WIse CHoices in the ICU (WICH-ICU): Protocol and Statistical Analysis Plan for a Stepped-Wedge Cluster Randomized Trial of Choosing Wisely Interventions in Swedish Intensive Care Units.

BACKGROUND: Healthcare overuse, including unnecessary diagnostic testing and procedures, contributes to patient harm, increased healthcare costs, and resource waste. The Choosing Wisely initiative aims to reduce low-value care through evidence-based recommendations. However, limited evidence exists regarding the safety and effectiveness of implementing Choosing Wisely recommendations in intensive care settings. METHODS: The WICH-ICU is a registry-based stepped-wedge cluster randomized controlled trial designed to evaluate the implementation of specific Choosing Wisely interventions in Swedish

Development and Validation of Puberty Interpreter, a Mobile Clinical Decision-Support Tool for Pubertal Disorders.

OBJECTIVE: To develop and validate Puberty Interpreter, a mobile application-based point-of-care clinical decision support tool for pubertal disorders. METHODS: Puberty Interpreter recommendations in 306 children with puberty concerns (199 early and 107 delayed puberty) were compared to the gold standard, a pediatrician, a senior trainee, and a junior trainee. RESULTS: Puberty Interpreter concordance with the gold standard was 303/306 (99.0%) for diagnosis, 303/306 (99.0%) for management, and 606/612 (99.0%) overall. This was higher than the pediatrician (476/612, 77.8%), the senior trainee

On interpreters: the ethics of interpreter use in general practice.

In the absence of language congruency between patient and general practitioner (GP), the use of an interpreter is essential for a safe and effective clinical consultation. A substantial proportion of individuals living in the UK lack sufficient command of the English language to allow direct communication with their GP. Interpreters in UK general practice can be classified into three major categories: Casual, professional in-person and professional telephone interpreters. The use of casual interpreters threatens the safety and quality of care provided in interpreted consultations, increases th

Effect of the use of artificial intelligence tools on clinical education and clinical decision-making of medical students during clinical rotations: A scoping review.

Medical education is transforming due to new technologies such as artificial intelligence (AI). Although AI is being considered for medical education, its integration into the curriculum remains limited. Furthermore, few studies evaluate how these tools affect students' diagnostic accuracy during clinical rotations. This study aims to evaluate available evidence regarding AI tools in medical students, identifying their effects on clinical education and clinical decision-making, as well as their implications for medical practice during clinical rotations. For this purpose, a scoping review was

FeaInfNet: Diagnosis of Medical Images With Feature-Driven Inference and Visual Explanations.

Interpretable deep-learning models have received widespread attention in the field of image recognition. However, owing to the coexistence of medical-image categories and the challenge of identifying subtle decision-making regions, many proposed interpretable deep-learning models suffer from insufficient accuracy and interpretability in diagnosing images of medical diseases. Therefore, this study proposed a feature-driven inference network (FeaInfNet) that incorporates a feature-based network reasoning structure. Specifically, local feature masks (LFM) were developed to extract feature vectors

Navigating Emergency Care After Injury: A Qualitative Study of Asylum Seekers and Refugees in England and Wales Using the Candidacy Framework (BE SURE Study).

BACKGROUND: Asylum seekers and refugees (ASRs) face significant challenges when accessing emergency and pre-hospital care following injury. Language barriers, unfamiliarity with service pathways and limited trust in authorities hinder access to emergency services. We report results of focus groups with asylum seekers and refugees who have accessed emergency services to understand their use of services and care received. METHOD: We recruited participants through community organisations and support networks which advocate and support asylum seekers and refugees across four areas in England and

How Does Routine Prostate-specific Membrane Antigen Positron Emission Tomography/Computed Tomography Modify the Current Management of Prostate Cancer? A Multidisciplinary View.

