Nontraditional 'aids' to hearing: assistive listening devices.

Unresolved hearing loss and its associated sense of isolation burden the hearing-impaired elderly. The plight of both listeners and talkers justifies every effort to encourage and enhance communication. Assistive listening devices can benefit many family practice patients. The following service welcomes questions or comments about implementing these devices: Audiology-Speech Pathology Service, VA Medical Center, 700 South 19th Street, Birmingham, AL 35203; (205)933-8101, ext. 6701 and 6702. In addition, local professional audiologists certified by the American Speech-Language-Hearing Associati…

[Curriculum development in speech-language therapy and audiology: basis and principles].

The aim of this article is to discuss some of the important issues that form the basis of curriculum development for education in speech-language therapy and audiology. The demands of the profession are such that a horizontal occupational structure is no longer adequate in the multicultural, multilingual RSA context. It is therefore necessary to investigate alternative educational options to accommodate current needs. These may include diploma training for technicians, certificate programmes for community rehabilitation workers and professionally directed masters courses. It is also apparent t

The physiology of the normal and pathological cochlear neurones--some recent advances.

This is the first of two auditory science tutorials designed for otolaryngologists, audiologists, and others dealing with deafness and its consequences. This tutorial is intended as an update on recent advances in the understanding of cochlear function both in the normal person and in patients with hearing loss of cochlear origin. It draws largely on studies involving electrophysiological recordings from cochlear nerve fibres in "animal models" of cochlear hearing loss. These data allow considerable insight into the underlying causes, for example, loudness recruitment, tinnitus, and poor speec…

A critical evaluation of the glycerol test in Ménière's disease.

The glycerol test is currently widely used to evaluate Menière's disease, particularly in regard to its suitability for decompressive surgical therapy. Our previous findings have shown that patients with Menière's syndrome respond extremely well to placebo treatment. We have investigated the effect of psychological factors in the outcome of the glycerol test. It was administered twice to each of a group of patients with a typical Menière's syndrome. On one occasion the patient was told the hearing would improve, and on the other that the hearing would worsen. The same dose of glycerol was prov

Future directions for screening and treatment in congenital hearing loss.

Hearing loss is the most common neurosensory deficit. It results from a variety of heritable and acquired causes and is linked to multiple deleterious effects on a child's development that can be ameliorated by prompt identification and individualized therapies. Diagnosing hearing loss in newborns is challenging, especially in mild or progressive cases, and its management requires a multidisciplinary team of healthcare providers comprising audiologists, pediatricians, otolaryngologists, and genetic counselors. While physiologic newborn hearing screening has resulted in earlier diagnosis of hea…

Clinical consensus document for fitting non-surgical transcutaneous bone conduction hearing devices to children.

This clinical consensus document addresses the assessment, selection, and fitting considerations for non-surgical bone conduction hearing devices (BCHD) for children under the age of 5 years identified as having unilateral or bilateral, permanent conductive or mixed hearing losses. Children with profound unilateral sensorineural hearing losses are not addressed. The document was developed based on evidence review and consensus by The Paediatric Bone Conduction Working Group, which is composed of audiologists from North America who have experience working with BCHDs in children. The document ai…

Electrocochleography: methods and clinical applications.

The use of electrocochleography (ECoG) as a tool in the diagnosis, assessment, and monitoring of certain neuro-otologic disorders has attracted renewed attention in the United States. The use of ECoG among otologists and audiologists has increased in recent years, primarily due to the availability of relatively easily applied, noninvasive extratympanic electrodes. Information derived from ECoG recordings has been shown to be helpful in the objective identification and monitoring of Meniere's disease and endolymphatic hydrops. In addition, ECoG may be used to enhance our ability to observe the …

An amplitude-bandwidth expansion method for hearing-aid adjustment.

The basic concept for a technique that facilitates the adjustment of a hearing aid by a person of normal hearing is proposed. The technique involves processing using an amplitude-bandwidth expansion method to expand the hearing-aid output to fit to the user's hearing requirements and loudness recruitment. The expansion is a reversal of the cochlear compression model. As the expansion method introduces distortion of the waveform and a reduction of the expansion rate, the support technique presented here includes solutions to these problems. The difference between the original speech signal and

Audiological and medical considerations for children with cochlear implants.

Recent advancements in technology have resulted in the development of implantable devices--cochlear implants--designed to maximize the sensation of hearing in the hearing-impaired population. For children who meet the numerous preselection criteria, the cochlear implant can be effective and beneficial. The implants must be inserted surgically, which involves placing an electrode array in the cochlea and attaching the receiver/stimulator to the skull. The child can usually be discharged from the hospital by the third postoperative day. The external device is fitted and initially stimulated abou

Predicting hearing sensitivity and audiometric slope for mentally retarded persons.

The sensitivity prediction from the acoustic reflex (SPAR) test was administered to 90 subjects who were classified according to four levels of mental retardation and five levels of hearing sensitivity. SPAR results showed accurate hearing level prediction for 58% of the subjects, one-category errors for 29%, and two- or three-category errors for 13%. Audiometric slope predictions were also made and were accurate 58% of the subjects. SPAR results indicated that the greater the hearing loss, the less likely that normal hearing would be predicted. Hearing level predictions for the retarded popul

Tablet and web-based audiometry to screen for hearing loss in adults with cystic fibrosis.

INTRODUCTION: Individuals with chronic lung disease (eg, cystic fibrosis (CF)) often receive antimicrobial therapy including aminoglycosides resulting in ototoxicity. Extended high-frequency audiometry has increased sensitivity for ototoxicity detection, but diagnostic audiometry in a sound-booth is costly, time-consuming and requires a trained audiologist. This cross-sectional study analysed tablet-based audiometry (Shoebox MD) performed by non-audiologists in an outpatient setting, alongside home web-based audiometry (3D Tune-In) to screen for hearing loss in adults with CF. METHODS: Hearin…

Perilymphatic fistula, usually in the round or oval window, causes a variety of symptoms, including sensorineural hearing loss. Surgical rep … nted a severe sensorineural hearing loss with no useful discrimination for speech. The authors usge audiologists and otolaryngologists to consider the possibility of fistula in cases of sudden hearing loss, even when years have elapsed since the trauma. Their experience suggests that surgery may be successful in restoring hearing after many years.

[Language development disorders--studies of preschool children of the Leipzig special language school].

Children with severe speech disorders were examined to determine the causes of their abnormal speech development. Thirty-seven preschool and school-age children were followed up and results compared. All children had low intelligence quotients, with most of the children coming from low social backgrounds. However, both of these factors (intelligence and low social background) had no influence on the improvement of speech levels after therapy. Two-thirds of the children with moderate conductive hearing losses showed improvement after therapy, so that the effect of this hearing loss was believed

Down syndrome: a multidisciplinary perspective.

Trisomy 21, a genetic disorder resulting from a chromosomal abnormality, is one of the most common forms of mental disability in the United States. Individuals with Down syndrome frequently present with a constellation of medical problems including conductive hearing loss and, to a lesser degree, sensorineural hearing loss. As part of a health care team, audiologists must be sensitive to and aware of medical conditions prior to establishing intervention strategies. Medical conditions, by necessity, precede audiologic interventions and, therefore, a close working relationship among team members…
