Somatostatin, gastrointestinal peptides, and the carcinoid syndrome.

During a control infusion noradrenaline and alcohol each provoked carcinoid flushing in four of five patients and pentagastrin in two of five patients. When tetradecapeptide somatostatin was infused on another day no patient flushed at any time, even when 16 microgram of either noradrenaline or pentagastrin were administered. Carcinoid flushing was not associated with release of gastrin or any of the other vasoactive or postprandially released gut regulatory peptides measured. In a sixth patient with severe prolonged carcinoid flushing, subcutaneous Des AA1, 2, 4, 5, 12D Trp8 somatostatin mark…

Influence of metiamide and atropine on acid and pepsin secretion in the anaesthetized Heidenhain cat.

The inhibition of acid and pepsin secretion by metiamide and atropine was studied in the anaesthetized Heidenhain-pouch cat. Metiamide strongly inhibited acid secretion induced by histamine, pentagastrin or carbachol. Metiamide had not effect on pepsin secretion stimulated by pentagastrin and carbachol but was effective on histamine-stimulated pepsin secretion. Atropine strongly inhibited pepsin secretion but was not as effective as metiamide in the inhibition of acid secretion. Reperfusion of the pouch with acid + salicylic acid also increased pepsin secretion. This increased pepsin output wa…

The effects of beta-adrenergic receptor blockade on the inhibition of gastric acid secretion, and the release of pancreatic polypeptide by antral distension in healthy subjects.

In a randomized double-blind study of seven healthy subjects an intravenous injection of metoprolol and propranolol did not change the submaximal gastric acid response to pentagastrin. The inhibition of pentagastrin-stimulated acid secretion by balloon distension of the antrum was unchanged by a preceding intravenous injection of metoprolol or propranolol. The plasma level of pancreatic polypeptide in peripheral venous blood showed a significant rise during antral distension. This rise was unaltered by a preceding injection of metoprolol and slightly depressed by propranolol. We conclude that …

Effects of gastrointestinal hormones on Oddi's sphincter and duodenal myoelectric activity and pancreatobiliary pressure. Studies in the opossum.

Myoelectric activity of Oddi's sphincter and duodenum was correlated with common duct and pancreatic duct pressures in conscious opossums after infusion of the following substances: cholecystokinin, pentagastrin, glucagon, secretin, and acetylcholine. Cholecystokinin and pentagastrin increased and glucagon and secretin decreased the frequency of spike potentials in Oddi's sphincter and the duodenum. Acetylcholine was associated with the largest increase in spike potentials in both Oddi's sphincter and the duodenum. Although there was variation in the number of pressure elevations related to Od…

Inhibitory effect of DS-4574, a mast cell stabilizer with peptidoleukotriene receptor antagonism, on gastric acid secretion in rats.

We examined the inhibitory effect of DS-4574 (6-(2-cyclohexylethyl)[1,3,4]thiadiazolo[3,2-alpha]-1,2,3- triazolo[4,5-d] pyrimidin-9(3H)-one), a mast cell stabilizer with peptidoleukotriene receptor antagonism, on gastric acid secretion stimulated by several secretagogues in rats. In anesthetized rats with acute gastric fistulas, DS-4574 (50 mg/kg, intraduodenal) significantly inhibited gastric acid secretion induced by both carbachol (50 micrograms/kg, s.c.) and pentagastrin (75 micrograms/kg, s.c.) but not by histamine (2.5 mg/kg, s.c.). In unanesthetized pylorus-ligated rats, DS-4574 (10 and…

The effect of antral distension on the endocrine pancreas in man.

Isolated antral distension by a 150 cm3 balloon in man significantly elevated the plasma concentrations of pancreatic polypeptide and glucagon in portal venous blood. The concentrations of plasma pancreatic glucagon, plasma insulin, and plasma glucose were unchanged in peripheral venous blood during antral distension with or without a background infusion of pentagastrin. Antral distension significantly increased the plasma concentration of pancreatic polypeptide in peripheral venous blood. Antral distension also inhibited basal and pentagastrin-stimulated gastric acid secretion in healthy subj…

The effect of cimetidine and propantheline on the symptoms of a patient with systemic mastocytosis.

A 24 year old white woman with a lifelong history of systemic mastocytosis and symptoms of diarrhea and flushing was demonstrated to have a normal gastric analysis and inconsistent steatorrhea. She responded well to oral cimetidine therapy for 11 months. A symptomatic recurrence was controlled with the addition of propantheline. Gastric secretory studies demonstrated cimetidine suppression of both basal acid and basal pepsin secretion, as well as maximal pentagastrin-stimulated acid secretion; suppression of stimulated pepsin secretion was minimal. The combination of cimetidine and propantheli…

Synthesis of nitric oxide in the dorsal motor nucleus of the vagus mediates the inhibition of gastric acid secretion by central bombesin.

1. Central administration of bombesin inhibits gastric acid production independently of the centrally or peripherally-acting stimuli employed. This study evaluates the role and location of the cerebral nitric oxide (NO) implicated in the inhibitory effect of central bombesin on in vivo rat gastric acid secretion, as induced by distension with 15 cm H2O, insulin (0.75 u.i. kg-1 i.p.) TRH (1.2 microg kg-1, i.c.) or pentagastrin (100 microg kg-1, i.p.). 2. The acid-inhibitory effect of i.c. bombesin (40 ng kg-1) was prevented by prior administration of L-NAME (80 microg kg-1) in the dorsal motor …

Peripheral acid inhibitory action of corticotropin releasing factor: mediation by nongastric mechanisms.

