INTRODUCTION: Proximal humeral fractures with a spiral line of fracture extending from the humeral head to the diaphyseal region are increasing. Treatment for these fractures is comparable to that for shaft fractures. The purpose of this study was to evaluate the use of a new "Long" humeral nail for this type of lesion and identify the best distal locking.
MATERIALS AND METHODS: Forty-three patients treated with a Long Diphos Nail® were selected for this study: main exclusion criteria were poor cognitive and responsive ability to physical therapy, four-part fracture requiring humeral head rep

BACKGROUND: Supracondylar humerus (SCH) fractures comprise about 20% of pediatric fractures. Displaced SCH fractures require reduction and percutaneous pinning, most commonly making use of conventional C-arm (CCA) fluoroscopy. Many authors have proposed the use of mini C-arm (MCA) in the upper extremity, given multiple proposed benefits including decreased radiation. We investigated the utility of MCA in the operative treatment of pediatric SCH fractures.
METHODS: Patients who underwent operative fixation of SCH fractures using MCA from 20202022 were reviewed. Demographic, surgical, and radio

BACKGROUND: Supracondylar humerus fracture (SCHF) is the most common elbow fracture in children. For displaced fractures, percutaneous Kirschner wire (K-wire) fixation is the preferred treatment. Although standard wire placement is traditionally recommended, clinical constraints-such as soft tissue swelling, limited surgical access, and variable anatomy-may necessitate deviations from standard configurations. This study aimed to evaluate the clinical efficacy and safety of nonstandard K-wire placement in pediatric SCHF, providing evidence to support practical surgical decision-making.

Background and Clinical Significance: Non-prosthetic peri-implant fractures (NPPIFs) are rare injuries occurring around internal fixation devices, and are distinct from periprosthetic fractures. While most studies focus on the femur, humeral NPPIFs remain poorly documented. This case illustrates a complex humeral NPPIF and highlights key surgical considerations. Case Presentation: A 62-year-old woman presented with a spiral humeral shaft fracture (AO 12B2) after a fall. Following closed reduction and antegrade intramedullary nailing, an intraoperative peri-implant fracture occurred at the dist

BACKGROUND: The authors modified the anterolateral approach for Holstein-Lewis humeral shaft fractures using a plating technique to achieve sufficient distal fixation by minimal splitting of the brachioradialis muscle to fix the most distal screws. The purpose of this retrospective study was to evaluate the efficacy of our modified anterolateral approach for Holstein-Lewis humeral fractures and document clinical and functional results.
MATERIALS AND METHODS: Between 2008 and 2014, 18 patients (mean age 35.4 years) with a Holstein-Lewis humeral shaft fracture who underwent open reduction and i

INTRODUCTION: The management of the pink pulseless hand (PPH) following paediatric supracondylar humerus fractures remains controversial, with no clear consensus between orthopaedic management and early neurovascular surgical exploration. This study aimed to compare outcomes of these strategies through systematic review and meta-analysis.
METHODS: A systematic review was conducted in accordance with PRISMA guidelines. Multiple electronic databases were searched up to December 2024. Retrospective cohort studies evaluating management of PPH in paediatric supracondylar fractures were included. P

Distal humerus fractures are challenging injuries due to their soft tissue envelop, and the complex 3 dimensional anatomy including 3 articular surfaces. Modern fixation principles have allowed for improved outcomes even in previously unreconstructible fracture patterns. Despite these advances bone loss, and articular comminution may require alternative or adjunctive strategies such as grafting, fusion, hemi-arthroplasty and total elbow arthroplasty options.

BACKGROUND: Distal humerus fractures are a challenge to treat, and the current standard of care, open reduction internal fixation with a double-plate, has a high rate of complications. We proposed a novel internal fixation configuration, lateral intramedullary nail and medial plate (LINMP) and verified its rigidity through biomechanical tests and finite element analysis.
METHODS: The study involved biomechanical testing of 30 synthetic humerus models to compare 2 different fixation systems for an AO 13C-2.3 type fracture. The orthogonal double-plate (ODP) group and the LINMP group were compar

