OBJECTIVE: Degenerative hepatic artery aneurysms (HAAs) are an uncommon pathologic process. The aim of this study was to evaluate the approach to and outcomes of operative management of HAA with open techniques (OTs) and endovascular techniques (ETs).

The trend to preserve the spleen whenever possible after its blunt trauma forced the necessity of the workout of the differentiative approach to the problem. The most disputable situation is the formation of the laminar subcapsular hematoma of the spleen. The conservative treatment of such lesions can lead to the two-stage rupture with severe intraabdominal bleeding. Nevertheless, the results of the morphological analysis demonstrate the early beginning of reparative process around the hematoma, securing the possibility of the septic inflammation.

This report is a retrospective study over a 13 year period of 267 patients who underwent splenectomy. The incidence of associated major abdominal injuries following blunt trauma in these patients was 8%. Over half of these associated injuries were diagnosed pre-operatively.
