The free iliac crest and fibula flaps in vascularized oromandibular reconstruction: comparison and long-term evaluation.

BACKGROUND: A variety of free flaps have been successfully used for mandible reconstruction. This study compared the short- and long-term results of using the free iliac crest and fibula flaps. METHODS: We conducted a retrospective analysis of 117 patients who underwent mandibular reconstruction, 59 patients with iliac crest and 58 with free fibula. Accurate long-term functional assessment was possible in 31 cases in the iliac crest group and in 48 patients with fibular reconstruction. Anterior or combined anterolateral defects formed 72% and 64% in the iliac crest and fibula groups, respecti

Confrontation naming of familiar sounds and pictures by individuals with Down syndrome.

We investigated whether adolescents and young adults with Down syndrome experience difficulty in finding words to name familiar objects. In Experiment 1, IQ- and MA-matched individuals with Down syndrome and with mental retardation due to other causes orally identified familiar pictures and sounds. Results showed no evidence of syndrome-specific word-finding difficulty in accuracy or speed. Although both groups identified auditory stimuli less accurately and more slowly than visual stimuli, results of Experiments 2 and 3 confirmed that young children without mental retardation and college stud

The corpus callosum and cerebral speech lateralization.

In order to specify the callosal involvement in the establishment of cerebral lateralization, a dichotic listening task was administered to six subjects with congenital absence of the corpus callosum, two callosotomized patients, and two hemispherectomized patients. The acallosal subjects were also compared to six subjects matched for age, sex, and hand dominance as well as to six subjects also matched for IQ. Our findings indicate that language functions, as assessed by dichotic listening performance, are more strongly lateralized in callosal agenesis subjects than in IQ-matched normal contro

Beyond electronic health record data: leveraging natural language processing and machine learning to uncover cognitive insights from patient-nurse verbal communications.

BACKGROUND: Mild cognitive impairment and early-stage dementia significantly impact healthcare utilization and costs, yet more than half of affected patients remain underdiagnosed. This study leverages audio-recorded patient-nurse verbal communication in home healthcare settings to develop an artificial intelligence-based screening tool for early detection of cognitive decline. OBJECTIVE: To develop a speech processing algorithm using routine patient-nurse verbal communication and evaluate its performance when combined with electronic health record (EHR) data in detecting early signs of cogni

Evidence that an internal schema adapts swallowing to upper airway requirements.

KEY POINTS: To swallow food and liquid safely, airway protection is essential. Upward and forward movements of the hyoid and larynx in the neck during swallowing vary in magnitude between individuals. In healthy human adults, hyoid and laryngeal movements during swallowing were scaled by differences in initial upper airway area before swallowing. Individuals increased laryngeal elevation during swallowing in response to increased airway opening before swallowing. We show that when upper airway protection requirements change, individuals use an internal sensorimotor scaling system to adapt move

A Randomized, Controlled Trial of Behavioral Voice Therapy for Dysphonia Related to Prematurity of Birth.

OBJECTIVES: Dysphonia is a potential complication of prematurity. Preterm children may sustain iatrogenic laryngeal damage from medical intervention in the neonatal period, and further, adopt compensatory, maladaptive voicing behaviors. This pilot study aimed to evaluate the effects of a voice therapy protocol on voice quality in school-aged, very preterm (VP) children. METHODS: Twenty-seven VP children with dysphonia were randomized to an immediate intervention group (n = 7) or a delayed-intervention, waiting list control group (n = 14). Following analysis of these data, a secondary analysis

Categorical speech perception deficits distinguish language and reading impairments in children.

We examined categorical speech perception in school-age children with developmental dyslexia or Specific Language Impairment (SLI), compared to age-matched and younger controls. Stimuli consisted of synthetic speech tokens in which place of articulation varied from 'b' to 'd'. Children were tested on categorization, categorization in noise, and discrimination. Phonological awareness skills were also assessed to examine whether these correlated with speech perception measures. We observed similarly good baseline categorization rates across all groups; however, when noise was added, the SLI grou

Diagnosis and treatment of reading disabilities based on the component model of reading: an alternative to the discrepancy model of LD.

