[Rehabilitation of anoxic brain injured patients after reanimation].

Authors report rehabilitation outcomes of anoxic brain injured patients, following reanimation. In the past 12 years (1988-1999) they treated 54 patients. The patients were selected from other hospitals after successful reanimation, but sustained diffuse anoxic brain injury. The CT and MR examination was generally negative, but small hypodensities were shown rarely (18/54). At the time of admission 21 patients were quadriparetic, 5 patients were hemiparetic, 2 were paraparetic and there were no any paresis in 26 cases. The mean age of the patients was 32 years (7-61). At the time of admission

Ion-pairing high-performance liquid chromatographic method for the detection of N-acetylaspartate and N-acetylglutamate in cerebral tissue extracts.

An ion-pairing high-performance liquid chromatographic method for the determination of N-acetylaspartate and N-acetylglutamate using a C-18 column and a UV detection at 210 nm wavelength, by means of a diode array detector, is presented. A buffer containing 2.8 mM tetrabutylammonium hydroxide, 25 mM KH(2)PO(4), 1.25% methanol, pH 7. 00, is utilized for the isocratic separation of these N-acetylated amino acids, at a flow rate of 1 ml/min and a column temperature of 23 degrees C. The suitability of this chromatographic separation (without additional chromatographic steps prior to HPLC assay) to

Category specific semantic loss in dementia of Alzheimer's type. Functional-anatomical correlations from cross-sectional analyses.

In the context of focal brain injury, selective loss of semantic knowledge in the domain of either natural kinds or artefacts is usually considered to reflect the differential importance of temporal and frontoparietal regions to the representations of perceptual and functional attributes, respectively. It is harder to account far as a feature of a more diffuse process, and previous cross-sectional analyses of patients with dementia of Alzheimer's type (DAT) have differed over whether category effects occur. In our series of 58 patients with probable DAT, we demonstrated a significant group adv

Traumatic brain injury, posttraumatic stress disorder, and vascular risk are independently associated with white matter aging in Vietnam-Era veterans.

OBJECTIVE: Traumatic brain injury (TBI), mental health conditions (e.g., posttraumatic stress disorder [PTSD]), and vascular comorbidities (e.g., hypertension, diabetes) are highly prevalent in the Veteran population and may exacerbate age-related changes to cerebral white matter (WM). Our study examined (1) relationships between health conditions-TBI history, PTSD, and vascular risk-and cerebral WM micro- and macrostructure, and (2) associations between WM measures and cognition. METHOD: We analyzed diffusion tensor images from 183 older male Veterans (mean age = 69.18; SD = 3.61) with (n =

Posterior reversible encephalopathy syndrome following eclampsia in a primigravida at 35 weeks of gestation: A case report.

Posterior reversible encephalopathy syndrome (PRES) is a reversible neurological disorder most commonly triggered by eclampsia and characterized on imaging by vasogenic edema, typically involving the parieto-occipital regions. We describe a 26-year-old primigravida at 35 weeks of gestation who presented with headache, dyspnea, and a generalized tonic-clonic seizure, with severe hypertension (200/110 mmHg), 3+ proteinuria, serum creatinine (1.1 mg/dL), and platelet count (100,000/mm3) consistent with severe preeclampsia with superimposed eclampsia. She underwent emergency lower-segment cesarean

Radiological findings in fulminant cerebral malaria: a rare neuroimaging presentation.

BACKGROUND: Cerebral malaria (CM) is a life-threatening complication of Plasmodium falciparum infection, characterized by coma, seizures, and multiorgan dysfunction. MRI findings in adult CM are underreported, particularly in cases with fulminant progression despite treatment. CASE PRESENTATION: A 54-year-old woman with a history of hypertension and hypothyroidism presented with fever, seizures, and altered sensorium. On admission, the patient was in shock with severe metabolic acidosis and multiorgan failure. A rapid diagnostic test and peripheral smear confirmed P. falciparum infection. CT

Altered white matter diffusivity and subtle motor function in a pilot cohort of adolescents with sports-related concussion.

BACKGROUND AND OBJECTIVE: Adolescents with sports-related concussion (SRC) demonstrate acute and persistent deficits in subtle motor function. However, there is limited research examining related neurological underpinnings. This pilot study examined changes in motor-associated white matter pathways using diffusion tensor imaging (DTI) and their relationship with subtle motor function. METHODS: Twelve adolescents with SRC (12-17 years) within two-weeks post-injury and 13 never-injured neurotypical peers completed DTI scanning. A subset of 6 adolescents with SRC returned for a follow-up visit p

Indications for the performance of neuroimaging in children.

Pediatric neurology relies on ultrasound, computed tomography (CT), and magnetic resonance (MR) imaging. CT prevails in acute neurologic presentations, including traumatic brain injury (TBI), nontraumatic coma, stroke, and status epilepticus, because of easy availability, with images of diagnostic quality, e.g., to exclude hemorrhage, usually completed quickly enough to avoid sedation. Concerns over the risks of ionizing radiation mean re-imaging and higher-dose procedures, e.g., arteriography and venography, require justification. T1/T2-weighted imaging (T1/T2-WI) MR with additional sequences

CST recovery in pediatric hemiplegic patients: Diffusion tensor tractography study.

