Water disinfection is critical to minimizing microbial risk, but unintentionally produces disinfection byproducts (DBPs). Exposure to the currently regulated chloro- and bromo-DBPs in water cannot fully account for the health effects observed from epidemiological studies. Iodinated disinfection byproducts (I-DBPs) are among the most toxic DBPs, but they have not been fully characterized because of significant technical challenges. We report here the first exposome-scale study of iodinated disinfection byproducts formed under environmentally relevant conditions. Here, "exposome-scale" refers to

AIMS: Despite numerous risk factors being associated with hypertension, the breadth of research remains constrained, with a notable absence of systematic, data-driven exploration into established and novel factors across a broad spectrum of exposures. This study aims to construct an atlas on known and emerging factors for hypertension through comprehensive epidemiological and genetic analyses.
METHODS AND RESULTS: We conducted exposome-wide association studies (ExWAS) via Cox regression models on two equally sized datasets for discovery and replication in UK Biobank, a large prospective cohor

BACKGROUND: Environmental risk factors contribute to functional impairment, which can persist even after symptomatic remission of a first episode of psychosis (FEP). To quantify this effect, we calculated the association between exposome score for schizophrenia (ES-SCZ), a cumulative environmental liability score, and functioning in remitted FEP patients.
METHODS: Data of 286 participants were derived from the HAMLETT study. General functioning was assessed using the Global Assessment of Functioning (GAF) scale at baseline, when participants were in remission for 3-6 months. ES-SCZ was calcul

Metabolic associated fatty liver disease (MAFLD) is a common liver disease with a prevalence of up to 25%; it not only adversely affects human health but also aggravates the economic burden of society. An increasing number of studies have suggested that the occurrence of chronic noncommunicable diseases is affected by both environmental exposures and genetic factors. Research has also shown that environmental pollution may increase the risk of MAFLD and promote its occurrence and development. However, the relationship between these concepts, as well as the underlying exposure effects and mecha

Febrile seizures are convulsions predominately occurring in young children. The effects of various exposomes, including influenza infection and external environmental factors, on febrile seizures have not been well-studied. In this study, we elucidated the relationships between ambient temperature, air pollutants, influenza infection, and febrile seizures using 22-year territory-wide hospitalization data in Hong Kong. The aggregated data were matched with the meteorological records and air pollutant concentrations. All-type and type-specific influenza-like illness positive (ILI+) rates were us

The identity and role of environmental factors in the etiology of sporadic amyotrophic lateral sclerosis (sALS) is poorly understood outside of three former high-incidence foci of Western Pacific ALS and a hotspot of sALS in the French Alps. In both instances, there is a strong association with exposure to DNA-damaging (genotoxic) chemicals years or decades prior to clinical onset of motor neuron disease. In light of this recent understanding, we discuss published geographic clusters of ALS, conjugal cases, single-affected twins, and young-onset cases in relation to their demographic, geograph

Exposures constitute a dense network of the environment: exposome. Here, we argue for embracing the exposome paradigm to investigate the sum of nongenetic "risk" and show how predictive modeling approaches can be used to construct an exposome score (ES; an aggregated score of exposures) for schizophrenia. The training dataset consisted of patients with schizophrenia and controls, whereas the independent validation dataset consisted of patients, their unaffected siblings, and controls. Binary exposures were cannabis use, hearing impairment, winter birth, bullying, and emotional, physical, and s

Biological ageing begins before birth, with early-life exposures shaping late-life health. These exposures drive health inequities early, yet specific exposures and the composition of the ageing exposome remain largely undefined. This gap may persist as the field lacks agnostic investigations accounting for non-linearity, interactions and subtle signals. We aimed to identify exposures predictive of epigenetic ageing accumulated during childhood and adolescence and explore the composition of the "missing" exposome. In the FinnTwin12 cohort (847 participants measured at ages 12, 14, 17, and 22),

Pathogenic variants in the Leucine-rich repeat kinase 2 (LRRK2) gene are a primary monogenic cause of Parkinson's disease (PD). However, the likelihood of developing PD with inherited LRRK2 pathogenic variants differs (a phenomenon known as "reduced penetrance"), with factors including age and geographic region, highlighting a potential role for lifestyle and environmental factors in disease onset. To investigate this, household dust samples from four different groups of individuals were analyzed using metabolomics/exposomics and metagenomics approaches: PD+/LRRK2+ (PD patients with pathogenic

INTRODUCTION: There is a critical need to identify risk factors of Alzheimer's disease and related dementias (ADRD).
METHODS: We included 1227 participants (mean age [standard deviation]: 74.1 [7.9] years) from the Multi-Ethnic Study of Atherosclerosis with brain MRI scans. We investigated joint associations of 13 natural, social, and built-environment exposures with gray matter volume (GMV) (n = 1219), white matter fractional anisotropy (WM FA) (n = 1102), and white matter hyperintensity volume (n = 1210) using weighted quantile sum regression.
RESULTS: A one-decile increase in all harmful

