Subclavian artery puncture repair with Angio-Seal deployment.

BACKGROUND: Inadvertent placement of a port-a-catheter in an artery during central venous cannulation is rare and can result in devastating complications. Although traditional closure devices have been employed as prompt and adequate treatment, more innovative devices such as collagen plugs are being studied for their efficacy. CASE: We report a case involving a 63-year-old woman who was treated with chemotherapy for recurrent metastatic uterine leiomyosarcoma. During port-a-catheter placement, puncture of the subclavian artery occurred. Vascular surgery and interventional radiology consultat

Large-bore arterial access in the era of structural cardiovascular disease.

Over the last two decades, the medical community witnessed an outstanding and accelerated development on minimally invasive therapies. The definition of what constitutes large bore access is subject to discussion, however within the field it is generally accepted to reflect a catheter diameter exceeding 8-French. We sought in this review to explore the evolution, characteristics and vascular compatibility of the current commercially available devices, analyze the devices along with access site-specific complications rates and finally review the present methods for percutaneous vascular closure

Nitinol clip distal migration and resultant popliteo-tibial artery occlusion complicating access closure by the StarClose SE vascular closure system.

Lower extremity ischemia following deployment of a vascular closure device for access site closure after a transfemoral endovascular procedure rarely occurs. A 68-year-old woman diagnosed with subarachnoid hemorrhage due to a ruptured anterior communicating aneurysm was treated by endovascular coil embolization. The StarClose SE device was deployed for right femoral arteriotomy closure. After 2 days, critical ischemia occurred on her right lower leg due to total occlusion of the popliteo-tibial artery. Emergent surgical embolectomy was performed and the nitinol clip of the StarClose device was

Transcathater percutaneous device closure of a large PDA closed in a 3 kg infant.

Patent ductus arteriosus (PDA) is common congenital heart disease which may require treatment as transcatheter percutaneous device closure (PDA device closure) or surgical ligation in symptomatic full-term patients. Surgical ligation is an invasive procedure and has more complications especially in the neonates. Problems in PDA device closure are difficult vascular access, manipulation of catheters and sheath, residual shunts, residual obstruction of major arteries etc and these complications increase in low birth weight babies, but it is a less invasive procedure and has fewer complications t

Femoral Closure with Single ProGlide® in Transcatheter Aortic Valve Implantation: A Registry-Based Study.

Background: Vascular closure of the femoral artery during transcatheter aortic valve implantation (TAVI) remains a critical step prone to complications, despite advancements in introducer technology. The traditional technique involves using two ProGlide® suture closure devices (2P), but alternative approaches, such as employing a single ProGlide® device (1P), have emerged. Aims: We sought to evaluate the efficacy and safety of the 1P strategy compared to the standard 2P closure technique during transfemoral TAVI procedures. Methods: A registry-based study was conducted at the University Hospit

Ventricular Septal Defect Closure Devices, Techniques, and Outcomes.

Transcatheter closure of ventricular septal defects (VSDs) was first documented in 1988. The early studies of VSD closure were successful but there were high complication rates, particularly early and late-onset complete heart block. However, the development and use of new vascular occlusion devices in an off-label fashion has improved the range of patients who can be treated and reduced the complication rates. In particular, the rate of complete heart block documented in contemporary studies of VSD closure has fallen to levels at or below those documented in the surgical VSD closure literatur

Inadvertent stenting of patent ductus arteriosus with Amplatzer Vascular Plug.

We report a 12-month old patient who presented for murmur evaluation after percutaneous closure of type C patent ductus arteriosus (PDA) using a 10 mm Amplatzer Vascular Plug (AGA Medical Corporation, Golden Valley, MN) at an outside institution. Echocardiography revealed a large left-to-right shunt through the implanted device, inadvertently stenting the PDA instead of closing it. The patient underwent repeat catheterization with successful coil implantation within the Amplatzer Vascular Plug, completely eliminating the large residual ductal shunt. Although challenging, this case illustrates

Hybrid approach to surgical correction of tetralogy of Fallot in all patients with functioning Blalock Taussig shunts.

BACKGROUND: In total surgical correction of tetralogy of Fallot (TOF) with functioning Blalock Taussig shunts (BTS), shunt take down increased surgical time, bleeding, and might injure phrenic and recurrent laryngeal nerve and thoracic duct. OBJECTIVES: A routine hybrid approach using transcatheter BTS closure immediately before total surgical correction of TOF in all patients might reduce these problems. We analyze the safety and feasibility of this approach. METHODS: Transcatheter BTS closure was achieved using single or multiple stainless steel embolization coils, Amplatzer vascular plugs

Applying a clampless haemostatic device for left ventricular assist device outflow graft anastomosis.

Performing a left ventricular assist device outflow graft anastomosis in redo cases with a short ascending aorta and open vein grafts requires side clamping of the aorta and may necessitate temporary clamping of the vein grafts to optimize the surgical field. We aim to describe the technique and report our experience with applying the HeartString® device for the anastomosis of the left ventricular assist device outflow graft in 2 male patients with ischaemic cardiomyopathy following coronary artery bypass surgery. The feasibility of using the HeartString device has been shown and no procedure-

Ultrasound-guided deployment of ProGlide™ device in transfemoral transcatheter aortic valve implantation and risk reduction of vascular complications: A propensity-matched cohort study.

BACKGROUND: ProGlide is a percutaneous suture-mediated closure device used in arterial and venous closure following percutaneous intervention. Risk of vascular complications from use, particularly related to failure in hemostasis, or acute vessel closure, remains significant and often related to improper suture deployment. We describe a technique of ultrasound-guided ProGlide deployment in transfemoral transcatheter aortic valve implantation (TF-TAVI). AIMS: The aim of this study is to assess vascular outcomes for ultrasound-guided deployment of ProGlide vascular closure devices in patients u…

A Comparison of Two Vascular Closure Strategies in Transcatheter Aortic Valve Replacement: Suture and Plug versus Suture Alone - A Systematic Review and Meta-Analysis.

INTRODUCTION: Vascular complications following transcatheter aortic valve replacement (TAVR) significantly contribute to morbidity and mortality. Conventional suture-based closure technique has been widely utilized for large-bore arterial access closure. Recent findings on hybrid strategy combining plug and suture-based devices has been on spotlight as it may improve the hemostatic efficacy and lower the access-site related complications and clinical outcomes. METHODS: We performed a systematic review and meta-analysis of studies comparing a suture-based approach with a hybrid closure strateg

Comparative Study of Single- Versus Double-ProGlide Device Strategy for Access Site Closure of Transfemoral Transcatheter Aortic Valve Replacement.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) is a first-line treatment for severe symptomatic aortic stenosis (AS) patients, and the preferred approach for TAVR is transfemoral. The double-ProGlide preclose technique offers an effective and safe method for closure of transfemoral access site after TAVR. However, the debate is still ongoing whether a single-ProGlide strategy is equal or even superior to a double one in terms of hemostasis and vascular complications. AIMS: The aim of this study is to examine the safety and efficacy of a single- versus double-ProGlide strategy for v

Interventional pediatric cardiology: device closures.

Surgical treatment of various septal defects has been long established. With the advances in transcatheter therapy dilatation techniques for valular stenosis and vascular obstruction have become established procedures. Closure of septal defects in the catheterization laboratory has also been introduced; some of these have come into regular use in current practice. In 1967, Porstmann et al reported the use of Ivalon plug to close patent ductus arteriosus (PDA). Since then, several devices have been used including Rashkind PDA occluder (not being used now), Gianturco coils, detachable coils (for