BACKGROUND AND OBJECTIVE: Prostate-specific membrane antigen positron emission tomography/computed tomography (PSMA-PET/CT) and new treatment modalities have expanded the possibilities for diagnosing and managing metastatic prostate cancer, but have also raised questions about their implementation in daily clinical practice. We sought consensus on definitions, preferred imaging modality for staging, and treatment selection in the era of next-generation imaging. METHODS: A modified Delphi method involved two voting rounds and a face-to-face multidisciplinary meeting with 40 Dutch prostate canc

Association of recreational cannabis legalization with changes in medical, illegal, and total cannabis expenditures in Canada.

BACKGROUND: Recreational cannabis legalization (RCL) is being adopted by a growing number of jurisdictions internationally. RCL aims to displace the illegal cannabis market and has the potential to disrupt the medical market, yet few studies have examined these dynamics empirically. METHODS: We used interrupted time series analysis to evaluate whether RCL (legislative passage in October 2017/implementation in October 2018) was associated with changes in quarterly national household expenditures on medical cannabis, illegal cannabis, and all cannabis types combined (licensed, illegal, and medi

Gastrointestinal Bleeding at CT Angiography and CT Enterography: Imaging Atlas and Glossary of Terms.

Gastrointestinal (GI) bleeding is a common potentially life-threatening medical condition frequently requiring multidisciplinary collaboration to reach the proper diagnosis and guide management. GI bleeding can be overt (eg, visible hemorrhage such as hematemesis, hematochezia, or melena) or occult (eg, positive fecal occult blood test or iron deficiency anemia). Upper GI bleeding, which originates proximal to the ligament of Treitz, is more common than lower GI bleeding, which arises distal to the ligament of Treitz. Small bowel bleeding accounts for 5-10% of GI bleeding cases commonly manife

Prediction of respiratory distress syndrome using the Abbott FLM-II amniotic fluid assay.

BACKGROUND: Most laboratories using the Abbott FLM-II assay for assessing fetal lung maturity follow the manufacturer's recommendations for interpreting the surfactant to albumin ratio (S/A). Thus, values >55 mg/g are considered mature and values <40 mg/g, immature-leaving a wide range of indeterminate values. Little data is available to assist the clinician in interpreting values between 40 and 55 mg/g. The goal of this study was to determine decision levels that would more clearly identify risk for RDS based on S/A results. METHODS: Respiratory distress syndrome was identified based on medi

Different Methods for Traumatic Brain Injury Diagnosis Influence Presence and Symptoms of Post-Concussive Syndrome in United States Veterans.

Common methods for evaluating history of traumatic brain injury (TBI) include self-report, electronic medical record review (EMR), and structured interviews such as the Head Trauma Events Characteristics (HTEC). Each has strengths and weaknesses, but little is known regarding how TBI diagnostic rates or the associated symptom profile differ among them. This study examined 200 Veterans recruited within the VA Portland Health Care System, each evaluated for TBI using self-report, EMR, and HTEC. Participants also completed validated questionnaires assessing chronic symptom severity in broad healt

Visualization of the relationship between electrogustometry and whole mouth test using multidimensional scaling.

Interpreting the relationship between different taste function tests of different stimuli, such as chemical and electrical stimulation, is still poorly understood. This study aims to analyze visually as well as quantitatively how to interpret the relationship of results between taste function tests using different stimuli. Patients who underwent the whole mouth test and Electrogustometry (EGM) at a tertiary medical center between August 2018 and December 2018 were reviewed retrospectively with electronic medical records. Of the 110 patients, a total of 86 adults who self-reported that their ta

A Rare Case of Intracranial Hemorrhage Mimicking a Neoplasm in a Child With Hereditary Hemorrhagic Telangiectasia.

Hereditary hemorrhagic telangiectasia (HHT) is an autosomal dominant disorder characterized by abnormal blood vessel formation, resulting in the development of arteriovenous malformations and telangiectasias throughout the body. We present a rare case of an intracranial hemorrhage mimicking a neoplasm in a medically complex pediatric patient with HHT, a diagnosis that was unbeknownst to both the clinical team and the interpreting radiologist early in the patient's clinical course. This case not only demonstrates the importance of obtaining a thorough medical history but having a clear understa

Kartagener's syndrome: A rare condition diagnosed in a young male patient.

