[The causes and preventive methods of misdiagnosis on wrist scaphoid bone fracture].

OBJECTIVE: To retrospectively analysis the curative effect of wrist scaphoid bone fracture, and explore the causes and preventive methods of misdiagnosis. METHODS: From September 2007 to September 2010,16 patients with wrist scaphoid bone fractures were treated with plaster cast and cannulated screws fixation. There were 10 males and 6 females, ranging in age from 26 to 44 years with an average of 35 years. Among them, 12 cases manifested swelling pain of radial lateral wrist, tenderness at snuffbox area, wrist pain aggravated when stretching wrist joint, thumb or forefinger; 4 cases manifested…

Trans-scaphoid perilunate dislocation with fractured carpal bones in a child.

Transcarpal fractures and dislocations in children are rarely reported in the orthopedics literature. This is a report of a 10-year-old boy who sustained a trans-scaphoid perilunate dislocation with fractures across the carpal structure: these included injuries to the scaphoid, capitate and triquetrum bones. Treatment consisted in a closed reduction for the dislocation and using the dorsal approach of an open reduction with internal fixation of the fractures. The injury healed well with a full return of good wrist function. This unusual pattern of injury is described so that it may be more rea

Treatment of scaphoid non-union with a percutaneous corticocancellous bone graft.

Thirty-three symptomatic scaphoid non-unions were treated by a simple, minimally invasive procedure using a percutaneous autologous corticocancellous bone graft. After an average follow-up of 3.5 years, union was observed in 29 cases. These patients had no, or mild, pain at work and an almost normal range of motion and grip strength. No progression to osteoarthritis was observed.

Primary vascularized distal radius bone graft for nonunion of the scaphoid.

A vascularized bone graft from the dorsoradial aspect of the distal radius was used with internal fixation to treat nonunion of the scaphoid in ten patients who had not received any previous surgical treatment. Five cases were classified as Lichtman type I and five as type II. The average age was 30 years (range, 18-40 years). Associated avascular necrosis was observed in five cases. Postoperatively pain was relieved and union was achieved in all cases. The mean time to union was 6.5 weeks. Range of motion, grip strength and pinch strength were also restored satisfactorily. These results sugge

[Long-term outcome of partial alloplastic replacement of the scaphoid bone].

The operative treatment of scaphoid nonunion with a small, sclerotic, or avascular proximal fragment and with accompanying radioscaphoid arthrosis is difficult and often disappointing. Excision of the proximal fragment, styloidectomy, partial replacement of the scaphoid, and insertion of a silicone-rubber lunate prosthesis has been recommended in these cases. From 1980 to 1984, eleven patients (all male, average age 42 [25 to 59] years) with conditions described above were treated by partial replacement of the scaphoid. In one patient, the prosthesis dislocated dorsally and was removed five mo

Treatment of scaphoid nonunions by Russe bone grafting--a retrospective analysis.

Forty-five patients with symptomatic established delayed union and nonunion of the scaphoid were operated between January 1983 and December 1997. The Russe anterior corticocancellous bone-grafting was performed in twenty-five cases. Using a grading system, we analyzed the suitability of this method, for three types of nonunion. We attained a 92% bony union rate of all nonunions grafted. Review of the relevant literature shows that there was same significant difference in union rates between the types of nonunion, the vascularity of the proximal fragment and the presence of dorsal intercalated

Diagnostic problems of scaphoid fractures: the value of radionuclide bone scintigraphy.

The diagnosis fracture of the scaphoid bone based on mere X-ray examination is unreliable. A correct diagnosis of scaphoid fractures is essential for the prevention of avascular necrosis of the proximal fragment and of pseudo-arthrosis. Thirty-three patients with a clinically suspected scaphoid fracture and a normal radiograph were examined. These patients had supplementary 99mTc-MDP scintigraphy. Eight of the 33 patients had normal scans, 12 patients had a scaphoid fracture and 17 had other fractures. Based on our findings, radionuclide bone scintigraphy is useful and accurate for the diagnos…

[Fracture of the scaphoid bone--surgical treatment, indications, method and results].

In fractures of the scaphoid bone, conservative treatment leads to an osseous consolidation in 95% of all cases. A primary surgical treatment seems justified in case of instability by torsion, tilting, or shifting of the fragments, associated fractures of the same arm, as well as de Quervain dislocation fracture. The different surgical possibilities are described and the complications are compiled in a special study. The question if a recent fracture of the scaphoid bone should be managed by conservative or surgical treatment ist not justified considering the rate of 14.5% of complications, i.…

[Immobilization of the scaphoid bone (alterative between upper and forearm plaster)].

As a result of the lengthy rehabilitation of the non-injured but fixed elbow in the treatment of scaphoid fractures or pseudarthroses in a longarm plaster cast, a modification has been developed, which inhibits supination and pronation of the forearm while the flexion-extension movement of the elbow is permitted by the incorporation of a hinge.

[Bilateral lunato-triquetral synostosis with bilateral fracture of the scaphoid bone].

The authors have treated a case of bilateral fracture of the scaphoid bone in a young man who had complete and bilateral lunato-triquetral synostosis, without any other associated congenital condition. Consolidation was obtained by orthopedic treatment on one side, but a Matti-Russe operation was necessary on the other. The authors emphasize but then rule out the hypothesis of a congenital bipartite scaphoid associated with the synostosis. A review of the literature on lunato-triquetral synostoses sums up the main points. The condition is rare in whites and occurs more frequently in blacks. It…

[Technic and results of tendon interposition arthroplasty of the lunate and scaphoid bones].

Between 1971 and 1982 tendon interposition arthroplasty in combination with denervation of the wrist joint was performed on 55 patients with either Kienböck's disease, irreducible lunate dislocations with or without scaphoid dislocation, or scaphoid non-union with a small proximal fragment. Follow-up was possible on 38 patients (69%). In two third of the cases good and very good results are achieved with this operative technique. The significant points were the arthrosis in the wrist joint and the complete obliteration with tendon material of the cavity left by the extirpated carpal bone. Ulna

The diagnostic value of displacement of the fat stripe in fracture of the scaphoid bone.

Soft tissue abnormalities on radiological examination can be useful in evaluating injury to various regions. In a retrospective study of X-ray pictures of 125 patients with injury to the wrist, it was possible to demonstrate an abnormal scaphoid fat stripe (SFS) in seventy-three out of seventy-eight cases with fracture of the scaphoid bone, compared to ten out of forty-seven cases without fracture.

The surgical approach and source of bone graft for symptomatic nonunion of the scaphoid.

In 2 groups of patients treated by bone grafting for scaphoid nonunions, one group had a dorsal or dorsolateral surgical approach with a bone graft from the radius while the other group had a volar approach with an iliac graft. The best results were obtained with an iliac graft. Through the volar approach it is relatively easy to avoid damage to the scaphoid arterial supply. A small proximal vascular pole led to a failure rate of 36%.
