We report a case of myelodysplastic syndrome with peculiar fibril formation in granulocytes shown through electron microscope and complex karyotypic abnormalities including ring chromosomes. The patient, a 76-year-old male, was consulted for mild pancytopenia in February 1987. After 5 month, his hematological findings showed severe pancytopenia getting worse rapidly and presence of blasts in the peripheral blood. He had slightly hypercellular marrow with marked trilineage dysplasia and increased number of blasts (12.6%). Chromosome analysis from the bone marrow cells revealed its various struc

A 23-year-old Chinese man presented with a 16-month history of white patches on his abdomen and neck. He had previously received an intralesional injection of psoralen along with narrowband psoralen ultraviolet B radiation (PUVB) therapy. Blue macules had appeared in and around the injection sites 1 week later. Dermoscopy revealed blue spots and reticular telangiectasia within the white patches. Histological examination revealed an absence of epidermal melanocytes and pigment in the basal layer, as well as deposition of melanophages between collagen bundles or surrounding blood vessels and app

A 66-year-old man with profound total deafness received a 22-channel cochlear implant. After surgery, his hearing improved and the postoperative course was uneventful except transient vertigo. The patient died of lung cancer after 2.5 years. Twelve hours after death, the temporal bone was harvested for histological and electron microscopic examination. The mastoid air cells were preserved, although a few mucosal folds were present. The Dacron mesh tie had largely been absorbed, but part remained embedded in fibrous tissue. No inflammation was noted in the tympanic cavity or the round window th

The presenting features and treatment responses of six children with junctional ectopic tachycardia are evaluated. Two of the patients were siblings and both presented in early childhood with cardiopulmonary failure. The elder sibling died, the surviving sibling was controlled on a combination of amiodarone, digoxin, and sotalol. The remaining four patients presented in later childhood with tachycardia induced cardiomyopathy. Two of the patients were diagnosed incidentally and have normalised their myocardial function on sotalol therapy. The other two presented in congestive cardiac failure. R

Ablation of the atrioventricular junction consists in creating a therapeutic AV block to facilitate the treatment of symptoms caused by atrial arrhythmias refractory to drug therapy. The technical performance of ablation has been improved by restricting the indication to atrial fibrillation, by using radiofrequency currents, by choosing a nodal rather than His bundle ablation site, and by improving the function of cardiac pacemakers (rate adapting, back-up). The functional results are excellent but the outcome is punctuated by rare cases of sudden death, the cause of which is not fully underst

Generalized normolipemic plane xanthomatosis is a rare cutaneous disorder, frequently associated with reticuloendothelial diseases and some disorders with inflammation. Relapsing polychondritis is also a rare disease that shows an association with various immune-mediated diseases. We report a case of generalized normolipemic plane xanthomatosis associated with relapsing polychondritis in a 56-year-old Japanese man. We have already reported the clinical picture of relapsing polychondritis as well as an increase in urinary glycosaminoglycans excretion in this patient. During subsequent treatment

A case of single ventricle with pulmonary stenosis operated at 20 years of age by direct atrio-pulmonary anastamosis is presented. This procedure in contrast to classical techniques does not use an intraventricular patch or a valved ventriculo-pulmonary conduit. It is an adaptation of the Fontan technique initially proposed for tricuspid atresia. The operation consisted of closing the right atrioventricular orifice with a piece of Dacron, suturing the pulmonary valves (which were stenosed) and connecting the right atrium and pulmonary artery by a direct anastamosis using the auricle. The clini

Background/Objective: Pacing-induced cardiomyopathy (PICM) is a common complication of right ventricular (RV) pacing, affecting 6-25% of patients with frequent RV pacing, due to electrical and mechanical dyssynchrony. Certain clinical and electrocardiographic risk factors have been identified, including high RV-pacing burden and longer paced QRS, but their ability to predict the development PICM remains limited. Additionally, other forms of PICM have been described, including heart failure with preserved ejection fraction and RV failure. The goal of this narrative review is to summarize the cu

BACKGROUND: Conduction system pacing (CSP) has emerged as a promising modality for cardiac resynchronization therapy, but evidence of CSP in heart failure (HF) remains controversial.
OBJECTIVES: The authors aimed to compare the efficacy and safety of CSP, including left bundle branch area pacing (LBBAP) and His bundle pacing, to biventricular pacing (BiVP) in HF.
METHODS: The MEDLINE, EMBASE, and CENTRAL databases were searched from inception until May 2026. The primary endpoint was a composite of hospitalization for heart failure (HHF) and all-cause mortality. Secondary endpoints included c

