Beyond the Bottle: Lessons From the Laser in Glaucoma and Ocular Hypertension Trial.

PURPOSE: To review contemporary evidence supporting selective laser trabeculoplasty (SLT) as an initial and longitudinal therapy for glaucoma and to examine its implications for clinical outcomes, functional disease control, and health system sustainability. RECENT FINDINGS: Randomized clinical trials and extension studies have demonstrated that SLT-first strategies provide intraocular pressure (IOP) control comparable to medication-first therapy while substantially reducing treatment burden. In the Laser in Glaucoma and Ocular Hypertension (LiGHT) Trial, ∼70% of patients initially treated wi

Navigating the Angles: A Retrospective Analysis of One-year Outcomes of Gonioscopy-assisted Transluminal Trabeculotomy in Primary Open-angle Glaucoma.

This study aimed to evaluate postoperative angle changes following gonioscopy-assisted transluminal trabeculotomy (GATT) in patients with primary open-angle glaucoma (POAG) over 1 year of follow-up. We conducted a retrospective analysis of 60 eyes from 41 POAG patients who underwent GATT, assessing intraocular pressure (IOP), number of ocular hypotensive medications, and gonioscopic findings preoperatively and at 3, 6, and 12 months postoperatively using the Spaeth classification. Postoperative IOP and medication use were significantly reduced throughout follow-up. Gonioscopic examination reve

Minimally invasive glaucoma surgery in the surgical landscape of primary angle closure glaucoma: Current innovations and future trends.

Primary angle-closure glaucoma (PACG) is a major cause of irreversible blindness worldwide, with the highest burden in Asia. Its pathogenesis involves the interplay of multiple mechanisms, including pupillary block, plateau iris configuration, lens-related crowding, and dynamic choroidal expansion. Traditional surgical management has relied on trabeculectomy, lens extraction, or transscleral cyclophotocoagulation, which often achieve substantial intraocular pressure (IOP) control but carry significant risks and require prolonged recovery. Minimally invasive glaucoma surgery (MIGS) expands trea…

Minimally invasive glaucoma surgery in angle closure.

PURPOSE OF REVIEW: This review discusses the evidence on the efficacy, safety and role of minimally invasive glaucoma surgery (MIGS) in eyes with angle closure glaucoma. While cataract surgery remains the most established surgical treatment for primary angle closure glaucoma (PACG), the intraocular pressure (IOP) may remain elevated after cataract surgery despite open angles due to trabecular meshwork damage from chronic iridotrabecular contact. RECENT FINDINGS: There is emerging evidence that combining cataract surgery with MIGS in eyes with PACG, though an off-label indication for some MIGS

Inadvertent filtering bleb due to extracapsular cataract extraction wound reopening after mitomycin C use: a case report.

BACKGROUND: Mitomycin C has been used adjunctively in various procedures, including pterygium excision. Delayed wound healing, the long-term complication of mitomycin C, can occur several years later and may rarely cause a subsequent inadvertent filtering bleb. However, conjunctival bleb formation from the reopening of an adjacent surgical wound after mitomycin C use has not been reported. CASE PRESENTATION: A 91-year-old Thai woman had undergone pterygium excision 26 years ago, with adjunctive mitomycin C, as well as an uneventful extracapsular cataract extraction in the same year. The patie