The convergence of climate change, adverse environmental exposures, and rapid population aging necessitates an equity-based paradigm shift in safeguarding brain health across the life course. Older adults, especially those in marginalized and low-resource settings, face disproportionate exposure to air pollution, toxicants, and climate-related hazards. Emerging evidence links these biophysical, social, and cultural vulnerabilities to cognitive decline and a heightened risk of developing Alzheimer's disease and related dementias (ADRDs), underscoring the need to examine cumulative exposures acr

Polluted air is a major global health risk factor, yet the chemical composition and toxicity of airborne gases and particles remain underexplored due to their complexity and difficulties in sampling. We recently introduced how polydimethylsiloxane (PDMS) foam─or silicone foam─can be synthesized for passive air sampling, enabling simple and cost-effective nontarget chemical profiling of indoor air. Here, we demonstrate expanded applications, indoors and outdoors, with commercial PDMS-foam, including for: (i) wide-scope target analysis of >220 priority substances by quantitative liquid- and gas

According to WHO data, approximately 57 million people worldwide were affected by dementia in 2021, with prevalence projected to rise. Alzheimer's disease (AD), responsible for 60%-80% of dementia cases, continues to be a leading cause of mortality, with current treatments offering limited efficacy and disease-modifying therapies lacking widespread adoption or conclusive safety evidence, shifting the focus toward prevention and risk modification. Risk factors for AD include both non-modifiable elements, such as age, genetics, and gender, and modifiable factors, like environmental pollution, he

Multi-omic investigations into environmental effects on health and disease are aided by inclusion of microbial microbiomes with assessment of mirobes producing metabolites that differentially modulate host organ functions. The gut microbiome is key because many environmental toxicants enter the body orally and may disrupt gut microbes that help digest food, as well as the microbiome-gut-brain axis, which produces regulatory metabolites with systemic effects. Environmental stressors may differentially alter brain development and function, even among identical twins, in that over time, there may

BACKGROUND: Investigations of causal pathways for psychosis can be guided by the identification of environmental risk factors. A recently developed composite risk tool, the exposome score for schizophrenia (ES-SCZ), which controls for intercorrelations between risk factors, has shown fair to good performance. We tested the transdiagnostic psychosis classifier performance of the ES-SCZ with the Bipolar-Schizophrenia Network for Intermedial Phenotypes data and examined its relationship with clinical-level outcomes.
STUDY DESIGN: We computed the case-control classifier performance for the ES-SCZ

Because of the direct interaction of lungs with the environment, respiratory diseases are among the leading causes of environment-related deaths in the world. Chronic obstructive pulmonary disease (COPD) and cystic fibrosis (CF) are two highly debilitating diseases that are of particular interest in the context of environmental studies; they both are characterized by a similar progressive loss of lung function with small bronchi alterations, and a high phenotypic variability of unknown origin, which prevents a good therapeutic efficacy. In the last years, there has been an evolution in the app

The repeated failures of anti-amyloid therapies in Alzheimer's disease (AD) have exposed critical limitations of the linear amyloid cascade hypothesis and underscored the need for a broader etiological framework. The objective of this review is to synthesise mechanistic evidence for an exposome‑based model of sporadic AD that integrates metabolic, inflammatory, and sleep/circadian contributions to brain aging. Epidemiological evidence indicates that up to 40% of AD risk is attributable to modifiable environmental and lifestyle factors. These factors, collectively termed the aging exposome, rep

Cadmium exposure has been implicated in the development of atherosclerosis, yet its relationship with carotid plaque composition and echogenicity remains poorly understood. A better understanding of the environmental and biological factors influencing plaque characteristics may facilitate more accurate stratification of patients at high risk of ischemic stroke. The aim of this study was to evaluate the relationships between environmental cadmium exposure, plaque calcium content, lifestyle-related factors, and carotid plaque echogenicity, and to assess their combined value for predicting a high

There is a paucity of research examining the impact of environmental chemical (EC) mixtures on sleep and the role of body mass index (BMI) in this association. We prospectively evaluated 1) the association between EC exposure (bio)marker-mixtures during peri-pregnancy and sleep outcomes after pregnancy, and 2) mediation by BMI. We studied ∼393 mothers from the PROGRESS cohort with 52 EC (bio)markers measured during pregnancy (estimated air pollutants, blood/urinary metal(loid)s, urinary high- and low-molecular-weight phthalate [HMWP, LMWP] and pesticide metabolites) and 1-month postpartum (ser
