A newly designed thorax support vest prevents sternum instability after median sternotomy.

OBJECTIVE: Sternum infection remains one of the primary causes of postoperative morbidity and mortality after median sternotomy. We report the clinical efficacy for primary reinforcement of the sternum with a new design of thorax support vest. METHODS: A prospective randomized study including 455 patients was started in September 2007 to evaluate the effectiveness of the Posthorax sternum vest (Epple Inc., Vienna, Austria). One hundred and seventy five patients were treated with the sternum dressing postoperatively (group A), 227 patients did not receive the vest (group B) and 53 patients ref…

Successful surgical repair of congenital total cleft sternum with partial ectopia cordis.

Total bifid sternum with or without ectopia cordis is a rare congenital anomaly which is treated successfully in a limited number of cases. Several methods of correction have been described using tissue grafts (cartilage, bone), inert artificial prostheses (acrylic plaques, marlex mesh), and techniques to approximate the sternal bands by relaxation of costal cartilages. In our case the method described by Verska was used to approximate the bifid sternal bands. The successful result in our patient proves the efficiency of this technique.

Effect of chicken sternum cartilage on bone mineral density and its mechanism.

BACKGROUND: Osteoporosis is defined by decreased bone mineral density (BMD) - a core factor for bone strength and fracture risk - with progression often exacerbated by chronic inflammation. However, the role of chicken sternum cartilage (CSC) in improving bone health and its anti-inflammatory-related mechanisms remain unclear. Thus, this study investigated CSC's effect on bone health and elucidated its potential anti-inflammatory mechanism. RESULTS: In vitro experiments using MC3T3-E1 osteoblasts showed that CSC at a concentration of 1 mg mL-1 significantly promoted the growth, differentiatio…

Primary Ewing's Sarcoma of the Sternum in an Adult Male: A Rare Case Report.

Ewing's sarcoma, a rare primary bone malignancy primarily affecting adolescents and young adults, typically manifests in the pelvic bones and femur. Primary Ewing's sarcoma of the sternum is exceptionally rare, constituting less than 1% of cases. We present a case of a 34-year-old man with a 2-month history of anterior chest wall pain initially attributed to muscular spasm. Subsequently, the patient developed a palpable mass and imaging demonstrated a mid-lower sternal lesion with cortical destruction and soft tissue involvement, confirmed as Ewing's sarcoma on biopsy. In addition, a suspiciou…

Rib and sternum fracture risks for restrained occupants in frontal car crashes.

OBJECTIVE: Most car occupant fatalities occur in frontal crashes and the thorax is the most frequently injured body region. The objectives of the study were, firstly, to quantify the relation between risk factors (such as speed and occupant age) and rib and sternum fracture injury probability in frontal car crashes, and, secondly, to evaluate whether rib fracture occurrence can predict sternum fractures. METHODS: Weighted German data from 1999-2021 were used to create the injury risk curves to predict both, at least moderate and at least serious, rib and sternum fracture risks. A contingency …

Spectrum of anatomical variants, normal findings and pathology in and around the paediatric sternum.

Abnormalities of the sternum and adjacent structures are an uncommon presentation in the paediatric population and can have a variety of benign and malignant causes, including normal and developmental variants of the chest wall. Although there is overlap with adults, many sternal abnormalities are unique to the paediatric population. Following clinical examination, radiography is usually the first type of imaging used; however, it is limited and often ultrasound and cross-sectional imaging are needed for further assessment. An understanding of the normal anatomy is important; however, this can…

Sternum-Sparing HVAD Implantation with Attachment of the Outflow Graft to the Descending Aorta.

The standard approach for implanting an HVAD left ventricular assist device (LVAD) is performing a median sternotomy and sewing the outflow graft to the ascending aorta. However, in patients with sternal comorbidities, it may be advantageous to use a sternum-sparing approach. We retrospectively studied eight patients who underwent HVAD implantation for destination therapy via a left subcostal or fifth/sixth intercostal space incision. With this procedure, the outflow graft was sewed to the descending aorta. Ninety-day and one-year survival rates were 87.5% and 75%, respectively. Two patients (…

The ossification of the vertebral column, thorax and sternum in the quail (Coturnix coturnix japonica).

The ossification centers onset of the quail vertebrae, ribs, and sternum in embryos and hatchling birds was studied. Specimens were cleared, stained with Alcian Blue and Alizarin Red S and examined using stereomicroscope. The chondral rudiments of the vertebrae were observed at the 6th day of incubation (E6). The osteogenesis of the vertebrae was accomplished with both perichondral and endochondral ossifications. The cervical vertebrae began to ossify at E9-E10, whereas the thoracic ones began at E10-E11. The synsacral vertebrae began to ossify at E11-E13. In the caudal vertebrae, ossification…

Sternal talon offers a solution for secondary sternum osteosynthesis in patients with nonunion.

BACKGROUND: Median sternotomy may be associated with postoperative complications such as nonunion after conventional metal wire closure. The Sternal Talon device (KLS Martin, Jacksonville, FL) has recently been introduced as an alternative for osteosynthesis after median sternotomy and may also be beneficial for patients with persistent sternal nonunion. METHODS: A consecutive series of 24 patients underwent Sternal Talon repair for sternal nonunion or acute mediastinitis, or both, after sternal wire closure. Patient data--including demographics, surgical history, and indication for operation

[Resistant arterial hypertension and a prominent sternum in a 77-year-old woman].

The case of a 77-year-old woman who was admitted with resistant arterial hypertension is reported. In view of a history of pheochromocytoma 2 years ago, catecholamine levels were examined and found to be elevated; in addition, MIBG scintigraphy showed a positive area in the anterior mediastinum. Computer tomography showed a tumor in the sternum. Histology confirmed metastasis from the pheochromocytoma, and the corpus was removed surgically. Currently, the patient is without any evidence of relapse.

Sclerotic multiple myeloma with an unusual sunburst periosteal reaction occurring in the sternum.

Multiple myeloma is a disseminated neoplastic monoclonal gammopathy that usually affects the skull, clavicle, rib, pelvis, spinal column, and proximal portions of the humerus and femur. The initial manifestation of multiple myeloma in the sternum is rare. The classic radiological presentations of multiple myeloma are multiple "punched-out" areas of bone destruction, expansile lytic lesions, and generalized osteoporosis. Primary sclerotic presentation is rare and occurs in only 3 % of cases. A sclerotic multiple myeloma with a sunburst periosteal reaction occurring in the sternum has not been r…

[Spontaneous sternum fracture in a pathologic area. Case report and literature review].

A male, 62-year-old patient presented with chest pain with no apparent cause. Imaging studies revealed solution of continuity of the bone tissue at the level of the sternum, which led to paradoxical respiration. Open reduction and fixation with an LCP plate and screws were performed to address the problem. An important part of the patient's history was lumbar pain treated conservatively with analgesics and muscle relaxants. The pain did not subside and X-rays were done with findings of osteolysis in several parts of the body. The patient had chest pain and respiratory distress and was referred…

The impact of intra-operative sternum vertical displacement on the sagittal curves of the spine.

Patient positioning is an important step in spinal surgeries. Many surgical frames allow for lumbar lordosis modulation due to lower limb displacement, however, they do not include a feature which can modulate thoracic kyphosis. A sternum vertical displacer (SVD) prototype has been developed which can increase a subject's thoracic kyphosis relative to the neutral prone position on a surgical frame. The kyphosis increase is obtained by lifting the subject's torso off the thoracic cushions with a dedicated sternum cushion that can be displaced vertically. The objective of this study was to evalu…