Kartagener's Syndrome is a rare autosomal recessive genetic condition, that affects the structure and function of cilia and includes a condition of situs inversus, chronic sinusitis, and bronchiectasis associated sometimes with infertility. A young patient who had a long-time fever, cough, and infertility after a clinical evaluation performed a chest X-ray and a CT scan that revealed the unexpected condition of Situs Inversus Totalis (SIT). Imaging also showed bronchiectasis and sinusitis: all findings consistent with Kartagener's syndrome, confirmed a second time by the genetic test. This cas

Patent ductus arteriosus ligation and adverse outcomes: causality or bias?

Observational studies have associated patent ductus arteriosus (PDA) ligation in preterm infants with increased chronic lung disease (CLD), retinopathy of prematurity, and neurodevelopmental impairment at long-term follow-up. Although the biological rationale for this association is incompletely understood, there is an emerging secular trend toward a permissive approach to the PDA. However, insufficient adjustment for postnatal, pre-ligation confounders, such as intraventricular hemorrhage and the duration and intensity of mechanical ventilation, suggests the presence of residual bias due to c

Heinrich Heine and syphilis.

Though Heinrich Heine died 151 years ago, the underlying illness responsible for his suffering of many years has never been fully clarified. The diagnosis put forward most frequently in retrospect was that of a venereal disease. However, this diagnosis was the result of an interpretation in disregard of the historical context. Only by evaluating and interpreting the entire source material in its historical context (Heine's complete works, his correspondence, records of conversations kept by contemporaries and prescriptions) can this diagnosis be confirmed on a reliable basis. Above all, medica

Small bowel obstruction: the role of computed tomography in its diagnosis and management with reference to other imaging modalities.

Small bowel obstruction is a leading cause of acute surgical admissions for abdominal pain. There is an increasing tendency for initial conservative management rather than immediate operative intervention, as a proportion of cases will resolve spontaneously. This has resulted in a growing reliance on radiological investigations to reassure the surgeon that medical therapy can be safely instituted. The onus therefore rests with radiologists to guide their surgical colleagues by correctly interpreting the plain abdominal radiograph and suggesting appropriate further investigation if warranted. The validity of proxy-based data in suicide research: a study of patients 50 years of age and older who attempted suicide. I. Psychiatric diagnoses.

OBJECTIVE: To investigate the validity of best-estimate methodology for making psychiatric diagnoses among individuals who attempted suicide. METHOD: Subjects were 80 patients admitted for treatment following a suicide attempt. Psychiatric diagnoses based on structured interviews with subjects were compared with diagnoses made based on interviews with proxy respondents. In both cases, interview information was supplemented with pre-admission psychiatric and medical records to inform diagnoses. RESULTS: Diagnostic agreement, based on kappa coefficients, was substantial for major depression an

A Pilot Study of Junior Medical Officers in Sydney, Australia: Knowledge and Attitudes Towards Artificial Intelligence.

Introduction Artificial intelligence (AI) use is growing generally and specifically in healthcare. It has multiple applications across medicine, including summarising patient consultations, interpreting investigations and improving patient flow. Junior Medical Officers (JMOs) working in Northern Sydney Local Health District (NSLHD), in New South Wales (NSW), are amongst the first generations of clinicians to experience the broadest expansion of the use of AI and are likely to determine the guidelines for its use. However, they remain an underrepresented group in the literature. Aim This study

Quality control and quality assurance of medical committee performance in the Israel Defense Forces.

PURPOSE: The paper aims to focus on a unique quality control system that was set up ten years ago to evaluate Medical Classification Committee decision-making processes at the Israel Defense Forces (IDF) conscription center. DESIGN/METHODOLOGY/APPROACH: Two main approaches were deployed by the control system to assess medical classification committees' performance. The first was direct assessment of the medical committees' clinical work and decision-making processes. The second applied data mining procedures to the computerized medical databases. The functional classification codes (FCCs)--co
