A selective, non-ischemic, non-pharmacological left ventricular failure animal model.

Selective left ventricular failure was induced in 13 acute anesthetized, closed chest dogs ranging in weight from 18-26 kg. Failure was induced by passing a single, high intensity pulse of current from a defibrillator connected to a left ventricular catheter electrode and a left chest electrode. The intensity of the myocardial damaging shock was related to the predicted current required for transventricular defibrillation, based on heart weight. Thermodilution cardiac output, left ventricular pressure, impedance stroke volume, the cardiac electrogram, and lead II ECG were recorded, along with

[Double-wire technique for implanting a left ventricular venous lead in patients with complicated coronary venous anatomy].

INTRODUCTION AND OBJECTIVES: Occasionally, implanting a left ventricular pacing electrode for cardiac resynchronization therapy via the coronary sinus may be complicated by the presence of anatomical structures that obstruct the access to the target vein. Our objective was to report on experience using a double-wire technique for implanting left ventricular venous leads gained at the Dr Negrín Hospital in Gran Canaria, Spain. METHODS: In 20 (12%) of 170 consecutive patients (67 [9] years, 72% male) undergoing implantation of a cardiac resynchronization device, a second parallel hydrophilic gu

Transesophageal echocardiography in hypotensive patients after cardiac operations. Comparison with hemodynamic parameters.

Because it is sometimes difficult to determine the cause of hypotension in patients after cardiac operations, we assessed the value of transesophageal echocardiography in this respect, and we studied 60 consecutive patients who had hypotension despite positive inotropic medication and, in some patients, mechanical support. Echocardiographic diagnoses were compared with diagnoses based on hemodynamic parameters. Follow-up examinations were completed in all patients to confirm the final diagnoses. Echocardiographic signs of hypovolemia were present in 14 patients, tamponade in six, left ventricu

[Efficacy of transesophageal echocardiography immediately after open heart surgery: diagnosis of coagula tamponade].

Massive pericardial coagula were detected by transesophageal echocardiography in some patients with low cardiac output failure immediately after open heart surgery. Characteristically, the coagula were localized anterior to the right atrium and right ventricle in all patients. The coagula could not be detected by transthoracic echocardiography. Despite of low output, absence of wall motion abnormality of the left ventricle, tachycardia, elevated atrial pressure and small left ventricular cavity lead to diagnose cardiac tamponade by pericardial coagula. Neither the right atrial nor the right ve

Pathophysiology of vasopressin in edematous disorders.

Sodium and water retention is characteristic of edematous disorders including cardiac failure, cirrhosis, nephrotic syndrome and pregnancy. In recent years the use of a sensitive radioimmunoassay for plasma vasopressin has implicated the role of nonosmotic vasopressin release in the water retention of these edematous disorders. In experimental studies and studies in humans it has been found that the nonosmotic release of vasopressin is consistently associated with activation of the sympathetic nervous and renin-angiotensin-aldosterone systems. Moreover, the sympathetic nervous system has been

A Path FORWARD: Development of a Comprehensive Multidisciplinary Clinic to Create Health and Wellness for the Child and Adolescent with a Fontan Circulation.

Today, it is anticipated most individuals diagnosed with single-ventricle malformation will survive surgical reconstruction through a successful Fontan operation. As greater numbers of patients survive, so has the recognition that individuals with Fontan circulation face a variety of challenges. The goal of a normal quality and duration of life will not be reached by all. The hurdles fall into a variety of domains. From a cardiovascular perspective, the Fontan circulation is fundamentally flawed by its inherent nature of creating a state of chronically elevated venous pressure and congestion,

Myocardial dysfunction in meningococcal septic shock.

PURPOSE OF REVIEW: The underlying pathophysiology of sepsis has long been disputed. Systemic vasodilatation is important in the development of shock and, in septic critically ill adults who have been volume resuscitated, the systemic pressure is often low and the cardiac output high. In septic children however, and especially in those with meningococcal septic shock, poor cardiac output as a consequence of depressed myocardial function seems to be important, often being the cause of death in these patients. There is much evidence for disturbance of myocardial performance, yet despite the liter

Propofol infusion syndrome in critically ill patients.

