Experience with patients presenting with the clinical features of Holt-Oram syndrome: a single center retrospective study.

BACKGROUND: Holt-Oram syndrome, also known as atrio-digital syndrome, is a rare autosomal dominant genetic disorder primarily characterized by upper limb malformations and congenital heart defects, associated with mutations in the TBX5 gene. Diagnosis is typically based on clinical presentation, and the prognosis for affected individuals is closely related to the severity of cardiac involvement. Our study aims to highlight some key features of Holt-Oram syndrome to improve the level of clinical diagnosis. METHODS: We collected 11 patients with clinical features strongly suggestive of Holt-Ora

Cone reconstruction for severe tricuspid valve dysplasia in a dog.

Tricuspid valve dysplasia (TVD) is a congenital cardiac malformation in dogs characterized by abnormal tricuspid valve morphology, leaflet tethering, impaired leaflet coaptation, and right atrial and ventricular remodelling. This report describes cone reconstruction for severe TVD in a dog. A three-year-and-10-month-old female Labrador retriever presented with a grade IV/VI systolic murmur and exercise intolerance. Echocardiography revealed severe tricuspid regurgitation, a right atrial-to-left atrial ratio of 1.57, malformed and tethered tricuspid valve leaflets, impaired leaflet coaptation,

Diagnosis of Congenital Heart Disease in Adulthood: How Often, How Relevant?

Congenital heart disease (CHD) is typically detected during fetal life, infancy, or early childhood. However, there is no published data regarding the proportion of congenital heart defects that are diagnosed in adulthood or the impact of these defects. Retrospective analyses of all consecutive patients (n = 1,010) referred to an adult CHD unit between 2018 and 2023. We analyzed the proportion of cases diagnosed in adulthood, defining the type of defect, reasons for diagnosis, complications, and need for intervention. In total, 26.5% of patients were diagnosed in adulthood (mean age 47 ± 16 ye

Early outcomes of transcatheter tricuspid valve-in-valve implantation: a case series.

OBJECTIVES: In this study, we sought to evaluate early outcomes of transcatheter valve-in-valve implantation in patients with a degenerated bioprosthesis in the tricuspid position. METHODS: We used a prospective study, which included patients with a degenerated bioprosthesis in the tricuspid position who were considered high-risk by our heart team and who underwent a valve-in-valve implantation. The procedures were performed via transjugular venous access and were done with the patient under general anaesthesia with transoesophageal echocardiographic and fluoroscopic guidance. Perioperative d

Radiofrequency ablation of the cavotricuspid isthmus for management of atrial flutter in patients with congenital heart disease after tricuspid valve surgery: A single-center experience.

BACKGROUND: Typical atrial flutter involving the cavotricuspid isthmus (CTI) is the most common reentrant arrhythmia in congenital heart disease and ablation is effective in its management. However, congenital heart disease patients often require surgical interventions on their tricuspid valve that utilize prosthetic material, making CTI ablation technically challenging. OBJECTIVE: To describe the techniques and outcomes of CTI ablation in the presence of prior tricuspid valve repair or replacement. METHODS: We included all patients who had undergone tricuspid valve repair utilizing an annul

Durability of bioprostheses for the tricuspid valve in patients with congenital heart disease.

OBJECTIVES: Only little data exist on the durability of bioprostheses in the tricuspid position in patients with congenital heart disease (CHD). The aim of the study was to determine the reoperation rate and the valve function after primary implantation. METHODS: Between 1990 and 2013, 51 patients with CHD underwent tricuspid valve (TV) replacement with a bioprosthesis. The median age at operation was 32 years (range: 8-69). The underlying morphology was Ebstein's anomaly in 62% of the patients. Implanted valves included 38 pericardial and 13 porcine valves. All available echocardiographic ex

Cardiothoracic ratio from postero-anterior chest radiographs: a simple, reproducible and independent marker of disease severity and outcome in adults with congenital heart disease.

OBJECTIVE: The wide spectrum of intracardiac anatomy and reparative surgery available for adults with congenital heart disease (ACHD) makes uniform measurement of cardiac size and disease severity challenging. The aim of this study was to assess the prognostic potential of cardiothoracic ratio, a simple marker of cardiomegaly, in a large cohort of ACHD. PATIENTS AND SETTING: Chest radiographs from 3033 ACHD patients attending our institution between 1998 and 2007 and 113 normal controls of similar age were analyzed blindly. DESIGN: Cardiothoracic ratio derived from plain postero-anterior che

Is left ventricular noncompaction in children truly an isolated lesion?

Left ventricular noncompaction (LVNC) is a form of cardiomyopathy resulting from a disorder of endomyocardial morphogenesis. It has been associated with significant morbidity and mortality. The aim of this study was to characterize associated cardiac findings in children with LVNC and to identify risk factors associated with increased mortality. From our echocardiography database, we identified 46 patients diagnosed with LVNC between December 1999 and February 2005. The mean age at presentation was 3.6 +/- 5.6 years, and the mean duration of follow-up was 1.9 +/- 2.1 years. Left ventricular ej

Consensus on timing of intervention for common congenital heart diseases: part II - cyanotic heart defects.

The purpose of this review/editorial is to discuss how and when to treat the most common cyanotic congenital heart defects (CHDs); the discussion of acyanotic heart defects was presented in a previous editorial. By and large, the indications and timing of intervention are decided by the severity of the lesion. While some patients with acyanotic CHD may not require surgical or transcatheter intervention because of spontaneous resolution of the defect or mildness of the defect, the majority of cyanotic CHD will require intervention, mostly surgical. Total surgical correction is the treatment of

The incidence of postoperative arrhythmia was 35.2% in 264 cases with complex congenital heart diseases underwent open heart surgery between Feb, 1988 and Aug, 1989. Postoperative arrhythmia was closely related to preoperative hematocrit, cardiomegaly, prolonged aortic occlusion, ratio of RV/LV systolic pressure greater than 0.75, hypokalemia and low cardiac output syndrome. Infusion of colloid solution and both potassium and magnesium as well as successful surgery and the preventive administration of antiarrhythmic agent played an important role in the prevention and treatment of postoperativ
