Clinical and CT perfusion outcomes after direct STA-MCA bypass in moyamoya and non-moyamoya steno-occlusive disease: an Indonesian single-center cohort.

BACKGROUND: Direct superficial temporal artery-to-middle cerebral artery (STA-MCA) bypass is used for selected haemodynamic cerebrovascular disorders, but comparative data between moyamoya and non-moyamoya disease remain limited in Indonesia. METHODS: Consecutive patients undergoing direct STA-MCA bypass from 2021 to 2025 were retrospectively analysed. Clinical and operative outcomes were assessed in 68 patients; paired CT perfusion data were available in 41. RESULTS: The cohort included 40 patients with moyamoya disease and 28 with non-moyamoya disease, predominantly intracranial atheroscle

Mechanical Thrombectomy in Distal Vessels: Revascularization Rates, Complications, and Functional Outcome.

INTRODUCTION: Pivotal trials have shown the tremendous efficacy of mechanical thrombectomy in proximal occlusions. However distal occlusions involving second-order branches of the middle cerebral artery and beyond, anterior cerebral and posterior cerebral arteries were not represented. In this study, we investigated the feasibility and safety of distal circulation mechanical thrombectomy. METHODS: A retrospective review of patients presenting with distal circulation acute ischemic stroke who underwent mechanical thrombectomy 2010 and 2018. RESULTS: Of 453 patients who underwent mechanical th

Extracranial-intracranial bypass for internal carotid/middle cerebral atherosclerotic steno-occlusive diseases in conjunction with carotid endarterectomy for contralateral cervical carotid stenosis: clinical results and cognitive performance.

Clinical results as well as cognitive performances after extracranial to intracranial (EC-IC) bypass in conjunction with contralateral carotid endarterectomy (CEA) are poorly understood. Data from 14 patients who underwent unilateral EC-IC bypass for atherosclerotic internal carotid artery (ICA)/middle cerebral artery (MCA) steno-occlusive disease in conjunction with CEA for contralateral cervical carotid stenosis were retrospectively reviewed. Postoperative results were evaluated by MRI imagings. Nine patients also underwent neuropsychological examinations (NPEs), including assessment by the

Posterior auricular artery as an alternative donor vessel for extracranial-intracranial bypass surgery.

BACKGROUND: Sometimes the superficial temporal artery (STA) is not available for an extracranial-intracranial (EC-IC) bypass procedure. An alternative vessel for an EC-IC bypass is the posterior auricular artery (PAA) if it extends to the temporoparietal area with a diameter large enough. We assessed the prevalence of an appropriate PAA as an alternative donor vessel and report three illustrative cases in which the PAA was used for EC-IC bypass surgery. METHODS: A literature search was performed on the use of the PAA as a donor vessel for bypass surgery. Secondly, a prospective database of by

"Malignant" carotid artery dissection.

PURPOSE: Carotid artery dissection resulting in occlusion or severe narrowing and massive intracranial embolism can result in life-threatening hemispheric ischemia. Aggressive endovascular and microsurgical measures may be necessary to salvage life and minimize stroke morbidity in this extreme situation. PATIENTS AND METHODS: We have treated two middle-aged women who presented within an hour of spontaneous cervical internal carotid artery (ICA) dissection causing hemiplegia, forced head and eye deviation, and declining consciousness. The first patient had a carotid occlusion through which a c

Percutaneous transluminal angioplasty of supra-aortic arterial stenoses in patients with concomitant cerebrovascular and coronary artery diseases--report of two cases.

Percutaneous transluminal angioplasty (PTA) was performed on supra-aortic arterial stenoses in two patients with concomitant cerebrovascular disease (CVD) and coronary artery disease (CAD). One patient had suffered small cerebral infarctions in the left frontal and temporo-occipital regions, and the other a lacunar stroke in the left pontine region. The former had stenosis of the left internal carotid artery as well as severe triple-vessel CAD. The latter had stenosis of the left subclavian artery as well as severe single-vessel CAD of the left circumflex artery. The PTA procedure successfully

Delayed Cortical Hyperperfusion Induced by Positional Carotid Compression in Stylocarotid Syndrome: A Case Report.

Cortical hyperperfusion is commonly discussed in the context of cerebral hyperperfusion syndrome after carotid revascularization; however, hyperperfusion-related cortical injury may also occur after transient ischemia. A 76-year-old man developed a recurrent impaired consciousness and left hemiparesis 18 days after cervical laminoplasty. Although the initial imaging was normal, delayed cortical diffusion abnormalities with concordant hyperperfusion on arterial spin labeling emerged despite preserved arterial patency. Computed tomography revealed a markedly elongated right styloid process (78 m

Moyamoya Disease in an Eight-Year-Old Female Child-A Case Report.