OBJECTIVE: To describe the clinical presentation of propofol infusion syndrome in critically ill adults. DATA SOURCES: Clinical literature was accessed through MEDLINE (1966 - March 2001). Key search terms included Diprivan, propofol, and propofol infusion syndrome. Case reports and small case series evaluating the use and toxicity of propofol in sedating critically ill adults were reviewed. DATA SYNTHESIS: The association between propofol infusion syndrome and death in children secondary to myocardial failure is well documented. However, few data are available regarding the syndrome in crit

Obstruction of ECT seizure by submaximal hyperventilation: a case report.

Extreme obstruction of ECT seizure induction occurred in an elderly female with a low cardiac ejection fraction after a 20 to 25 s interval between the last ventilation and the electrical stimulus. Stimuli of 378 and 504 mC induced no motoric or EEG activity after an initial stimulus of 227 mC produced just 1 s of motoric seizure activity. After prolonged hyperventilation and immediate stimulation a seizure resulted with 33 s of motoric activity. This case suggests that patients with a low cardiac output might be particularly sensitive to the quality of ventilation.

A model of human microvascular exchange.

A compartmental model consisting of the circulation, a general interstitium, and the lymphatics, is formulated to describe the transport and distribution of fluid and plasma proteins (albumin) in the human microvascular exchange system. Transcapillary mass exchange is assumed to occur via a coupled Starling mechanism. Unknown or poorly quantified model parameters are estimated by statistical fitting of simulation predictions to five different sets of experimental data. The data consist of steady-state and transient plasma and interstitial volumes and colloid osmotic pressures measured under la

Efficacy of isoproterenol on the failing transplanted heart during early acute rejection.

Acute rejection often leads to severe myocardial failure and death. The beneficial hemodynamic effects of isoproterenol in improving immediate postoperative heart failure have prompted its routine use after transplantation. However, because of the physiopathological alterations documented during rejection, an inappropriate response of the graft to isoproterenol administration could be expected. Six dogs received orthotopic transplants and were prepared with implantable devices for serial hemodynamic studies. The studies were performed on the resting unanesthetized subject 3 hours after operati

Circulatory support in infants with post-cardiopulmonary bypass left ventricular dysfunction using a left ventricular assist device.

Extracorporeal membrane oxygenation has been advocated as the most appropriate mode of circulatory support in the paediatric age group for post-cardiopulmonary bypass ventricular dysfunction. The results in infants who have predominantly left ventricular failure, or who require such support in order to be weaned off bypass, have been disappointing. Three infants with severe left ventricular dysfunction following cardiopulmonary bypass for correction of congenital heart defects have been managed with a left ventricular assist device. Two required this form of circulatory support in order to be

Cardiovascular collapse following an overdose of prostaglandin F2 alpha: a case report.

A case report is presented of a parturient who suffered severe hypotension and pulmonary oedema following an overdose of intramyometrial prostaglandin F2 alpha. Oxytocin induction of labour in this patient led to a rapid delivery, followed by a hypotonic uterus and postpartum haemorrhage. After resuscitation with blood and crystalloid fluids, the uterus was explored under general anaesthesia. The uterus was free of retained products but the lower uterine segment failed to contract despite bimanual uterine compression and intravenous oxytocin. Prostaglandin F2 alpha was injected into the lower

[Hemorrhagic pulmonary edema and cardiac failure following isolated head injury. Treatment with dobutamine and nitroglycerin (author's transl)].

A 12 year boy with an isolated head injury and subdural hematoma developed neurogenic pulmonary edema and intraoperatively a low cardiac output syndrome. The postoperatively depressed cardiac function and hemorrhagic pulmonary edema were treated with Dobutamine and Nitroglycerin given intravenously. The beta-1-stimulating catecholamine was used because of its positive inotropic effect, to reduce pulmonary capillary pressure and to improve the afterload. Nitroglycerin applied in a low dosage with exclusive venodilator effect was added for treatment of pulmonary edema and left ventricular failur

Altered microvascular responses of the small intestine to sepsis during renovascular hypertension.

Renovascular hypertension alters endothelial-dependent mechanisms to affect the response of small arterioles in skeletal muscle to sepsis. Small arteriole responses to sepsis differ between skeletal muscle and small intestine in normotensives. Our study now shows that renovascular (1K1C) hypertension alters small arteriole responses in the small intestine to Escherichia coli sepsis. Large arterioles (A1, A2) constricted by 10-20% in the small intestine of both normotensive and hypertensive rats during both high and low cardiac output sepsis. Small arterioles (premucosal A3 and preserosal A4) c
