Kawasaki disease: clustering in infants and pre-school children in Kuwait.

We report five children who presented within a 2-month period and who all fulfilled at least four of the five criteria essential for the diagnosis of Kawasaki disease. They were three girls and two boys aged between 5 months and 3 years. Two of them had atypical presentations; one mimicked infectious mononucleosis and the other had severe abdominal pain and was later found to have hydrops of the gall bladder. Although treatment was started within the 1st 10 days of the illness, echocardiographic changes were found in three cases: one had myocarditis and the other two showed in the coronary art

Overwhelming sepsis presenting as sudden unexpected death.

Four children, including three infants, who died suddenly and unexpectedly are described. In three of the cases group A beta haemolytic streptococcus was cultured and in the fourth Streptococcus pneumoniae. The organism was grown from multiple sites including blood in two of them. Without microbiological investigation the diagnosis would have been sudden unexpected death syndrome.

Lyme disease in northern California.

Lyme disease is a recently described clinical entity with cutaneous, neurologic, articular and cardiac manifestations. Since the original description of the disease in 1977, more than 500 cases have been reported. Although the vast majority of patients have been from the area near Lyme, Connecticut, we have seen four patients from northern California with various aspects of Lyme disease. This diagnosis should be considered in patients who have traveled to tick regions and who have a distinctive skin lesion (erythema chronicum migrans). Clinical and epidemiologic evidence suggests the disease i

Meso-scale ecology of anthrax in southern Africa: a pilot study of diversity and clustering.

It has only recently been possible to detect sufficient genetic diversity among anthrax isolates to allow genotype grouping (Keim et al. 1997). Early results of such grouping suggest that the southern African subcontinent may be the geographical origin of Bacillus anthracis. This report describes a pilot investigation of the genetic diversity of a study group of isolates from the Kruger National Park, South Africa, and efforts to detect spatio-temporal clustering within the study group. This study has also served as further validation for the newly developed Multi-Locus VNTR Analysis (MLVA), d

Disparities in spatiotemporal clustering of maternal mental health conditions before and during the COVID-19 pandemic.

Mounting evidence indicates the worsening of maternal mental health conditions during the COVID-19 pandemic. Mental health conditions are the leading cause of preventable death during the perinatal and postpartum periods. Our study sought to detect space-time patterns in the distribution of maternal mental health conditions in pregnant women before (2016-2019) and during (2020-2021) the COVID-19 pandemic in North Carolina, USA. Using the space-time Poisson model in SaTScan, we performed univariate and multivariate cluster analysis of emergency department (ED) visits for perinatal mood and anxi

Spatial and temporal variation in biting rates and parasite transmission potentials of onchocerciasis vectors in Ecuador.

The influence of spatial and temporal factors on onchocerciasis transmission by Simulium exiguum s.l. and S. quadrivittatum in Ecuador was investigated to help develop sampling protocols for entomological surveillance of ivermectin programmes. Flies were collected in alternate months (November 1995-November 1996) at four sites each in the hyperendemic communities of San Miguel and El Tigre. A fixed-effects analysis of variance was used to explore the influence on vector abundance of locality, site, month and hour. Infectivity rates detected by dissection and PCR assays were compared. Simulium

Viscerotropic disease following yellow fever vaccination in Peru.

Five suspected cases of yellow fever vaccine-associated viscerotropic disease (YEL-AVD) clustered in space and time following a vaccination campaign in Ica, Peru in 2007. All five people received the same lot of 17DD live attenuated yellow fever vaccine before their illness; four of the five died of confirmed YEL-AVD. The surviving case was classified as probable YEL-AVD. Intensive investigation yielded no abnormalities of the implicated vaccine lot and no common risk factors. This is the first described space-time cluster of yellow fever viscerotropic disease involving more than two cases. Ma

COVID-19 outbreak following a single patient exposure at an entertainment site: An epidemiological study.

We investigated an outbreak of COVID-19 infection, which was traced back to a bathing pool at an entertainment venue, to explore the epidemiology of the outbreak, understand the transmissibility of the virus and analyse the influencing factors. Contact investigation and management were conducted to identify potential cases. Epidemiological investigation was carried out to determine the epidemiological and demographic characteristics of the outbreak. We estimated the secondary attack rate (SAR), incubation time and time-dependent reproductive number (Rt ) and explored the predisposing factors f

Typhoid fever acquired in the United States, 1999-2010: epidemiology, microbiology, and use of a space-time scan statistic for outbreak detection.

Although rare, typhoid fever cases acquired in the United States continue to be reported. Detection and investigation of outbreaks in these domestically acquired cases offer opportunities to identify chronic carriers. We searched surveillance and laboratory databases for domestically acquired typhoid fever cases, used a space-time scan statistic to identify clusters, and classified clusters as outbreaks or non-outbreaks. From 1999 to 2010, domestically acquired cases accounted for 18% of 3373 reported typhoid fever cases; their isolates were less often multidrug-resistant (2% vs. 15%) compared

Spatial epidemiology of lumpy skin disease outbreaks in Thailand: proximity patterns, case concentrations, spatio-temporal clusters, and interpolated case distribution.

