Endoscopic sphincterotomy for common bile duct calculi.

A series of 74 patients having endoscopic sphincterotomy for common bile duct calculi is reported. Complete stone extraction was achieved in 53 cases (72%). Seventeen of 21 patients with retained calculi following recent biliary surgery had successful extractions (80%). Of 30 patients having had a cholecystectomy, 21 (70%) were successful, but only 15 of 23 patients with obstructive jaundice and no previous biliary surgery had the ducts cleared of calculi. Failure was due to multiple stones in the duct, or calculi too large to pass through the sphincterotomy. Endoscopic sphincterotomy is advoc…

Long-term follow-up after endoscopic sphincterotomy.

Endoscopic sphincterotomy has become a well-accepted procedure in choledocholithiasis and papillary stenosis. Long-term follow-up data covering an observation period of 7 years disclose that about 90% of the patients are either completely relieved of symptoms, or show marked improvement. Re-stenosis or recurrent concrements seem are events; in primary papillary stenosis, however, long-term results are less favorable.

The twelve o'clock sphincterotomy: technique, indications, results.

Outflow obstruction at the level of the external sphincter in patients with neurogenic bladder dysfunction is common and transurethral sphincterotomy the treatment of choice. Based upon the anatomy of the striated muscle fibres forming the external sphincter and its blood supply, we find from our results on 35 patients, 17 of whom had a follow-up of more than three years, a transurethral sphincterotomy only in the 12 o'clock position as sufficient and preferable. The results indicate that this comparatively simple technique is effective in order to diminish outflow resistance at the level of t…

Randomized clinical trial comparing oral nifedipine with lateral anal sphincterotomy and tailored sphincterotomy in the treatment of chronic anal fissure.

BACKGROUND: The conventional treatment of chronic anal fissure is lateral sphincterotomy (LAS). The alternative options of tailored sphincterotomy (TS) and 'chemical sphincterotomy' using medication such as nifedipine have recently become available. METHODS: A prospective randomized trial was conducted to compare LAS with TS and oral nifedipine. The main endpoints were fissure healing, symptom relief, recurrence and continence. RESULTS: One hundred and thirty-two patients were treated and followed up for 4 months. LAS was significantly more effective than TS in providing pain relief (P = 0.0…

Sphincter of Oddi dysfunction: pancreaticobiliary sphincterotomy with pancreatic stent placement has a lower rate of pancreatitis than biliary sphincterotomy alone.

BACKGROUND AND STUDY AIMS: Patients with suspected or documented sphincter of Oddi dysfunction (SOD) who undergo standard biliary sphincterotomy have high rates of post-procedure pancreatitis. Approximately 75% of such patients have elevated basal pressures of the pancreatic sphincter. Biliary sphincterotomy (BES) on its own leaves the pancreatic sphincter unablated and may cause transient edema which aggravates the increase in pancreatic sphincter pressure. Combined pancreaticobiliary therapy (PBR), using pancreatic stenting in addition to sphincterotomy may therefore be safer. PATIENTS AND …

The effect of Trolox on the rabbit anal sphincterotomy repair.

INTRODUCTION: Fecal incontinence (FI) is caused by external anal sphincter injury. Vitamin E is a potential strategy for anal sphincter muscle repair via its antioxidant, anti-inflammatory, anti-fibrotic, and protective properties against myocyte loss. Thus, we aimed to evaluate the water-soluble form of vitamin E efficacy in repairing anal sphincter muscle defects in rabbits. METHODS: Twenty-one male rabbits were equally assigned to the intact (without any intervention), control (sphincterotomy), and Trolox (sphincterotomy + Trolox administration) groups. Ninety days after sphincterotomy, th…

Wire-assisted access sphincterotomy of the minor papilla.

