The impact of Shanghai's community nursing center policy on chronic wound care practice: a qualitative study.

OBJECT: This qualitative study explores how the community nursing center policy reshapes chronic wound care. It captures frontline nurses’ experiences to identify strengths and barriers, providing evidence for optimizing and scaling community nursing center models. BACKGROUND: Global aging and lifestyle changes have intensified the demand for chronic wound care, a service predominantly delivered in primary care settings. To strengthen community capacity, Shanghai has been establishing community nursing centers since 2023. While this policy represents a novel care model, its impact on nurses an

Design of a Dyadic Digital Health Module for Chronic Disease Shared Care: Development Study.

BACKGROUND: The COVID-19 pandemic forced the spread of digital health tools to address limited clinical resources for chronic health management. It also illuminated a population of older patients requiring an informal caregiver (IC) to access this care due to accessibility, technological literacy, or English proficiency concerns. For patients with heart failure (HF), this rapid transition exacerbated the demand on ICs and pushed Canadians toward a dyadic care model where patients and ICs comanage care. Our previous work identified an opportunity to improve this dyadic HF experience through a s

A new prediction model for acute kidney injury in patients with sepsis.

BACKGROUND: Acute kidney injury is common in patients with sepsis and contributes to poor prognosis and mortality. Early identification of high-risk patients can provide evidence for clinical decision-making. METHODS: We developed a prediction model based on a cohort of 15,726 patients with sepsis from the Medical Information Mart for Intensive Care III critical care database. Logistic regression analysis was applied to develop a prediction model incorporating the selected risk factors. Discrimination and calibration of the prediction model were assessed using the C-index and calibration plot

The status and model of children primary nephrotic syndrome in continuing nursing.

BACKGROUND: Nephrotic syndrome (NS) is a common glomerular disease in children. Nursing during hospitalization alone cannot solve the psychological, physiological and social health problems of children. Continuing care models may provide patients with more continuous and efficient care services. Therefore, the present study aimed to provide theoretical support for the implementation and development of children's primary nephrotic syndrome (PNS) and children's chronic disease continuing nursing through the construction of a children's PNS continuing nursing model. METHODS: Each item of the tra

How Will a Treatable Traits Approach Reshape Clinical Practice in Chronic Cough?

Chronic cough, which affects 5%-18% of adults, has traditionally been considered a symptom defined by its duration. However, recent advances in understanding the pathophysiology of chronic cough have prompted the concept that chronic cough may exist as a distinct clinical entity driven by neuronal dysregulation and cough hypersensitivity. This evolving perspective recognizes various treatable traits and clinical characteristics which support the notion of cough as a disease entity and beyond that of a symptom-based classification. Initially developed for asthma and chronic obstructive pulmonar

The Parkinson's Universe: A person-centered care model for Parkinson's disease.

Parkinson's disease is the fastest growing neurodegenerative disorder worldwide, yet care delivery remains fragmented and inequitable. We propose a new construct called the Parkinson's Universe. It is a person-centered model that reimagines care as a coordinated universe. This commentary summarizes the model's elements including the patient as the sun or center of the universe, the caregiver as Mercury, the social support and multidisciplinary healthcare professionals as the other planets, mission control as care coordination, stigma as Pluto, barriers as asteroids, supportive technology as sa

Models of care for sickle cell disease in low-income and lower-middle-income countries: a scoping review.

Sickle cell disease has a growing global burden falling primarily on low-income countries (LICs) and lower-middle-income countries (LMICs) where comprehensive care is often insufficient, particularly in rural areas. Integrated care models might be beneficial for improving access to care in areas with human resource and infrastructure constraints. As part of the Centre for Integration Science's ongoing efforts to define, systematise, and implement integrated care delivery models for non-communicable diseases (NCDs), this Review explores models of care for sickle cell disease in LICs and LMICs.

Geographic Access to Primary Care and Racial and Ethnic Disparities in the Prevalence of Maternal Chronic Conditions 2020-2024.

INTRODUCTION: Unequal access to preventive health care before and between pregnancies contributes to risk for adverse perinatal outcomes. Few studies have examined access to primary care as a contributor to racial and ethnic disparities in chronic disease prevalence among women having a live birth. METHODS: We linked 2020-2024 natality data with Health Resources and Services Administration's Area Health Resource Files and used spatial Bayesian conditional autoregressive models to estimate county-level associations between geographic access to primary care and prevalence of chronic conditions

Clinical outcomes of HIV care delivery models in the US: a systematic review.

With over 1 million people living with HIV, the US faces national challenges in HIV care delivery due to an inadequate HIV specialist workforce and the increasing role of non-communicable chronic diseases in driving morbidity and mortality in HIV-infected patients. Alternative HIV care delivery models, which include substantial roles for advanced practitioners and/or coordination between specialty and primary care settings in managing HIV-infected patients, may address these needs. We aimed to systematically review the evidence on patient-level HIV-specific and primary care health outcomes for

Assessment of transition readiness in adolescents and young adults with chronic health conditions.

BACKGROUND: Transition from pediatric to adult health care is a vulnerable period for adolescents and young adults. Challenges include paucity of validated measures to assess patients' transition readiness. We evaluated the Transition Readiness Assessment Questionnaire (TRAQ) in adolescents and young adults with rheumatic, gastrointestinal, and endocrine disorders. We examined whether baseline TRAQ scores and other demographic variables predicted transition to adult care over a three year follow up period. METHODS: In this descriptive study at a single institution, eighty-nine adolescents at

Understanding the behavioral determinants of retention in HIV care: a qualitative evaluation of a situated information, motivation, behavioral skills model of care initiation and maintenance.

The current study provides a qualitative test of a recently proposed application of an Information, Motivation, Behavioral Skills (IMB) model of health behavior situated to the social-environmental, structural, cognitive-affective, and behavioral demands of retention in HIV care. Mixed-methods qualitative analysis was used to identify the content and context of critical theory-based determinants of retention in HIV care, and to evaluate the relative fit of the model to the qualitative data collected via in-depth semi-structured interviews with a sample of inner-city patients accessing traditio
