Non-odontogenic toothache.

Toothache is a common complaint in the dental office. Most toothaches have their origin in the pulpal tissues of periodontal structures. These odontogenic pains are managed well and predictably by dental therapies. Non-odontogenic toothaches are often difficult to identify and can challenge the diagnostic ability of the clinician. The most important step toward proper management of toothache is to be suspicious that the pain may not be of dental origin. The cardinal warning symptoms of non-odontogenic toothache are as follow: A. Spontaneous multiple toothaches. B. Inadequate local dental cause

General practice consultation patterns preceding diagnosis of eating disorders.

OBJECTIVE: To see whether patients with eating disorders consult general practitioners more frequently than control subjects and, if so, to describe the pattern of consultation. METHOD: General practitioner case record review of patients with anorexia nervosa, bulimia nervosa, and partial syndromes referred to a specialist eating disorder service. RESULTS: Seventy-eight of 100 case records were available for analysis. Eating disorder patients consulted significantly more frequently than controls over 5 years prior to the diagnosis of the eating disorder. They presented to the general practit

Efficacy of mindfulness- and acceptance-based cognitive-behavioral therapies for bodily distress in adults: a meta-analysis.

OBJECTIVE: Bodily distress, i.e., somatoform disorders and associated functional somatic syndromes, is highly prevalent, often persistent and highly disabling. It has been proposed that "third wave" therapies may be beneficial variants of cognitive behavioral treatments. However, evidence on their efficacy is scarce. This meta-analysis examines the efficacy of "third wave" psychotherapies (mindfulness-based cognitive therapy [MBCT], mindfulness-based stress reduction [MBSR], and acceptance and commitment therapy [ACT]) in adults with bodily distress. METHOD: We included randomized controlled …

[Post-traumatic embitterment disorder (PTED). Differentiation of a specific form of adjustment disorders].

Adaptation disorders are a heterogeneous group of mental disorders. Although they play a major role in clinical practice, are difficult to treat, and often lead to chronicity and disability, diagnostic algorithms are vague and scientific research is rare. Clinical practice has shown a subtype of adaptation disorder that is characterized primarily by lasting embitterment after exceptional though normal life events which violate basic beliefs. This disorder can be called post-traumatic embitterment disorder (PTED). A case vignette and results of a pilot study are reported. Critical life events w

Mediating effects of anxiety and depression on the relationship between somatic symptom disorder and quality of life among adults with epilepsy.

OBJECTIVE: Although quality of life (QOL) is impaired in patients with somatic symptom disorder (SSD), little is known about the relationship between SSD and QOL among patients with epilepsy (PWE) and the mechanisms underlying this association. This study aimed to identify the mediators of the association between SSD and QOL among PWE. METHODS: A cross-sectional study was conducted at West China Hospital between July 2020 and May 2022. A total of 749 adults with epilepsy who attended the epilepsy center were consecutively enrolled via convenience sampling. All patients completed questionnaire

Symptom profiles in long COVID compared to functional somatic disorder and the general population.

INTRODUCTION: Long COVID, characterised by persistent symptoms following COVID-19, affects about 10% of individuals recovering from SARS-CoV-2. The overlap of symptoms described in long COVID and functional somatic disorder (FSD) raises questions about shared pathophysiology. This report compares the prevalence and profiles of physical symptoms among patients with long COVID, patients with FSD and the general population. METHODS: Data from a cohort of patients with long COVID referred for diagnostics, a cohort of patients seen in a regional clinic for FSD and individuals from the general popu

Betrayal trauma and somatic symptoms among patients in a medically underserved primary care clinic.

OBJECTIVE: Betrayal Trauma Theory posits that victims of trauma are more prone to developing psychological and physical problems if the traumatic event includes the element of betrayal. We sought to evaluate the impact of betrayal trauma versus nonbetrayal trauma and no trauma exposure on the risk of patients' reporting somatic symptoms in six domains (gastrointestinal, cardiopulmonary, musculoskeletal, pseudoneurological, gynecological, or any symptom). METHOD: Medically underserved patients (N = 1,350) who presented to a primary care clinic in California completed a structured standardized

The influence of psychological factors on tinnitus severity.

OBJECTIVE: Subjective tinnitus is a frequent symptom characterized by perception of sound in the absence of a corresponding external stimulus. Although many people learn to live with tinnitus, some find it severely debilitating. Why tinnitus is debilitating in some patients, but not in others, is still incompletely understood. We aimed to assess the influence of different aspects of psychological distress on perceived tinnitus severity. METHODS: Three hundred seventeen patients diagnosed with chronic subjective tinnitus at two university clinics completed the Tinnitus Handicap Inventory (THI)

The relation between anger coping strategies, anger mood and somatic complaints in children and adolescents.

Attempts to explain the experience of somatic complaints among children and adolescents suggest that they may in part result from the influence of particular strategies for coping with anger on the longevity of negative emotions. To explore these relationships British (n = 393) and Dutch (n = 299) children completed a modified version of the Behavioral Anger Response Questionnaire (BARQ), and two additional questionnaires assessing anger mood and somatic complaints. A hierarchical regression analysis showed that for both the UK and Dutch samples two coping styles, Social support-seeking and Ru

Disease variables and depression affecting the process of coping in families with a somatically ill parent.

AIM AND METHODS: Based on the investigation of 134 families with a seriously ill parent and 214 offspring, we compared--by means of semistructured psychiatric interviews--the capacity of the ill and the healthy parent and their children to cope with various diseases. We evaluated how disease characteristics such as duration and severity of the disease (assessed by the Karnofsky Index) and associated depression [measured by the Beck Depression Inventory for parents and the Depression Inventory for Children and Adolescents (Depressions-Inventar für Kinder und Jugendliche) for children] influence

Evaluation of noncardiac chest pain: diagnostic approach, coping strategies and quality of life.

BACKGROUND: Approximately 30% of coronary angiograms are negative for significant coronary artery disease and patients are classified as having noncardiac chest pain (NCCP). So far, no systematic diagnostic approach to patients with NCCP investigating for possible esophageal, psychiatric and musculoskeletal abnormalities exists. Furthermore, coping strategies and quality of life are poorly characterized in NCCP patients. METHODS AND RESULTS: A simple diagnostic approach was applied to 37 consecutive patients (21 female, age 61+/-12 years) with angina-like chest pain and normal coronary angiog

[Functional gastrointestinal disorders].

The functional gastrointestinal disorders (FGID) are the most frequent clinical conditions seen in practice. The FGID are associated with significant work absenteeism, impaired quality of life and increased medical costs. Most patients also suffer from psychosocial problems. Therefore it is important to define the patient's complaints in terms of a biopsychosocial disorder rather than just a medical illness. Physicians must acknowledge the relevance of the psychosocial aspects to prepare the patient for a referral to a specialist (in psychosomatic medicine or a psychotherapist) and to get the
