Intraoperative intercostal nerve cryoablation vs. patient-controlled analgesia after thoracotomy: a single-center retrospective cohort study demonstrating comparable early pain and opioid use.

BACKGROUND AND AIMS: Thoracotomy is among the most painful surgical procedures and is associated with a high risk of chronic postoperative pain. Optimizing perioperative analgesia is essential for enhancing recovery and reducing opioid-related side effects. This study evaluated whether adding intraoperative intercostal nerve cryoablation to standard postoperative analgesia improves pain control and reduces opioid use following thoracotomy. MATERIALS AND METHODS: This retrospective cohort study included patients who underwent thoracotomy at a tertiary cancer center in 2024. Patients receiving

Safety and diagnostic yield of transbronchial lung cryobiopsy in patients with interstitial lung disease and severe pulmonary function impairment.

BACKGROUND: Transbronchial lung cryobiopsy (TBLC) has become an established diagnostic modality in interstitial lung disease (ILD). However, current guidelines frequently exclude patients with severely impaired lung function due to safety concerns. This study aimed to evaluate the safety and diagnostic yield of TBLC in patients with advanced pulmonary function impairment. METHODS: We conducted a retrospective analysis of consecutive patients with suspected ILD who underwent TBLC for diagnostic evaluation at Thoraxklinik Heidelberg, University Hospital Heidelberg, between November 2017 and Dec

The role of bronchoscopic cryoimmunotherapy in non-small cell lung cancer: current evidence and future perspectives.

Lung cancer is the leading cause of cancer deaths, and despite therapeutic advances, recurrence and resistance persist. Local tumor ablation can function as an in situ vaccine, but thermal techniques may disrupt antigen and extracellular matrix integrity, potentially limiting immunogenicity, whereas cryoablation has been shown to preserve tumor antigens and matrix architecture while inducing immunogenic cell death. Bronchoscopic cryoimmunotherapy (BCI) aims to prime antitumor immunity rather than achieve complete tumor eradication. We review preclinical and clinical studies evaluating cryoabla

Atrial fibrillation ablation in patients with type 2 diabetes mellitus: long-term follow-up study for atrial tachyarrhythmia recurrence and adverse cardiovascular events.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is a modifiable risk factor both for atrial fibrillation (AF) and for the development of cerebrovascular-events in patients with AF. The aim of our study was to determine the effect of cryoballoon ablation (CBA) treatment on long-term atrial-tachyarrhythmia recurrence and adverse cardiovascular (CV) events in paroxysmal AF patients with T2DM. METHODS: This retrospective-observational-cohort study included 748-patients (406-male,342-female,59.4 ± 11.8years) who underwent CBA with a diagnosis of paroxysmal AF between 2013-2023. Myocardial Injury and Incidence of Early Recurrence of Atrial Fibrillation after Catheter Ablation between Two Types of Cryoballoon Systems.

Two types of cryoballoon (CB) systems are currently available for catheter ablation of atrial fibrillation (AF). It is not clear how the difference between POLARx (Boston Scientific) and AFA-Pro (Medtronic) relates to myocardial injury and the incidence of early recurrence of fibrillation (ERAF).Patients (n = 137) who underwent catheter ablation for paroxysmal AF by 2 CB devices were included (AFA-Pro in 87; POLARx in 50). We assessed creatine kinase (CK)-MB pre and post-procedure, ERAF, and the number of atrial premature contractions (APCs) at Holter monitoring 1 month and 3 months after the

[Possibilities and Limitations of CT-guided Intervention].

CT-guided transthoracic lung biopsy (CT-TTNB) is an essential method for the diagnosis of pulmonary nodules and masses. With a sensitivity of 85-97% and a specificity of 85-100%, it offers high diagnostic accuracy. By using core-needle biopsies, high-quality tissue samples can be obtained that enable molecular analyses for personalised therapy. At the same time, lung ablation has established itself as a valuable minimally invasive therapy. Procedures such as radiofrequency ablation (RFA), microwave ablation (MWA) and cryoablation enable targeted tumour destruction, particularly in inoperable pa

Impact of alternative efficacy end point definitions on reported outcomes after ablation of paroxysmal atrial fibrillation - Insights from the COMPARE-CRYO study using continuous rhythm monitoring.

BACKGROUND: Recurrence after atrial fibrillation (AF) ablation is commonly defined as any atrial arrhythmia lasting ≥30 seconds. It is unclear how alternative efficacy end point definitions would change the observed success rates and how they would relate to patients' disease burden or health care utilization metrics. OBJECTIVE: This study aimed to evaluate the impact of alternative end point definitions after AF ablation based on AF-burden, duration or number of episodes using implantable cardiac monitors (ICM). Position change during hyperbaric oxygen therapy for arterial gas embolism.

Arterial gas embolism can be fatal and should be treated with care. Typically, the patient is placed in the supine position during treatment. However, we present a case where the patient's position was changed to facilitate treatment. A 78-year-old man with severely reduced heart function underwent cryoablation for chronic atrial fibrillation. During the procedure, he accidentally inhaled rapidly. Subsequently, he presented with stroke symptoms. Computed tomography (CT) revealed air in the brain and left ventricle, leading to a diagnosis of arterial gas embolism. He underwent hyperbaric oxygen

Additional diagnostic potential of transbronchial lung cryobiopsy in bronchoscopic assessment of sarcoidosis and chronic beryllium disease: a retrospective analysis of 321 patients.

INTRODUCTION: Non-necrotising granulomas are the histological hallmark of sarcoidosis and chronic beryllium disease (CBD). Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA), transbronchial lung forceps biopsy (TBLB) and endobronchial biopsy (EBB) are often used as standard methods for obtaining suitable tissue. Transbronchial lung cryobiopsy (TBLC) is a novel biopsy technique well studied in different fibrosing lung diseases; however, data on its use in sarcoidosis are scarce. This study aimed to assess whether TBLC provides additional diagnostic value compared with

Long-term outcomes of left atrial appendage isolation using cryoballoon in persistent atrial fibrillation.

AIMS: There is an increasing trend evaluating the role of non-pulmonary vein (PV) triggers to improve ablation outcomes in persistent atrial fibrillation (AF) as pulmonary vein isolation (PVI) strategy alone has modest outcomes. We investigated the long-term safety and efficacy of left atrial appendage isolation (LAAi) in addition to PVI using cryoballoon (CB) in persistent AF. METHODS AND RESULTS: In this multicentre retrospective analysis, we included a total of 193 persistent AF patients (mean age: 60 ± 11 years, 50.3% females) who underwent PVI and LAAi using CB.
