Cardiorespiratory fitness profile of volunteer firefighters during intermittent incremental exercise: A cross-sectional study.

PURPOSE: We aimed to assess the physiological profile of male volunteer firefighters at a broad spectrum of exercise intensities to determine whether their cardiorespiratory fitness level is adequate to meet the physiological demands of their occupational activities. METHODS: Thirty-one male volunteer firefighters performed an intermittent incremental treadmill running exercise (with 4 min steps, 1 km·h-1 increments and 30 s rest periods). Cardiorespiratory variables were continuously monitored and blood lactate concentrations were obtained at baseline, throughout rest periods and post-exerci

Workplace-based integrative care for firefighters: real-world evidence from a visiting healthcare program.

INTRODUCTION: Firefighters, as essential workers, face substantial barriers to healthcare access due to shift work, unpredictable schedules, and physically demanding duties. Workplace-based healthcare delivery models have emerged as a potential strategy to address these access gaps, yet real-world evidence remains limited. METHODS: This study evaluated a workplace-based visiting healthcare program implemented at fire stations in Seoul, Korea, between September 2023 and February 2024. A retrospective chart review assessed changes in pain intensity among firefighters receiving integrative care,

Descriptive analysis of exposure levels of organophosphate esters among firefighters, paramedics, and office workers.

BACKGROUND: Organophosphate esters (OPEs) are a class of chemicals used as flame retardants and plasticizers in many products and industries. While research shows that OPE exposure is widespread in the general population, studies on the working population are scarce, despite their potential of being at higher risk of exposure due to their handling of OPE containing materials and products across their lifecycle. OBJECTIVES: This study aims to characterize OPE exposure among firefighters, paramedics, and office workers, to describe internal exposure levels, evaluate changes over a 24-h period i

Prevalence of Hearing Loss and Perceptions of Hearing Health and Protection among Florida Firefighters.

Firefighters are exposed to extensive hazardous noise while on the job, both during routine tasks at the station and when responding to calls. However, little is known about firefighters' occupational noise hazards. This study employed mixed methods, including focus groups, a survey, and audiometric testing, to identify sources of noise in the firefighters' work environment, determine hearing protective strategies, discern firefighters' perceptions of occupational noise exposure and impacts to their health, and quantify the prevalence of hearing loss among South Florida firefighters. A total o

Low testosterone and cardiometabolic risks in a real-world study of US male firefighters.

Low serum total testosterone (TT) is associated with increased cardiovascular risk and metabolic derangements, with fatty liver (FL) emerging as an additional cardiometabolic threat. We investigated the associations between TT and cardiometabolic (CM) health in 298 US male firefighters. Cross-sectional data from occupational health examination were analyzed. TT was categorized as low (< 264 ng/dL), borderline (264-399 ng/dL), and reference (400-916 ng/dL). Conventional CM risk factors were compared among TT categories, and between firefighters with and without FL. 81% of firefighters were obes

A pilot survey of Southeastern firefighters: Safety practices, use of protective gear, and injury.

INTRO: There were over 60 firefighter line of duty deaths and roughly 30,000 injuries in the United States in 2016. Modern thermoprotective gear has reduced the risks firefighters face from both thermal and inhalation injury, but must be used properly to be effective. The purpose of this study is to examine gear use and associated injury in firefighters. METHODS: Surveys were distributed with questions about demographics, gear usage, and maintenance practices. If previously injured, firefighters described the injury, treatment, and recovery. RESULTS: Of the 50 surveys distributed, 37 were re

Pack hike test finishing time for Australian firefighters: pass rates and correlates of performance.

The pack hike test (PHT, 4.83 km hike wearing a 20.4-kg load) was devised to determine the job readiness of USA wildland firefighters. This study measured PHT performance in a sample of Australian firefighters who currently perform the PHT (career land management firefighters, LMFF) and those who do not (suburban/regional volunteer firefighters, VFF). The study also investigated the relationships between firefighters' PHT performance and their performance across a range of fitness tests for both groups. Twenty LMFF and eighteen age-, body mass-, and height-matched VFF attempted the PHT, and a

Cognitive appraisal, emotional responses, and emotion regulation in firefighters' occupational stress-A pilot study.

INTRODUCTION: Firefighting is a highly demanding profession, involving frequent exposure to stress and life-threatening situations. This pilot study investigates firefighters' cognitive and emotional responses underlying problem-solving and emotion regulation during stressful operations. It explores how and to what extent firefighters experience their thoughts and emotions, focusing on subjective experiences of valence and physiological arousal, and whether they perceive their thoughts and emotions as helpful or hindrances. Additionally, we examine habitual emotion regulation strategies, accou

When Chronic Risk Meets Acute Strain: Sudden Cardiac Death and Cardiovascular Vulnerability in Firefighters.

Cardiovascular disease remains the leading cause of line-of-duty mortality among firefighters. Sudden cardiac death occurs disproportionately during emergency operations, particularly fire suppression, suggesting an interaction between occupational stressors and underlying cardiovascular vulnerability. This narrative review integrates epidemiologic, mechanistic, and clinical data from observational studies, systematic reviews, and contemporary cardiovascular guidelines. A structured literature search of PubMed and Google Scholar identified studies addressing cardiovascular disease, sudden card

Per- and polyfluoroalkyl substances (PFAS) and microRNA: An epigenome-wide association study in firefighters.

The occupation of firefighting is classified as a Group 1 carcinogen. Increased cancer risk among firefighters may be partly attributable to increased occupational exposure to a range of chemicals, including per- and polyfluoroalkyl substances (PFAS). Some PFAS are known and suspect human carcinogens. Investigating epigenetic response to these PFAS exposures in firefighters may help to identify biological pathways of specific cancers, and previously unidentified health outcomes that are associated with PFAS. We therefore investigated the associations of serum PFAS concentrations with miRNA exp