BACKGROUND: Few studies have compared conventional testing with prolonged monitoring using an implantable loop recorder (ILR) following the first syncope episode in patients with bundle branch block (BBB) and negative workup.
OBJECTIVES: To compare two syncope evaluation strategies-primary use of an ILR (Group 1) versus conventional testing (Group 2)-and to estimate the prevalence of significant arrhythmias in the ILR patient subset.
METHODS: From January 2005 to December 2010, 78 patients admitted after one syncope episode were randomized to ILR (n=41) or conventional follow-up (n=37). Mean

The usual frequent tachyarrhythmias well known to the specialists in internal medicine, such as ventricular tachycardia and ventricular fibrillation, are not discussed in this publication; nor are the bradycardias connected with the sick sinus syndrome or with atrioventricular block of higher degrees (with one exception). In the first section a stratification of the risk after myocardial infarction is presented including the therapeutic implications. Severely reduced left ventricular function is of most negative prognostic value. After the poor results of the CAST study, which revealed a three

We report a 54-year-old man with progressive proximal muscle atrophy and gynecomastia. The patient had an insidious onset of weakness in his lower extremities at age 14, in that he noted a difficulty in standing up from a chair. Soon after he noted some difficulty in climbing up stairs. At age 35, he noted weakness in his arms; his weakness slowly progressed in that he became unable to walk or stand alone before 40 years of age. He also noted gynecomastia at that age. He was admitted to our hospital for the work up on September 16, 1993, when he was 54-year-old. On admission, he was alert and

BACKGROUND: The mechanism of bundle branch reentrant tachycardia has been described, and ablation of the right bundle branch (RBB) is a curative approach.
OBJECTIVE: The purpose of this study was to evaluate the left bundle branch (LBB)-Purkinje system during sinus rhythm and to test the feasibility of ablating the LBB in patients with bundle branch reentrant tachycardia.
METHODS: Thirteen consecutive male patients (age 62 +/- 12 years) with sustained bundle branch reentrant tachycardia were included in the study.
RESULTS: Surface ECG before ablation showed left bundle branch block (LBBB) i

Accessory atrioventricular pathways are a frequent cause of paroxysmal supraventricular tachycardias. This study analyses our results with a recently developed therapeutic approach-radiofrequency (RF) catheter ablation. This was applied in 150 consecutive patients (97 men, 53 women, mean age 42 +/- 15 years) with a total of 159 accessory pathways in all locations. All but 4 patients were symptomatic, with a spectrum ranging from palpitations (146 patients), syncope (39 patients) to aborted sudden death (2 patients). Prior to ablation, 115 patients had received long-term treatment with up to 4

This study describes the electrophysiologic effects of the Mustard repair of d-transposition of the great arteries on the sinus node and on conduction and refractoriness in the atrium, atrioventricular (AV) node and ventricle. Seventy-two electrophysiologic studies were performed on 64 patients after the Mustard operation. Standard rest and 24 hour ambulatory electrocardiograms were evaluated. Catheter endocardial atrial mapping, available in 67 of the 72 studies, revealed that sinus rhythm was present in the atria in 33 patients, ectopic atrial rhythm in 26 and junctional rhythm in 8. The endocardium was analyzed in all four chambers of 99 hearts with various types of congenital heart defects in which surgical repair was performed more than 6 weeks before death. The findings were compared with those of normal hearts in similar age groups. In some cases the endocardium was microscopically examined. This study revealed that in many cases all four chambers had fibroelastosis of the endocardium (diffuse regardless of the type of surgery done previously). These data suggest that diffuse fibroelastosis can occur as a result of surgical intervention and may be related to blocked l

BACKGROUND: Bundle branch reentrant ventricular tachycardia (BBR-VT) is a rare form of VT occurring in patients with structural heart disease (SHD). Rarely, it can also occur in the absence of SHD. Understanding its clinical and electrophysiologic (EP) properties and outcomes post-catheter ablation (CA) is crucial.
OBJECTIVE: We present a series of patients with bundle branch reentrant ventricular tachycardia (BBR-VT) and structurally normal hearts, comparing their clinical and electrophysiological characteristics, as well as long-term outcomes, with those of patients with the classic form of

BACKGROUND: Conventional right ventricular pacing (RVP) causes cardiac dyssynchrony, and increases risk of pacing-induced cardiomyopathy (PICM), heart failure hospitalization and mortality. Left bundle branch area pacing (LBBAP) is a promising physiological pacing modality, we compared the effect of LBBAP on cardiac function with RVP in patients with atrioventricular block (AVB).
METHODS: A total of 118 patients with AVB who successfully underwent LBBAP were enrolled between June 2019 and June 2022. Among them, 110 patients with a baseline LVEF ≥ 50% were propensity-matched 1:1 with 49 patien