BACKGROUND: Recommended techniques for minor papilla sphincterotomy include performing a standard pull-type sphincterotomy (PTS) or using a needle-knife over a stent. A wire-assisted access sphincterotomy (WAAS) technique may hold some technical advantages over these accepted methods, but has not been robustly described. OBJECTIVE: To describe the safety and efficacy of WAAS compared with PTS in a series of patients from our institution. DESIGN: Retrospective audit of initial minor papilla sphincterotomies over a 6-year period. Demographic and procedural data were abstracted, and the medical…

[Endoscopic sphincterotomy as an out-patient procedure: is it safe?].

BACKGROUND: Patients have been routinely admitted for observation for potential complications after therapeutic ERCP; however, in this era of cost containment it may be more cost-effective to perform these procedures on an out-patient basis. AIM: The purpose of this study was to determine safety and complication rates of endoscopic sphincterotomy in out-patients. MATERIALS AND PATIENTS: Over an 11-month period, 124 consecutive patient undergoing endoscopic sphincterotomy for biliary and pancreatic disease were enrolled in a prospective and randomized manner. Sixty patients (Group A) were obs…

Safety and efficacy of pancreatic sphincterotomy in chronic pancreatitis.

BACKGROUND: Endoscopic pancreatic sphincterotomy (EPS) is being performed with increasing frequency as a prerequisite to interventional measures in the pancreatic duct. The aim of this study was to evaluate EPS with regard to technique, success, complications, and mortality in patients with chronic pancreatitis. METHODS: Between January 1989 and September 1996, the results of all consecutive EPSs in patients with chronic pancreatitis were documented in a standardized form. Patients were followed by clinical investigation and blood sample analysis at 4, 24, and 48 hours after EPS. Complication…

Endoscopic sphincterotomy for acute pancreatitis: arguments in favour.

Pathogenetic factors of acute biliary pancreatitis include persistent or transient, absolute or relative, obstruction of Vater's ampulla or … for the use of early biliary decompression in patients with acute biliary pancreatitis. Endoscopic sphincterotomy offers a cost-effective and safe alternative to surgical biliary decompression with an overall morbidity rate of 8% and mortality rate of 2.4%. Randomized clinical trials showed that endoscopic sphincterotomy is useful in patients with predicted sev …

Lateral internal sphincterotomy for fissure-in-ano--revisited.

PURPOSE: A disturbing complication rate following lateral internal sphincterotomy (LIS) has recently been reported, and this study assessed our complication rate. METHODS: Charts of 312 patients who were operated on between January 1975 and December 1993 were reviewed. RESULTS: There were 148 men (47.5 percent) and 164 women (52.5 percent), ranging in age from 16 to 95 (mean, 40) years. Two hundred sixty-five patients underwent LIS as a primary and sole procedure. Mean follow-up time was 20.4 months (range, 2 weeks-215 months). Healing times were less than one month (11.6 percent), one month…

[Precut sphincterotomy in impacted choledocholithiasis at the Vater's ampulla].

BACKGROUND: Endoscopic sphincterotomy is an established treatment for common bile duct stones. Stone impaction at the ampulla makes deep cannulation and standard sphincterotomy more difficult. The use of precut papillotomy may facilitate stone extraction, although risks may be greater. AIM: To evaluate precut papillotomy in impacted common bile duct stone at the ampulla of Vater. PATIENTS AND METHODS: Between October 1990 and September 1995, 27 of 345 patients with common bile duct stones underwent needle knife precut papillotomy after conventional sphincterotomy failed due to impacted ampul…

Needle knife papillotomy for endoscopic sphincterotomy and cholangiography.

Needle knife papillotomy (NKP) was used to achieve diagnostic endoscopic cholangiography in 39 patients and endoscopic sphincterotomy in 24 patients when conventional endoscopic methods had failed. These patients represent 2.2% (cholangiography) and 4.2% (sphincterotomy) of the total number of patients undergoing these procedures during the same period. Cholangiography was achieved in 37 of 39 patients and sphincterotomy was successfully accomplished in all 24 patients with the assistance of NKP. Complication rates of 2.5% for cholangiography and 12% for sphincterotomy are higher than our own …
