Cystic tumors account for 15% of pancreatic tumors. Of these, serous microcystic adenomas represent 1-2% of pancreatic exocrine neoplasms. While typically benign, a small percentage possess malignant potential. Given imaging improvements, serous cystadenomas are being identified more frequently. A 63-year-old female was admitted with complaints of jaundice and unintentional weight loss. Abdominal computed tomography scan showed a 16 cm obstructive pancreatic mass near the porta hepatis region. Endoscopic ultrasonography and fine needle aspiration biopsy indicated a large pancreatic head cystic

BACKGROUND: According to the literature, benign bile duct tumors are exceedingly uncommon. To the best of our knowledge, we report the largest extrahepatic bile duct villous adenoma described in the literature.
CASE PRESENTATION: We present a case of a 77-year-old Caucasian woman with obstructive jaundice. Laboratory tests revealed that she had elevated bilirubin and liver enzyme levels. A computed tomographic scan showed a homogeneous 5 × 3-cm mass obstructing the common bile duct. The results of brush cytology were consistent with a bile duct villous papilloma. However, on the basis of the

We report the case of a 69-year-old male patient with diagnoses of sigmoid colon cancer, ascending colon cancer, and metastatic liver cancer. We performed sigmoidectomy, right hemicolectomy, and central venous port placement. Because the liver metastasis was multifocal, chemotherapy was first initiated and then hepatic resection was performed. However, during chemotherapy, ileus, with a peritoneal dissemination to the small intestine, developed. Small intestine resection and radiation therapy to the pelvic region of the transition were further performed. Thereafter, obstructive jaundice due to

Biliary obstruction due to pancreaticobiliary malignancy is often accompanied by bactibilia and cholangitis; which are associated with postoperative septic complications and may adversely affect the clinical course. The enterococcus species is most responsible for postoperative complications in patients with malignant biliary obstruction. Here a case of bactibilia in a 60-year-old male patient, admitted with peri-ampullary carcinoma is reported. The bile sent for Gram-stain showed the presence of pus cells and Gram-positive cocci in pairs and the bacteriological culture revealed the growth of

We report two cases of malignant afferent loop obstruction following pancreaticoduodenectomy (PD). Case 1. A 70-year-old woman, who had undergone PD for pancreatic cancer, was referred to our hospital because of fever, jaundice, and abdominal pain. Ultrasonography and abdominal computed tomography demonstrated dilatation of a small bowel loop in the right upper quadrant. Laparotomy confirmed the diagnosis of local recurrent tumor causing occlusion of the afferent limb, and Roux-en-Y bypass was performed. Case 2. A 72-year-old man, who had undergone PD for cancer of the major papilla, was hospi

OBJECTIVE: To evaluate the diagnostic and therapeutic value of endoscopic retrograde cholangiopancreatography (ERCP) for biliary complications after liver transplantation.
METHOD: ERCP was performed via electronic duodenoscopy in 12 patients suspected of having biliary complications such as obstructive jaundice after orthotopic liver transplantation.
RESULTS: ERCP displayed common bile duct stenosis or obstruction in 9 cases (including 2 cases with also calculi), common bile duct calculi in 2 cases (including 1 complicated by bile duct dilation) and intrahepatic bile duct thinning in the don

Trichobezoars are accumulations of hair casts in the stomach associated with trichophagia. The typical patient is an adolescent female who presents with alopecia and an upper abdominal mass which on moving can cause intermittent gastric outlet obstruction. Only a minority of patients have severe psychiatric disorders. When hair strands extend from the main mass in the stomach, all along the small bowel and reaching the cecum, the condition is termed the Rapunzel syndrome. In about 5% of patients there are separate hair masses in the stomach. The clinical presentation includes abdominal pain, l

Endoscopically placed biliary endoprostheses were used to treat obstructive jaundice in 71 patients with ampullary carcinoma. Successful placement of an endoprosthesis was achieved in 68 patients (95.8%). Bilirubin declined in 67 patients (98.5%). There was no procedure-related mortality. Twenty-two patients (31%) received further surgical therapy, and 47 received an endoprosthesis as their only therapeutic intervention. In the latter group, bilirubin normalized in 44 of 46 patients surviving longer than 30 days (95.7%). Mean survival was 466 days (median 410, range 23-1515), which compares fa