Currently, learning disabilities (LD) are diagnosed on the basis of the discrepancy between students' IQ and reading achievement scores. Students diagnosed with LD often receive remedial instruction in resource rooms. The available evidence suggests that the educational policy based on this discrepancy model has not yielded satisfactory results. This has led researchers to try other paradigms, such as the component model and response to intervention, for dealing with children with reading disabilities. The component model of reading (CMR) described in the present article identifies the reading

The role of non-auditory factors in measured and self-reported disability.

The effect of age per se on auditory disability in relation to other non-auditory factors, including personality and IQ, is unclear because of the close link with hearing threshold level. Auditory disability may be assessed in terms of either performance or self-report. It was measured using two tasks identifying words in sentences: (1) sentence identification in noise for spatially separated signal sources, and (2) identification of sensible and nonsensical sentences given at normal and artificially accelerated rates. Self-reported disability was assessed using the MRC Institute of Hearing Re

Communication abilities in children with hearing loss - views of parents and daycare professionals.

INTRODUCTION: Today, children with hearing loss (HL) are diagnosed and fitted with hearing devices at an early age. However, HL may still influence their communicative development. Thus, we need up-to-date research on how children perform in their everyday environments, such as at home or daycare. This study provides an overview of the communication abilities of early-diagnosed children with HL reported by parents and daycare professionals. The first aim of the study is to compare the results of children with bilateral hearing aids (BiHAs) or bilateral cochlear implants (BiCIs) with those of c

Adaptive trajectories and early risk factors in the autism spectrum: A 15-year prospective study.

Little is known about long-term outcomes. We investigate the adaptive trajectories and their risk factors in ASD. Data were obtained from 281 children prospectively followed untill adulthood. The final sample consisted of 106 individuals. Vineland scores were collected at baseline (T1), 3 (T2), 10 (T3), and 15 (T4) years later. A group-based method was used to identify homogeneous patterns of adaptive skills trajectories. Results show that among the children initially categorized as autistic, 82.6% remained over the ADOS diagnostic threshold, 11.9% converted to atypical autism, and 5.4% fell u

Free radial forearm flap reconstruction in surgery of the oral cavity and pharynx: surgical complications, impairment of speech and swallowing.

The first 100 consecutive free radial forearm flaps used in reconstruction for oral and pharyngeal cancer were investigated. Seven re-operations took place due to circulatory impairment of the flap. Two flaps underwent partial, and four total necrosis, one of those patients died of massive haemorrhage. Post-operative infection was significantly correlated to increased time between the last radiotherapy session and surgery. All but eight patients had post-operative impairment of swallowing and all but five patients had impairment of speech articulation or hypernasality. Post-operative time spen

Motor and cognitive profiles in children who are 3 to 6 years old and have autism spectrum disorder and other developmental disabilities.

IMPORTANCE: The debate on whether different motor and cognitive profiles of children with autism spectrum disorder (ASD) reflect distinct patterns of comorbidities with significant implications continues in autism research. OBJECTIVE: The aim of this study was to determine the motor and cognitive profiles of children with ASD alone as well as those with ASD and comorbid developmental disabilities. Additionally, it was assessed whether the cognitive abilities were linked to the motor performance of children with ASD. DESIGN: This study was a secondary analysis of a retrospective cohort. SETT

Are overreferrals on developmental screening tests really a problem?

BACKGROUND: Developmental screening tests, even those meeting standards for screening test accuracy, produce numerous false-positive results for 15% to 30% of children. This is thought to produce unnecessary referrals for diagnostic testing or special services and increase the cost of screening programs. OBJECTIVES: To explore whether children who pass screening tests differ in important ways from those who do not and to determine whether children overreferred for testing benefit from the scrutiny of diagnostic testing and treatment planning. METHODS: Subjects were a national sample of 512 p
