[Comparison of execution time and tissue damage by using four ready-to-use percutaneous emergency airway access devices available in Poland].

OBJECTIVE: Objective: Assessment of execution time and tissue damage during providing percutaneous airway access using four ready-to-use sets: Quicktrach I (Q1) and II (Q2), Portex Cricothyroidothomy Kit (PCK) and Surgicric I (SC). PATIENTS AND METHODS: Material and methods:Prospective laboratory test. Each of 54 participants carried out 1 puncture on porcine larynx preparation for each of 4 sets, which resulted in a total of 216 trials. Efficacy was defined as placing a cannula in the tracheal lumen ensuring adequate ventilation. Safety was assessed based on the size and type of laryngeal da

Laryngotracheal reconstruction and cricotracheal resection in children: recent experience at Great Ormond Street Hospital.

BACKGROUND: Surgery for paediatric airway stenosis is constantly evolving. Surgery is the primary treatment modality via either an open or endoscopic approach. The objective of this study was to review the results of laryngotracheal reconstruction (LTR) and cricotracheal resection (CTR) procedures performed at Great Ormond Street Hospital over the past 10 years. METHODS: All patients who underwent open airway reconstruction surgery from January 2000 to December 2010 were included in this study. Patients treated entirely endoscopically were excluded. The data was collected using the electronic

Laryngeal lipoma associated with Madelung's disease: a case report.

Multiple symmetric lipomatosis is an alteration in the neck, upper trunk and upper extremities fat deposits. It produces an aesthetic problem and sometimes upper airway obstruction when the larynx is infiltrated by the mass. We report the case of a male with Madelung's disease, which began with acute dyspnea caused by laryngeal fat deposits and obstructive lipoma.

Site involvement as a predictor of airway intervention in angioedema.

OBJECTIVES/HYPOTHESIS: To look for a relationship between which sites are involved in angioedema and the need for airway intervention (intubation, tracheotomy). STUDY DESIGN: Retrospective chart review. METHODS: Charts of 140 patients who were admitted to two hospitals at an academic medical center between July 1, 2006 and June 30, 2008 with the diagnosis of angioedema were reviewed. Charts were reviewed for pertinent data, including demographics, sites of involvement along the upper airway, medical therapy, and airway intervention. Subsites included lips, anterior tongue, floor of mouth, so…

Postoperative tracheal rupture in a patient with a difficult airway.

PURPOSE: Tracheal rupture is an uncommon and potentially life-threatening event. This report presents a case of postoperative tracheal rupture in a patient with a known difficult airway presenting to a rural hospital. CLINICAL FEATURES: A 29-yr-old man presented to a rural hospital with sudden onset neck pain and progressive dyspnea. Five days earlier the patient had undergone tracheal resection for tracheal stenosis related to prolonged intubation. The patient informed the emergency room staff that the attending anesthesiologist had made note of a "difficult airway". The community hospital h

CT and virtual endoscopy findings in congenital laryngeal web.

Laryngeal web is a rare lesion resulting from incomplete recanalization of the primitive larynx. Because the extent of airway involvement affects surgical management, patients should be studied thoroughly before treatment. Although the diagnosis is based on the endoscopy findings in most of the cases, laryngoscopy may fail to show the subglottic extension of the disease. Virtual endoscopy may provide the information needed for surgery in such cases. Here, we report the computed tomography and virtual endoscopy findings in a 5-year-old child with a laryngeal web.

A case of idiopathic tracheal stenosis.

Idiopathic tracheal stenosis (ITS) is an extremely rare disease. We report the case of a 32-year-old woman with ITS. She had no history of previous surgery, endotracheal intubation, neck trauma, granulomatous disease, or any other severe respiratory tract infections. She presented with progressive dyspnea on effort and had been treated for bronchial asthma for 3 years. Chest radiography and laboratory examinations revealed no abnormalities. Bronchoscopy demonstrated almost circumferential tracheal stenosis extending for 10 mm from about 20 mm below the vocal cords. Luminal diameter was about 4

[Cicatricial stenoses of the trachea. Etiological and therapeutic aspects. Apropos of thirty operated patients].