An eight-year-old girl presented with a first episode of focal seizure and transient loss of consciousness. Brain MRI revealed a large subacute infarct in the left fronto-parieto-temporal region, and MR angiography showed bilateral supraclinoid internal carotid and proximal posterior cerebral artery narrowing with extensive basal collateral formation, consistent with Moyamoya disease. The absence of aneurysm or arteriovenous malformation excluded other vascular anomalies. She was managed with hypertonic saline, intravenous antibiotics, and antiplatelet therapy, followed by stabilization and re

Endovascular treatment in Danon disease: a case report.

BACKGROUND: Danon disease is a lysosomal storage disorder with X-linked inheritance. The classic triad is severe hypertrophic cardiomyopathy, myopathy, and intellectual disability, with different phenotypes between both genders. Ischemic stroke is an uncommon complication, mostly cardioembolic, related to intraventricular thrombus or atrial fibrillation, among others. CASE REPORT: We report the case of a 14-year-old Caucasian male patient with Danon disease who suffered from an acute ischemic stroke due to occlusion in the M1 segment of the middle cerebral artery. He underwent mechanical thro

Effect of inhalational anesthesia on cerebral circulation in Moyamoya disease.

To clarify the effects of inhalational anesthesia on cerebral circulation in patients with Moyamoya disease, the authors measured regional cortical blood flow (CoBF), jugular bulb oxygen saturation (SjO2), and frontal regional oxygen saturation (rSO2) by near infra-red spectroscopy under total intravenous and inhalational anesthesia in 13 patients undergoing revascularization procedures. Cortical blood flow decreased in some regions under inhaled anesthesia in all cases, and the mean value decreased significantly (p<0.01). Patients' SjO2 levels rose during inhalational anesthesia in 10 of 11 c

Maxillary artery utilization in subcranial-intracranial bypass procedures: a comprehensive systematic review and pooled analysis.

The utilization of the internal maxillary artery (IMAX) in subcranial-intracranial bypass for revascularization in complex aneurysms, tumors, or refractory ischemia shows promise. However, robust evidence concerning its outcomes is lacking. Hence, the authors embarked on a systematic review with pooled analysis to elucidate the efficacy of this approach. We systematically searched PubMed, Embase, and Web of Science databases following PRISMA guidelines. Included articles used the IMAX as a donor vessel for revascularizing an intracranial area and reported at least one of the following outcomes

Regression of moyamoya vessels and hemodynamic changes after successful revascularization in childhood moyamoya disease.

Successful revascularization improves ischemic symptoms in the pediatric patients with moyamoya disease. However, it is not clear whether the revascularization prevents future intracranial hemorrhage from the residual moyamoya vessels. The purpose of this study is to investigate perioperative morphological and hemodynamic changes in the moyamoya vessels. Four pediatric patients (age < 15 years old) with bilateral moyamoya disease were selected for this study. To quantify the number of moyamoya vessels, T1-weighted transverse images at the level of the basal ganglia and the thalamus were select

Mechanical Thrombectomy for Isolated M2 Occlusions: A Post Hoc Analysis of the STAR, SWIFT, and SWIFT PRIME Studies.

BACKGROUND AND PURPOSE: Mechanical thrombectomy is beneficial for patients with acute ischemic stroke and a proximal anterior occlusion, but it is unclear if these results can be extrapolated to patients with an M2 occlusion. The purpose of this study was to examine the technical aspects, safety, and outcomes of mechanical thrombectomy with a stent retriever in patients with an isolated M2 occlusion who were included in 3 large multicenter prospective studies. MATERIALS AND METHODS: We included patients from the Solitaire Flow Restoration Thrombectomy for Acute Revascularization (STAR), Solit

Potential for the use of the Solitaire stent for recanalization of middle cerebral artery occlusion without a susceptibility vessel sign.

BACKGROUND AND PURPOSE: Absence of the MCA susceptibility vessel sign (negative MCA susceptibility vessel sign) on gradient recalled-echo MR imaging in acute stroke is commonly associated with in situ stenosis and thrombotic occlusion. We evaluated the effectiveness and safety of the Solitaire stent as the first-line device for the recanalization of MCA occlusion with a negative MCA susceptibility vessel sign. MATERIALS AND METHODS: Thirty-eight consecutive patients presenting with acute ischemic stroke due to MCA occlusion were treated by using the Solitaire AB stent alone or combined with t

Seizing the opportune moment: early use of antiplatelet therapy for thrombolysis.

Intravenous thrombolysis remains a cornerstone intervention for restoring cerebral perfusion in patients with acute ischemic stroke (AIS), with recombinant tissue plasminogen activator (rt-PA) being the most widely employed agent globally. Post-thrombolytic complications, including hemorrhagic transformation, vascular reocclusion, cerebral edema, seizures, and allergic reactions, pose significant clinical challenges. Among these, hemorrhagic transformation is a primary concern during early revascularization, particularly in patients treated within the therapeutic time window. Despite its clini
