We report a surgical technique in which donor sclera is used to create a flap in glaucoma filtering procedures. This simple technique is helpful when the sclera is too thin to safely and effectively form an adequate scleral flap. The use of donor sclera allows the procedure to continue as a guarded procedure and prevents early postoperative hypotony.

Medium and long term results of trabeculotomy and goniotomy operations for primary congenital glaucomas treated in Lausanne between 1965 and 1990 were studied retrospectively. Parameters evaluated were intraocular pressure (IOP) and final visual acuity (VA). Data on IOP were available for 38 eyes. 66% underwent trabeculotomy and/or goniotomy (first group) the others needed further surgical or medical treatment (second group). We studied one randomly selected eye of 50 consecutive primary open-angle glaucoma patients treated with argon laser trabeculoplasty to assess response differences as functions of anterior chamber anatomy and refraction. We found no significant differences related to these variables, and thus conclude that argon laser trabeculoplasty may be performed successfully in patients with crowded angles as long as the trabecular meshwork can be visualized.