The number of neonates with functional intestinal obstruction being admitted to this surgical unit is increasing. The obstruction may be transient and curable within a few days, or weeks or, more rarely, may be prolonged and intransigent, usually resulting in death despite parenteral nutrition. In the former group, the infants are often the result of an abnormal pregnancy or delivery. Recently, a small number of infants has been identified in whom prolonged jaundice is a notable feature, but the aetiology of this syndrome is not apparent. All of these infants must be screened to exclude mucovi

Endoscopic retrograde cholangiopancreatography was performed in 46 patients age 60 or older with various suspected pancreaticobiliary or duodenal diseases. Thirty-one patients (67.4%) had diagnostically satisfactory opacification of a duct system. Pathologic conditions obstruction injection of the contrast medium prevented opacification in eight patients (17.4%). Seven cannulations (15.2%) were technical failures. There was one complication, a Gram-negative septicemia that was successfully treated. The procedure is of value in the geriatric patient because it may provide preoperative evidence

Carcinosarcomas of the bile ducts are very rare tumors. Their diagnosis and management remain difficult. We here report the case of a patient in whom the diagnosis was suspected due to cholestatic jaundice associated with a deterioration in general condition. Biologically, bilirubin, alkaline phosphatase (ALP) and gamma-glutamyl transferase (GGT) levels were elevated. Imaging showed a tumor in the head of the pancreas. Surgical exploration, tumor biopsy and anatomo-immunohistochemical study confirmed the biliary origin. The purpose of our case report is to highlight the difficulties in diagnos

An 81-years-old female presented with obstructive jaundice and a non-specific clinical picture of nausea and appetite loss. Labs demonstrated a conjugated hyperbilirrubinemia (7.7 mg/dL), increased aspartate aminotransferase and alanine aminotransferase (10xULN and 8xULN, respectively), increased lactate dehydrogenase (10xULN) and serum lipase (3xULN). CA 19.9 was 342 U/mL (Ref value < 37 U/mL). There was no evidence of peripheral lymphadenopathy or hepatosplenomegaly. Imaging (Figure 1A and 1B) revealed a marked homogeneous enlargement of the pancreas (without any well-defined mass), dilation

Therapeutic plasma exchange (TPE) is an alternative treatment for hyperthyroidism, resulting in a rapid decline in plasma thyroid hormones and anti-thyroid antibodies. TPE has also been used both in primary liver disease and in drug-induced cholestasis. Data on thyrotoxic patients with severe hepatic complications are scarce. Cholestasis induced by imidazol-derived anti-thyroid drugs is extremely rare. The use of TPE for treating this complication was not previously reported. We report the experience of one such patient with a favorable response to TPE. A 45-year-old male patient with Graves'

Typhoid fever is a very common infectious disease, particularly in developing countries such as Sri Lanka. Although multiple organs are known to be affected by the disease, hepatic involvement could be considered the most important as studies have showed that it is associated with a higher relapse rate. We report a young patient who presented with fever and jaundice and found to have cholestatic hepatitis secondary to typhoid fever.

Prolonged cholestasis is a very rare complication of endoscopic retrograde cholangiography (ERC). Only few cases with this complication are reported in the English literature. We report persisting cholestatic jaundice in a 73-year old man after successful therapeutic ERC for choledocholithiasis. Serologic tests for viral and autoimmune hepatitis were all negative. A second-look ERC was normal also. He denied any medication except for prophylaxis given intravenous 1 g ceftriaxone prior to the ERC procedure. After an unsuccessful trial with ursodeoxycholic acid and cholestyramine for 2 wk, this

BACKGROUND: Feline infectious peritonitis (FIP) is a severe immune-mediated disease that develops in a small proportion of cats infected with feline coronavirus (FCoV). Clinical manifestations are variable and most commonly involve systemic inflammatory disease with or without effusion. Gastrointestinal signs, including vomiting or diarrhea, may be observed; however, focal intestinal lesions leading to mechanical obstruction are rare. Histopathologic changes resembling feline gastrointestinal eosinophilic sclerosing fibroplasia (FGESF) have only rarely been reported in association with FIP. CA
