyc

Onychopathy induced by temsirolimus, a mammalian target of rapamycin inhibitor.

Temsirolimus belongs to the mammalian target of rapamycin (mTOR) inhibitors, targeted therapies for which indications are booming in oncology. While their tolerance is usually good, mucocutaneous toxicity is the most common, including stomatitis, rashes, edemas, pruritus, dry skin and nail disorders. The latter are common in clinical practice but have not yet been well characterized. We report 2 cases of patients who developed, after 6-7 months with temsirolimus, a dystrophy of the 20 nails with fragility, distal onycholysis, yellow discoloration, associated in 1 case with painful paronychia.

Nail involvement in pemphigus vulgaris.

Nail involvement in pemphigus vulgaris is relatively rare. We describe a case of severe pemphigus involving both the skin and oral mucosa in which an acute exacerbation was preceded by the onset of nail involvement of all 4 extremities. Nail involvement occurred in the form of hemorrhagic paronychia of multiple digits. Oral, cutaneous, and nail manifestations of the disease were all well controlled by systemic therapy. A review of the literature on nail involvement in pemphigus reveals that this involvement may be manifested in multiple ways, with chronic paronychia and onychomadesis being the

Different clinical phenotypes of nail abnormalities in patients with severe atopic dermatitis: a single-centre cross-sectional study.

INTRODUCTION: Nail involvement in atopic dermatitis (AD) remains an underrecognized and understudied manifestation, despite its potential to significantly contribute to disease burden. AIM: This study objective was to characterize the prevalence, types, and patterns of nail abnormalities in patients with severe AD and to explore clinical factors associated with their occurrence. MATERIAL AND METHODS: This single-centre, cross-sectional study included adult patients with severe AD (EASI > 21) diagnosed according to the Hanifin and Rajka criteria. Demographic, clinical, and laboratory data wer

Nail Loss in Mild to Moderate Pemphigus Vulgaris.

INTRODUCTION: Symptoms of pemphigus vulgaris (PV) rarely include nail findings. When ungual involvement does occur, the most common manifestations are paronychia and onychomadesis. Onycholysis is very uncommon, and complete nail loss has not been reported in the literature. Ungual involvement is thought to be closely correlated with disease severity, with only severe PV cases demonstrating nail symptoms. CASE PRESENTATION: We report a case of a 34-year-old female presenting with mild to moderate PV yet severe onycholysis of her first toe leading to secondary nail loss in conjunction with paro

Risk Factor Analysis for Anti-epidermal Growth Factor Receptor Monoclonal Antibody-induced Problematic Skin Toxicities in Patients With Liver Metastatic Colorectal Cancer.

BACKGROUND/AIM: We previously reported that patients with metastatic colorectal cancer (mCRC) and baseline liver metastasis are at a higher risk of developing grade ≥2 overall skin toxicities when treated with anti-epidermal growth factor receptor (EGFR) monoclonal antibody. This study aimed to identify additional factors associated with skin toxicities induced by anti-EGFR treatment in patients with liver metastatic CRC. PATIENTS AND METHODS: Patients with liver metastatic CRC who initially received anti-EGFR monoclonal antibody-containing treatment (n=77) were retrospectively assessed. The

Evaluation of cytology collection techniques and prevalence of Malassezia yeast and bacteria in claw folds of normal and allergic dogs.

BACKGROUND: Canine bacterial and Malassezia paronychia are common secondary complications of atopic dermatitis and adverse food reactions. HYPOTHESIS/OBJECTIVES: The aim of this study was to compare three different sampling methods for claw fold cytology and to evaluate the numbers of bacteria, Malassezia yeast and inflammatory cells. ANIMALS: Sixty client-owned dogs were classified into three groups: (A) normal dogs; (B) allergic dogs with no clinical evidence of claw disease (brown staining, erythema, swelling, crusts or exudates); and (C) allergic dogs with clinical paronychia. METHODS:

Gefitinib-induced unilateral papulopustular dermatosis: an uncommon presentation.

Papulopustular eruptions are the most common cutaneous adverse effect of epidermal growth factor receptor (EGFR) inhibitors, typically arising within the first month of therapy. We describe an unusual late-onset presentation. A middle-aged woman with EGFR-positive metastatic non-small-cell lung carcinoma had received gefitinib 250 mg daily for 2.5 years. 15 days before the consultation, she developed painful, erythematous papules and pustules confined to the right face, trunk and thighs, together with paronychia of the right great toe. Skin biopsy revealed a subcorneal neutrophilic pustule and

Bacterial and Viral Infections of the Nail Unit.

Bacterial and viral infections of the nail unit are very common as primary infections, especially bacterial paronychia and warts, but they can also be superinfections complicating other nail disorders. In many nail unit infections, the clinical presentation is nonspecific: in these cases, diagnostic tests are mandatory before treatment, to avoid spread of the infection and drug resistance. The most common forms of bacterial and viral infections that may affect the nail unit are herein described in detail, with diagnostic and treatment options provided.

Severe Acrodermatitis Continua of Hallopeau With Atypical Dermoscopic Features: A Diagnostic Pitfall.

Acrodermatitis continua of Hallopeau is a rare, chronic, relapsing form of localized pustular psoriasis characterized by painful sterile pustules and progressive destruction of the nail apparatus. Its clinical and dermoscopic features may be atypical, leading to diagnostic delays and inappropriate treatment. We present the case of a 78-year-old man with a one-year history of painful hyperkeratotic lesions affecting several fingers, with recent worsening, extension, and purulent discharge. Examination revealed bilateral chronic paronychia with peripheral erosions involving both thumbs and the l