Many diffusion tensor imaging (DTI) studies have reported an association between corticospinal tract (CST) injury and motor dysfunction. In this study, we investigated CST recovery in 29 pediatric patients with clinical hemiplegia using DTI. We measured the fractional anisotropy (FA), apparent diffusion coefficient (ADC), and asymmetric anisotropy (AA) of both CSTs. The patients were classified into three groups according to severity of CST disruption of the more affected hemisphere. DTI was followed up for 9.34 ± 2.07 months after initial evaluation. The FA value of the more affected CST show

Imaging neurodegeneration in the mouse hippocampus after neonatal hypoxia-ischemia using oscillating gradient diffusion MRI.

PURPOSE: To investigate if frequency-dependent contrasts using oscillating gradient diffusion MRI (dMRI) can detect hypoxia-ischemia (HI) -induced neurodegeneration in the neonatal mouse hippocampus. METHODS: Pulsed- and oscillating-gradient dMR images (at 50, 100, and 150 Hz) were acquired from postmortem fixed brains of mice exposed to neonatal HI using the Rice-Vanucci model. MRI data were acquired at 1, 4, and 8 days following HI, and compared with histological data from the same mice for in situ histological validation of the MRI findings. RESULTS: The rate of change of apparent diffusi

Magnetic resonance imaging, risk factors and co-morbidities in children with cerebral palsy.

Cerebral palsy (CP) continues to be a major problem in India. The present study provides an insight into the various clinical and neuroradiological correlates of CP. The study included 102 children with CP and was subjected to magnetic resonance imaging (MRI) of the brain. Forty-seven (46%) patients belonged to the 1-3 years age group and 84 (82%) were born at term. Of 102 children, 39 (38%) were delivered at home. Based on their tone and topographic pattern of weakness, it was found that 47 (46%) had spastic diplegia and 35 (34%) spastic quadriplegia. Hemiplegic, dystonic, and atonic CP accou

Electrophysiological evaluation of cranial neuropathies.

BACKGROUND: Cranial nerve abnormalities occur frequently in both focal and diffuse neurologic disorders and can be evaluated by electrophysiological techniques available in most clinical neurophysiology laboratories. REVIEW SUMMARY: The optic nerve is evaluated by visual evoked potentials. Measurements of latency, amplitude, and waveform morphology are especially useful in detecting demyelinating lesions. Brain stem auditory evoked potentials evaluate the auditory portion of the eighth cranial nerve. Using an auditory stimulus, a number of waveforms are generated, and changes in the normal pa

Cognitive impairment in children with hemoglobin SS sickle cell disease: relationship to MR imaging findings and hematocrit.

BACKGROUND AND PURPOSE: Children with hemoglobin SS sickle cell disease are known to suffer cognitive impairment if they have silent infarct, but recent evidence suggests that patients with hemoglobin SS sickle cell disease may be impaired even if they are free of infarction. We test a hypothesis that cognitive impairment in children with hemoglobin SS sickle cell disease is associated with low hematocrit and MR imaging abnormalities. METHODS: A cohort of 49 patients was examined, all of whom had hemoglobin SS sickle cell disease but no history of clinical stroke. The Wechsler scales, which a

Diffuse cerebral microhemorrhages in West Nile virus neuroinvasive disease: a case report.

West Nile virus neuroinvasive disease (WNND) most commonly presents as meningitis, encephalitis, or acute flaccid paralysis, with variable neuroimaging findings. While MRI abnormalities classically involve deep gray matter and brainstem structures, hemorrhagic manifestations are rarely reported. We report a 77-year-old woman who presented with rapidly progressive altered mental status following a brief febrile prodrome. Cerebrospinal fluid studies demonstrated a lymphocytic pleocytosis consistent with aseptic meningitis. Brain MRI revealed an acute lacunar infarct and diffuse cerebral microhem

Dementia After Moderate-Severe Traumatic Brain Injury: Coexistence of Multiple Proteinopathies.

We report the clinical, neuroimaging, and neuropathologic characteristics of 2 patients who developed early onset dementia after a moderate-severe traumatic brain injury (TBI). Neuropathological evaluation revealed abundant β-amyloid neuritic and cored plaques, diffuse β-amyloid plaques, and frequent hyperphosphorylated-tau neurofibrillary tangles (NFT) involving much of the cortex, including insula and mammillary bodies in both cases. Case 1 additionally showed NFTs in both the superficial and deep cortical layers, occasional perivascular and depth-of-sulci NFTs, and parietal white matter rar

Structure of the Medullary Veins of the Cerebral Hemisphere and Related Disorders.

Deep medullary veins drain into subependymal veins with four convergence zones and show parallel distribution patterns adjacent to the body or inferior horn and a radial pattern in the frontal horn or trigon of the lateral ventricle. As white matter imaging develops such as diffusion tensor imaging or susceptibility-weighted imaging, requirements for understanding of white matter structures are increasing, not only for understanding of neuronal tracts but also for that of other structures including the fine anatomy of white matter vessels. Some disorders are related to deep medullary veins and

Regional alterations of ATP and heat-shock protein-72 mRNA following hypoxia-ischemia in neonatal rat brain.

Neonatal rats, 7 days of age, underwent unilateral carotid artery ligation followed by exposure to hypoxia (8% O2) for 80 min. At the end of the period of hypoxia, and after recovery for 2 or 24 h, regional levels of ATP and heat-shock protein-72 (hsp72) mRNA were measured in adjacent brain sections using ATP-luminescence histochemistry and in situ hybridization, respectively. At the end of hypoxia, ATP levels were decreased in a patchy pattern within the hemisphere ipsilateral to the carotid ligation. In the parietal cortex, the reduction of ATP often occurred in columns oriented perpendicula