From January 1984 to march 1990, we performed 30 tracheal resections for post-intubation or post-tracheostomy inflammatory stenosis. After discussing the mechanism and the pathology of the tracheal lesions, we emphasise the importance of careful history taking when evaluating a patient with tracheal stenosis in order to distinguish patients with normal lungs (who needed respiratory resuscitation for acute and reversible respiratory failure) from patients with compromised lungs (patients with chronic pulmonary disease who needed respiratory resuscitation for acute respiratory failure). When sel

Surgical treatment for laryngeal paralysis in infants and children.

Clinical and endoscopic data of 219 cases of laryngeal paralysis in newborns, infants, and children are briefly reported. The management of severe cases of persistent dyspnea then is discussed, according to the literature. Of 219 cases, 22 young patients underwent a surgical procedure because of lack of spontaneous recovery and poor tolerance of their disease after 6 to 9 months of follow-up. Arytenoidectomy technique has been used three times and arytenoidopexy 19 times, with fair to excellent results. Other possible treatments for infants are discussed. On the basis of this important series

Foreign bodies in the tracheobronchial tree. Special references to experience in 97 children.

The case records of 110 patients (97 children and 13 grown-ups) with foreign bodies in the tracheobronchial tree during a period of 14 years (1970-1983) are reviewed. Atypical histories, misleading clinical and radiological findings and delayed diagnoses due to misinterpretations by the patients or their physicians were seen from time to time. Other findings in this survey were that organic material prevailed, with the majority of the objects in the right bronchial tree; there was a male predominance and the most common age for inhalation of a foreign body was about two years. Bronchoscopic ex

Velo-pharyngo-laryngeal myoclonus: evaluation of objective tinnitus and extrathoracic airway obstruction.

Velo-pharyngo-laryngeal myoclonus, the rapid, rhythmic contraction of muscles of the pharynx and larynx, is a rare neurological manifestation of numerous disease processes affecting the cerebellum. In its most common form, palatal myoclonus, this disease frequently presents to the otolaryngologist as objective tinnitus. Impedance audiometry provides a useful means of verifying suspected palatal myoclonic activity through recorded changes in the middle ear pressure, as mediated by muscle activity at the proximal portion of the eustachian tube. A very rare case of velo-pharyngo-laryngeal myoclon

Acute epiglottitis in adults.

We treated four adults whose upper airway was compromised due to acute epiglottitis. We also reviewed the English literature for all reports of this condition in adults (18 years and older). Among the 158 cases, the infectious etiology was identified in 29 (H. influenzae 20, Streptococcus pneumoniae six, H. parainfluenzae two, Streptococcus pyogenes one). In the remaining cases, the etiology was uncertain. Bacteremia was documented in 23/32 patients (71.9%), but extra-epiglottic infections were strikingly rare (X = six). The clinical manifestations were sore throat (100%), fever (88%), dyspnea

Congenital laryngeal hemangioma.

Congenital subglottic hemangioma consistently occurs in a site below the vocal cord. It is a distinct clinical entity with typical clinical features. However, congenital hemangioma in other sites can cause airway obstruction. An anatomical classification is proposed on the basis of review of 23 consecutive cases. It is important to accurately identify the site and distribution of the lesion before commencing treatment. Radiation has been an acknowledged form of treatment, and in our hospital we have confirmed that radiation therapy, either by external beam, or by insertion of a radioactive gol

Comparison of esophageal, tracheal, and mouth occlusion pressure in patients with chronic obstructive pulmonary disease during acute respiratory failure.

In 14 acutely ill patients with chronic obstructive pulmonary disease who were either intubated or breathed via a tracheostomy, we measured the pressures generated in the esophagus and trachea during the first 0.1 s of spontaneous inspiratory efforts against closed airway (P0.1). No significant difference was found between P0.1 in the trachea and esophagus, where it was measured at 2 balloon levels (distance from balloon tip to nares: 35 and 45 cm). In 4 of the patients we also compared esophageal and mouth occlusion pressures during mouth breathing, the orifice of tracheostomy being temporari

[Costal stapling in severe thoracic traumas (author's transl)].

Thirty-two of 320 patients with severe thoracic trauma were treated with costal stapling. The technique, described by R. Judet in 1973, is basically simple but sometimes difficult to perform. In the most favourable cases (here 23/32 patients), it avoids tracheostomy and its complications and reduces the restrictive sequelae associated with pleural adhesion or thoracic wall deformities. In addition, active respiratory physiotherapy can be undertaken at an earlier stage. The authors describe the indications and limitations of this treatment on the basis of the results obtained in this series.