Corticotropin releasing factor (CRF) inhibits gastric acid secretion via both central neural and peripheral mechanisms. We examined whether local gastric factors may mediate the peripheral action of CRF. In pylorus-ligated anesthetized rats, CRF infusion (15 nmol/kg X h) produced roughly 60% inhibition of pentagastrin (16 micrograms/kg X h)-stimulated acid secretion. Similarly, in the gastric fistula dog, CRF (1 nmol/kg X h) inhibited pentagastrin-induced acid secretion by 74%. This action of CRF did not result from direct inhibition of gastric parietal cells, as concentrations of the peptide …

Effect of vagotomy on gastric acid secretion in the rat.

The effects of pentagastrin, histamine or feeding on gastric acid secretion were studied in conscious rats with total gastric by-pass, achieved by transection of the cardia and pylorus, followed by an oesophago-duodenostomy. After closure of the cardia, the by-passed stomach was connected to the small intestine through a Roux-en-Y loop. A chronic gastric fistula was fitted into the rumen. Basal acid output was low in chronically vagotomized rats, being 6% of that in the innervated animals. A clear-cut stimulation was observed after both pentagastrin and histamine in innervated as well as dener…

CGRP antagonists enhance gastric acid secretion in 2-h pylorus-ligated rats.

The influence of calcitonin gene-related peptide (CGRP) antagonists on gastric acid secretion was investigated in rats. Intravenous injection of the CGRP receptor antagonist, CGRP(8-37) (128 nmol/kg) or the CGRP antibody #4901 (4.8 mg/kg, IV) completely prevented alpha-CGRP (3.9 nmol/kg/h)-induced inhibition of acid response to pentagastrin in urethane-anesthetized rats with gastric fistula. CGRP antibody (4.8 mg/kg, IV) increased by 93% gastric acid secretion in conscious rats with pylorus ligation for 2 h. (CGRP(8-73) (128 nmol/kg, IV) also enhanced the acid response measured 2 h after pylor…

[Acid secretion and gastrin release following stimulation with intragastric instillation of a peptone solution in healthy probands and patients with duodenal ulcer].

In 19 DU-patients and in 15 control subjects acid secretion tests were performed by continuous aspiration or by intragastric titration. Pentagastrin (6 micrograms/kg subcutaneously) or a 20% peptone solution (intragastric instillation) were used for stimulation. In 10 control subjects pentagastrin stimulated acid secretion was within the normal range, hypersecretion (greater than 32 mmol/h) was found in the other 5. 14 DU-patients showed hypersecretion, 5 had normal acid secretion. In contrast to these findings intragastric peptone stimulation caused a very uniform acid secretion within each g…

Effects of cholecystokinin-octapeptide on gastric motility of anesthetized dogs.

The present experiments examined the local effects of cholecystokinin-octapeptide (CCK-8) and related peptides on gastric motility of anesthetized dogs. Peptides were injected through the gastroepiploic artery at doses of 1.0, 2.5, 5.0, 10.0, and 20.0 ng/ml. CCK-8 and its analogues (Glt-CCK-8, pGlu-CCK-8, and Suc1-MePhe8-CCK-7) increased gastric smooth muscle contraction in a dose-dependent manner. ED50 of CCK-8 was 2.97 +/- 0.63 ng/ml. Administration of atropine (100-200 micrograms/kg) inhibited the effects of both CCK-8 and pentagastrin; however, hexamethonium (5 mg/kg) failed to block the c…

Examination of somatostatin involvement in the inhibitory action of GIP, GLP-1, amylin and adrenomedullin on gastric acid release using a new SRIF antagonist analogue.

1. The effect of a new type 2 selective somatostatin (SRIF) receptor antagonist (DC-41-33) on somatostatin-induced inhibition of pentagastrin-stimulated gastric acid secretion in conscious, chronic gastric fistula equipped rats was studied. 2. Infused intravenously, DC-41-33 dose-dependently inhibits SRIF-induced inhibition of pentagastrin-stimulated gastric acid secretion with an IC50 of 31.6+/-1.2 nmol kg(-1) versus 10 nmol kg(-1) SRIF and blocks the inhibitory effects of SRIF when simultaneously co-infused. Its effectiveness provides additional evidence that SRIF-inhibition of gastric acid …

Codon Y791F mutations in a large kindred: is prophylactic thyroidectomy always indicated?

BACKGROUND: RET proto-oncogene mutations are associated with medullary thyroid carcinomas usually requiring preventive thyroidectomy in gene carriers. We present a large kindred with the Y791F mutation in the RET proto-oncogene that did not have medullary thyroid carcinomas. PATIENTS AND METHODS: Eight members of a Danish family with the Y791F mutation participated. All gene carriers underwent pentagastrin testing, and measurements of serum calcitonin. In the index person, exons 10, 11, and 13-16 of the RET proto-oncogene were screened. In the rest of the individuals only exon 13 was analysed…

Effects of in vivo stimulation on molecular forms of circulatory calcitonin and calcitonin gene-related peptide in man.

Multiple molecular weight forms of the potent vasodilator peptide calcitonin gene-related peptide (CGRP) are present in the circulation in man. The monomeric form of CGRP(1-37) and CT(1-32) account for only a fraction of the total immunoreactive CGRP and calcitonin (CT) in the circulation. In vivo release of various molecular weight forms of CGRP and CT, including the monomeric form, into the circulation was demonstrated by gel-permeation chromatography, following stimulation of the gastrointestinal tract by a meal, and C-cells of the thyroid gland with pentagastrin. Following these stimulatio…