INTRODUCTION: Lumpy skin disease (LSD) is an emerging transboundary disease affecting cattle and buffaloes, causing substantial economic and animal health consequences. Understanding its spatial dynamics is essential for formulating effective control and prevention strategies. This study aimed to characterize the spatial distribution of LSD outbreaks, identify significant outbreak clusters and interpolate the spatial distribution of LSD case values intensity across Thailand. METHODS: Data on LSD outbreaks in Thailand between March 2021 and December 2021 were analyzed. Nearest neighbor analysi

A different kind of contextual effect: geographical clustering of cocaine incidence in the USA.

STUDY OBJECTIVE: Outline the use of the pairwise odds ratio (PWOR) to quantify the extent to which a binary outcome clusters geographically. Quantify the extent to which first experience with cocaine is spatially correlated within US neighbourhoods and cities. Quantify geographical clustering of first experience with cocaine by neighbourhood context. DESIGN: Estimate the PWOR of incident cocaine experience at two levels (neighbourhood, city) and compare across years. Within years, estimate the PWOR by neighbourhood disadvantage and test for trend. SETTING: US National Household Survey on Dru

Temporal, spatial and space-time distribution of infections caused by five major enteric pathogens, Ontario, Canada, 2010-2017.

AIMS: In Canada, enteric diseases pose substantial health and economic burdens. The distribution of these diseases is uneven across both geography and time and understanding these patterns is therefore important for the prevention of future outbreaks. We evaluated temporal, spatial and space-time clustering of laboratory-confirmed cases of Campylobacter spp. (n = 28,728), non-typhoidal Salmonella spp. (n = 22,640), Shiga toxin-producing Escherichia coli (STEC; n = 1340), Yersinia spp. (n = 1674) and Listeria monocytogenes (n = 471) infections, reported between 2010 and 2017 inclusive in Ontari…

Environmental risk factors for haemorrhagic fever with renal syndrome in a French new epidemic area.

In France, haemorrhagic fever with renal syndrome (HFRS) is endemic along the Belgian border. However, this rodent-borne zoonosis caused by the Puumala virus has recently spread south to the Franche-Comté region. We investigated the space-time distribution of HFRS and evaluated the influence of environmental factors that drive the hantavirus reservoir abundance and/or the disease transmission in this area. A scan test clearly indicated space-time clustering, highlighting a single-year (2005) epidemic in the southern part of the region, preceded by a heat-wave 2 years earlier. A Bayesian regres…

Prospects and perils of antimicrobial resistance cluster detection using routinely collected data: an illustration from tertiary hospitals in Thailand representing different data contexts.

BACKGROUND: There are limited resources to detect and interpret cluster signals in resource-limited hospitals. The aim was to improve the interpretation of pathogen spatiotemporal clustering detected using the SaTScan algorithm - a method that uses space-time scan statistics to detect cluster signals that occur more often than expected. METHODS: Analysis of electronic data of inpatients with clinical specimens culture positive for seven antimicrobial-resistant pathogens in two tertiary hospitals in Thailand from January to December 2022 was performed. Space-time uniform scan statistics were a

Uncovering spatiotemporal development patterns of AIDS in China: A study using panel data with Joinpoint Regression analysis and Spatial Clustering.

The HIV/AIDS epidemic in China is severe and complex. Comprehensive spatiotemporal analysis provides valuable insights for intervention policy formulation. Previous studies often overlooked local changes in time trends and regional disease development patterns. In this study, we propose a new spatiotemporal analysis method based on the Joinpoint Regression (JPR) model and K-means clustering to refine the division of stages in China's AIDS epidemic and differentiate geographical areas based on development patterns. We then use hotspot analysis to describe the current status of AIDS, presenting

A cluster of acute hepatitis E infection in United Nations Bangladeshi peacekeepers in Haiti.

In the fall of 1995, within a month of deployment to Haiti for peacekeeping duty, four Bangladeshi soldiers developed acute icteric hepatitis in rapid succession. Hepatitis E virus (HEV) was found to be the etiology by demonstrating HEV genomic sequences in serum samples by the polymerase chain reaction (PCR) and serologically by the detection of elevated IgM titers to HEV. No case had serologic evidence of acute hepatitis A or C infection. The soldiers had probably acquired their infection while living in a cantonment area outside Dhaka, Bangladesh for one month prior to deployment. Cloning a

Scabies in chronic health care institutions.

BACKGROUND: The diagnosis and management of outbreaks of scabies in health care facilities may be difficult. As there is little information available regarding the extent of this problem we conducted a questionnaire study to provide an indication of the occurrence of scabies in chronic health care facilities in the province of Ontario, Canada. It also documented the facilities' approach to the management of the disease. OBSERVATIONS: Two outbreaks of scabies in chronic health care facilities are described. The questionnaire study revealed that 25% of the institutions reported that they had pr

Lyme disease. The hidden pandemic.

Physicians will recognize Lyme disease faster if they maintain a high index of suspicion in a young patient with arthritis accompanied by negative rheumatoid factor and antinuclear antibody in combination with cardiac conduction problems or lymphocytic meningitis. The Lyme spirochete (Borrelia burgdorferi) has notable sensitivity to tetracycline, penicillin, and erythromycin; therefore, proper and complete treatment of the disease, once it is identified, can be easily achieved. Finkel observed that Lyme disease manifests itself as a "great imitator," as do many disorders caused by a spirochete