PURPOSE: AI has shown promise in automating and improving various tasks, including medical image analysis. Distal humerus fractures are a critical clinical concern that requires early diagnosis and treatment to avoid complications. The standard diagnostic method involves X-ray imaging, but subtle fractures can be missed, leading to delayed or incorrect diagnoses. Deep learning, a subset of artificial intelligence, has demonstrated the ability to automate medical image analysis tasks, potentially improving fracture identification accuracy and reducing the need for additional and cost-intensive

INTRODUCTION: In children, the choice between percutaneous pinning (PP) and open pinning fixation (OPF) for the surgical treatment of fractures of the distal humerus remains controversial, especially the PP method for internal humeral condylar (IHC) fractures.
PATIENTS AND METHODS: Eighty fractures of the distal humerus in children were treated surgically in our hospital over a ten year period. 47% (n = 38) were supracondylar (SC), 20% (n = 16) comminuted (COM), 18% (n = 14) internal humeral condylar (IHC), and 15% (n = 12) lateral humeral condylar (LHC). We used PP, OPF and three times osteo

OBJECTIVE: Stable internal fixation of the humeral shaft by less invasive percutaneous plate insertion using two separate (proximal and distal) incisions, indirect reduction by closed manipulation and fixation to preserve the soft tissue and blood supply at the fracture zone. Early mobilization of the shoulder and elbow to ensure a good functional outcome.
INDICATIONS: Humeral shaft fractures (classified according to AO classification as: 12-A, B, C). Humeral shaft fractures extending to the proximal or distal shaft, small or deformed medullary canal or open growth plate.
CONTRAINDICATIONS:

BACKGROUND: Operative treatment of humeral shaft fractures (AO/OTA 12) is being performed more frequently. Accordingly, it is important to understand the complications associated with plate fixation. This study analyzes risk factors associated with mechanical failure following plate fixation of humeral shaft fractures in order to further elucidate the mode and location of failure.
METHODS: A retrospective review of 351 humeral shaft fractures was completed at a single level I trauma center. Eleven of eighty-five humeral shaft fractures had aseptic mechanical failure requiring revision (12.9%)

We report the case of a 49-year-old woman with severe elbow ankylosis 10 weeks after a trochlea fracture treated with open reduction and internal fixation. Imaging confirmed failure of open reduction and internal fixation with a displaced and severely damaged trochlea. We treated the nascent malunited trochlea and associated elbow ankylosis with a distal humeral hemiarthroplasty and circumferential elbow arthrolysis. The patient regained functional range of motion of the elbow and had minimal pain. Distal humeral hemiarthroplasty, which has been indicated for acute fractures involving the capi

BACKGROUND: Although cross-pinning is widely used to treat pediatric supracondylar humeral fractures, it involves risks of nerve injury, malunion, and compartment syndrome. In this two-center retrospective observational study, we compared the outcomes of humero-ulnar external fixation using an adult wrist fixator with those of conventional pinning for pediatric supracondylar humeral fractures. Given the technical complexity of Slongo lateral external fixation, we evaluated a simpler joint-spanning external fixation method.
METHODS: This study enrolled 37 children: 19 treated with pinning betw

OBJECTIVE: Olecranon osteotomy and paratricipital approaches were widely used in the treatment of type C distal humerus fracture but some disadvantages exist, so a combined medial and lateral approach was designed. The objective of this study was to investigate and compare the clinical outcomes of combined medial and lateral approach with the paratricipital approach in open reduction and internal fixation of type C distal humerus fractures.
METHODS: From May 2018 to April 2020, 37 patients with type C distal humerus fracture who accepted open reduction and internal fixation in our hospital we

Humeral neck fracture is rarely associated with injury of the nearby axillary artery and in the English literature only 29 such cases have been reported. An injury of the distal axillary artery secondary to reduction and fixation of a humeral neck fracture is a very rare and unusual complication, to the best of our knowledge has never been reported in literature. In this paper we report the case of entrapment of the distal axillary artery in the humerus rhyme fracture in a 9-year-old child who suffered a nighttime car-accident with her father. At admission, the child presented a left humeral n

Separation of the distal humeral epiphysis in the neonate or young child can be diagnosed confidently by combining roentgenographic and clinical information. Such diagnosis is critical, as two abnormalities included in the differential diagnosis--irreducible elbow dislocation and fracture to the lateral humeral condyle--often require open reduction, unlike distal humeral epiphyseal separation, which can be treated conservatively. We describe two patients and review the pertinent literature.
